Employee was moving a box of bolts at the start of EE's shift in the maintenance shop. Box weight approx. 40lbs. Employee was moving from floor to shelf approx. 31" from the floor. Employee felt sharp pain lower left back area.
Employee was pulling a sample from Rotary Kiln 2. The sample window would not stay up. Employee opened the access door to pull the sample. When employee closed the door, they pinched right pinky finger between the latch and coal pipe support. This resulted in an open fracture and laceration to the tip of the right pinky finger.
On May 31, 2025 EE was working on-site at Magnesita Refractories Co. Working on the Coal Mill installing new plates, put pole jacks on half the plates, rotated 180 degrees, start setting the bottom in, when installing the bottom plates, the pole jack left loose, the plates fell down on EE, fell on EE's back
While performing clean-up duties, employee stepped on rock and turned EE's right ankle resulting in sprain.

Employee was replacing springs on cooler ports. Latch holding spring did not hold. It released causing right ring finger to get pinched between two pieces of metal resulting in fracture to finger.
Employee using skid steer bobcat to clean up Travelways behind our Grinding Plant building. The bucket of the bobcat caught old RR tracks that were buried under the mud/muck, causing the unit to come to an abrupt / Rough stop. The employee was thrown forward striking their head on the rearview mirror and bracket. The employee was wearing seatbelt (lap belt).
2 employees were removing a brick saw from operations. They were lifting saw onto a cart to transport to the dock area and place into the loader hopper. During the lift onto the cart, the saw slipped causing the injured employee to shift quickly and EE felt a pinch in their left shoulder.
Employees were cleaning out the 8026-elevator pit to clear it for maintenance work. 3 Employees involved in cleaning. The injured was using a dingo to push dirt off cement pad. IP switched directions to clean up a pile of dirt when IP became wedged between the dingo and the set of stairs.
Rotary employees were utilizing the ring removal tool when a shell became jammed. The operator waited approximately 10 minutes before trying to clear the jam. The employee was using a flathead screwdriver to attempt to clear the shell when it discharged causing the brass casing to burst. A piece of the brass caused a laceration the employees right bicep area.
EE was trying to remove shaft coupling using a 3 legged puller in a hydraulic jack. EE had pressure on jack while applying heat to shaft coupling, when a leg of the puller broke and recoiled, hitting EE in the left side of face.
Employee was using a push broom to sweep floor in building above Meal Bins. Employee felt tightness and pain in lower back.
3 employees working together to remove drive belts from out 20 screen in the Rotary Dept. Finish End building. 2 of the employees (1 being the injured) were using pry bars to apply upward pressure in order for employee 3 to remove the belts. During this time, the injured employee felt a pinch and burning sensation in the lower abdomen/groin area.
Cleaning up a stone spill from a surge bin overflow using a shovel. Employee felt a strain in EE's lower back.
EE was cutting a piece of wood with a circular saw. EE stated saw became lodged & a metal screw was under the board causing the wood to roll off the gang box. The EE tried to grab the wood, when EE did this, EE's right hand went under the piece of wood & came in contact with the saw blade.
EE was tightening a course of brick with a pneumatic porta power pump. EE had left hand on the pump to ensure pump was in place. EE stated pump had 2.500 PSI when incident occurred. While hand was on pump, the threads failed, causing EE's hand to be pinched.
Rotary Kiln employee was performing daily cleanup operations shoveling wet material from under the peck carrier and placing into buckets. Employee noticed pain/tightness in EE's back.
Employee emptied hot kiln feed material from rotary feed end recirculating chamber into dump hopper. Employee used fork truck to transport dump hopper to outside stall and dumped the hopper. Employee stepped into dumped hot material causing some hot material to spill into top of boot causing burn to left ankle.
Employee walking from break room back to work truck, slipped on ice and twisted back resulting in pain in lower back.
While driving fork truck over uneven ground to deliver pallets of bricks for a kiln re-line, employee experienced back pain from bumping around while seated in fork truck.
While removing side panels from a grain elevator, employee turned towards the elevator after setting panel down and struck left forehead (above safety glasses and below hard hat) on a side panel edge resulting in laceration to forehead.
No specific job task has been identified as causing the injury. Employee experienced pain in left forearm area while performing multiple job tasks. Physician diagnosed nerve issue initiating in the shoulder area. Employee has been prescribed physical therapy for both shoulders.

While working on a slide gate at 10 Bin, employee got debris in right eye causing scratch to eye.
Employee was completing preventative maintenance on a conveyor motor, when an unexpected start up occurred due to incorrect lockout, causing a motor shaft coupling to come loose and contact employee's right forearm resulting in a laceration and fracture.
While working on repairs on No. 3 Elevator in the drying plant, a piece of dirt/rust got in left eye. Rust was removed by ophthalmologist and given prescription eye drops.
Employee was using belt sander to clean an edge on a tool. The tool was grabbed by the sander causing the employee to hit pinky finger on tool rest causing laceration requiring stitches.

Maintenance employees were changing the tail pulley on #1549 conveyor in Feed End Building. The work involved removing the old pulley and installing the new pulley on the conveyor. The employee was kneeling, tightening the components of the conveyor. When the employee stood up, employee felt sharp pain in EE's back.
While operating the ring remover to remove a ring in Rotary Kiln #2, employee felt pain/strain in back and left side of torso.
Employee was attempting to walk through mud at grinding plant transfer building. The employee strained left knee when walking. This was initially reported as first aid on 02/23/2018, but was not placed on restricted duty until 03/09/2018.
Employee got dust in right eye causing corneal abrasion while loading scrap material from No.6 Storage Bin into loader bucket.
Strain to right forearm while moving wheel chock from under wheel of 514 Pay loader and placing it in its storage location on rear bumper.
While employee was shooting out an ash ring in rotary kiln, gun malfunctioned causing shell to misfire inside gun. Employee received puncture wounds and lacerations to left hand from pieces of metal from shell. Medical treatment was needed to remove some pieces of shell from employee's hand.
Employee had pain and discomfort in upper abdomen while performing shoveling clean-up duties in various areas of the mine site. Employee diagnosed with strain of muscle, fascia, and tendon of the abdomen.
Abrasion to cornea of right eye from foreign body falling behind safety glasses while walking from Meal Bin Tunnel to Meal Bin Office
Employee was lifting, carrying, pushing, pulling screens while replacing screens in the Grinding Plant (3rd Floor). Employee strained right side of lower back.
Employee sustained a sprained right knee from carrying pieces of steel up and down steps while performing repairs on RK#1 Coke Feed Conveyor. This was initially reported as a first aid. Employee requested to be seen by a physician. Physician placed work restrictions on employee on January 4, 2017, resulting in a reportable case.

Employee was unloading 80lb bags of concrete out of the back of pick-up truck onto hand cart and felt a pull/strain in right side groin area. This was initially reported as a first aid. This did not become a medical case until 08/17/2016.
Employee sustained laceration to right ring finger requiring stitches when finger was pinched between fan and framework while employee was helping another employee move a RK fan on a catwalk beside a rotary kiln.
Sustained a strain to right shoulder while brushing battery terminals after removing truck battery from piece of mobile equipment.
Employee was in a squatting position underneath Rotary Kiln #1, at Pier #5, picking up tools after maintenance job was completed. As employee was starting to stand up, employee felt popping sensation and pain in right knee.

Employee was driving front end loader. Loader ran over pot hole which caused the employee to bounce in his seat, jarring his back causing pain. The original event occurred on April 14, 2014. Employee did not seek medical treatment at the time. Medical treatment did not occur until April 29, 2014 at which time it became recordable.
Employee was cleaning floor in baghouse compartment. Employee was in a kneeling position. Due to limited space, employee used a shovel handle to assist in standing back up. The shovel slipped, causing the handle of the shovel to hit employee in the mouth, resulting in a chipped right front tooth.
Small puncture wound to right index finger from piece of metal while opening door to diesel fuel pump house.
Employee was removing a portable pump - While removing hose, employee stepped on a rock and twisted ankle, causing his hip to jamb, injuring his lower back.

Employee working from a kneeling position wiping down bearing housing. Employee rising up to reposition himself and experienced sharp pain in back. Employee had been working in a kneeling position for extended period of time over the past several days.
Employee was trouble shooting the operation of the Fuller Kenyon pump at the Grinding Plant.This is a dusty dirty environment. While standing and watching start up of pump, employee got foreign body in right eye. Employee tried to flush out the foreign body but needed medical assistance to remove foreign body. Employee was wearing safety glasses. Should have also used dust goggles.
Jammed stone into right knee while crawling under catwalk to install grease line on elevator bearing.
Slipped while climbing and hyper extended his left knee.
Small burn to elbow while welding overhead, spark went down back of his shirt and into his sleeve.The employee did not report it until 10/9/12 when it became red and swollen.
Slipped on mud and fell while walking causing bruised elbow and sore neck.

Employee had a maintenance vehicle up on hydraulic lift changing oil, ducked under framework of lift, hitting head on framework jamming his neck. Employee was wearing a hard hat.
Employee was installing side liners on a cooler. The location of nut and bolt fasteners in some locations placed the employee in awkward positions at times. Employee experienced pain in his neck as a result of doing this job task. Employee was diagnosed with a sprained neck.
Employee attempted to empty a wheelbarrow that was loaded off center with fines. When employee attempted to lift up on handles, the wheelbarrow began to tip, causing a strain of his low back. Employee reported immediately but did not request care until much later. Discomfort did not go away so employee requested to see a Chiropractor in March.
Employee was lifting 50 pound boxes of shells off a wooden pallet and dumping shells onto a wooden tray at chest height.
Torn bicep muscle on right arm from lifting a piece of piping
Experienced pain and stiffness in muscles of back while sitting in an awkward position when riding inside a loader while training a new loader operator.
Strained muscles in lower back while trying to realign dump hopper back onto its tracks after dumping a load of material.

Employee exited locker room area door and was proceeding to company truck when he slipped on ice, rolling his left ankle underneath himself.
During an MSHA site inspection, MSHA inspector climbed up onto mobile equipment hauler to talk to operator. Operator opened his driver's side door to talk to the inspector. After the conversation was complete, the inspector inadvertently shut the door on the operator's left hand, injuring his left index and middle fingers.
Cut and broken little finger; bruised ring finger left hand. Removing a faulty reverse air valve from #1 compartment on #1 baghouse dust collector, pinched two fingers on flanges when wood blocking shifted.

Dust blew into employee's left eye causing irritation. The Commercial Loadout screw was discharging lime onto the stockpile truck. The water spray system was replaced with a new system and it was being tested.
Employee fell down steps causing a fracture to his right arm & right heel.
While exiting a skidloader, rolled ankle while stepping off of bucket.

Employee was walking down steps and stepped on a hard lime ball causing employee to fall. Employee injured lower back and right hip area.
Employee strained back tying his shoes prior to the start of his work shift. Injury occurred in employee locker room.
Employee developed a back strain from ackward positioning during welding of 846 hauler. Employee was bent over for extended period of time. Equipment did not contribute to the injury.
Employee was operating an automatic welder inside of primary crusher and received Keratoconjunctivitis or welder flash in right eye.
Employee was removing ceramic tiles from a vibrating conveyor. He attempted to use his hand to brush debris out of his way and sliced the end of his right middle finger requiring 3 sutures!
Employee stepped backwards catching his heel on an uneven floor surface, falling backwards. He reached back to catch himself and broke his left wrist.
Employee experienced a lumbar strain while swinging open the access door to the primary crusher.
Employee was assigned the task of servicing the #1 kiln drive motor. When he stepped over the framework at the kiln drive, his foot slipped on lubricant. Employee pulled his lower back catching his balance.
Electrician was trouble shooting outdoor lighting problem at night. Unknowingly contacted poison ivy at base of lighting pole at lighting control box. Poison Ivy was sprayed to eliminate potential exposure in future. This was a remote location, employees not normally exposed.
Employee was using hammer to loosen material out of dust screw. Pinched left little finger between hammer handle and screw housing. Employee reported incident on first aid card but did not notify us that finger needed further medical attention. Safety Coordinator followed up with employee and sent for treatment.
Employee descending loader, slipped off of bottom rung of ladder due to mud on bottom step. Employee caught himself with right arm, resulting in 2 degree strain of right arm with right ulna nerve irritation.
Maintenance mechanic was changing cooler grates and experienced a thoracic back strain while tightening bolts with wrench.
Employee was installing rubber boot on cooler. Pipe fell from shelf above, striking left hand, fracturing left little finger. Injury did not interfere with employee's normal job duties.
Miner was loading a truck with material. Miner was closing the small door on the tailgate of a tractor/trailer. Miner smashed his finger between the handle of the small door and the tailgate of the trailer. Miner fractured and smashed his ring finger on his right hand. His finger required 6 sutures.
EE was using a sledge hammer, hitting the loader bucket to clear out snow. As he was swinging, his foot slipped and he pulled a muscle in his back.
EE AND CO-WORKER WERE MANUALLY HANDLING A 55 GAL DRUM USED AS A BASE FOR A FREE STANDING DIRECTION SIGN. DRUM FELL ONTO EE'S RIGHT FOOT CAUSING FRACTURE.
EMPLOYEE IS NOT SURE WHERE INJURY OCCURRED BUT WAS OPERATING A LOADER AND CLEANING UP AT BALL GRATE THAT DAY.

EMPLOYEE WAS WORKING PECK CARRIER WITH A WRENCH. EMPLOYEE WAS APPLYING PRESSURE AND FELT A PULL IN BACK. GOT UP NEXT MORNING AND WAS VERY SORE. EMPLOYEE SENT TO SEE A DOCTOR.

EE BENT OVER TO PICK UP A PIECE OF METAL STRAPPING AND STRAINED A BACK MUSCLE WHILE BENDING OVER. STRAIN NOT A RESULT OF LIFTING, JUST BENDING OVER. EE HAS A HISTORY OF BACK PROBLEMS. EE RECEI VING CHIROPRACTIC TREATMENT WITH NO RESTRICTED DUTY. WORKING THROUGH INJURY.
EE WAS USING AIR ARC TO CUT OFF STEEL CLIPS INSIDE #1 KILN. THIS IS A CONFINED WORKING AREA. HE WAS USING WELDING HOOD BUT WITH THE COMPRESSED AIR, THERE WAS A LOT OF AIR TURBULANCE. EE NOTICE D AFTER SHIFT A SPEC ON HIS LEFT EYE.
AFTER CHOKE UP OF A SCREW CONVEYOR, EE WAS SUMMONED TO TROUBLE SHOOT AND HELP WITH REPAIRS. BREAKER HEATER WAS KICED OUT SO EE INSTRUCTED OPERATOR TO RESET AND TRY SCREW AGAIN. MOTOR SWITCH WA S TURNED OFF BY OPERATOR. EE SWITCHED MOTOR SWITCHE BACK ON AND WAS STANDING CLOSE TO SEE IF BELTS WERE SLIPPING. WHEN OPERATOR TURNED ON BREAKER, MOTOR AND FRAMEWORK STRUCK EE ON FACE, NECK,
EMPLOYEE WAS MOVING THE TRIPPER OVER THE RAW FEED BINS. A LEVER ALLOWS THE TRIPPER TO MOVE, PULLING UP TO MOVE, PUSHING DOWN TO LOCK IN PLACE. A PIECE OF STEEL IS WIELDED ONTO THE TRIPPER FRAM E TO HOLD THE HANDLE UP. EMPLOYEE PINCHED LITTLEFINGER ON RIGHT HAND BETWEEN LEVER AND HOLDER. LACERATION REQUIRED 5 SUTURES.
EMPLOYEE WAS REPAIRING OVERHEAD LIGHTS THE DAY BEFORE ONSET OF PAIN. REACHING OVERHEAD TO REPAIR LIGHTS TUESDAY, 8/22/00. HE WOKE WEDNESDAY, 8/23 WITH PAIN. WIFE DROVE HIM TO WORK AND HE OB TAINED MEDICAL CARE; HE DIDN'T REPORT ANY INCIDENT TO HIS SUPERVISOR OR OUR SAFETY REPRESENTATIVE.
EMPLOYEE WAS REMOVING BOLTS FROM A DIVERTER FLANGE, USING A WRENCH, EMPLOYEE STRAINED LEFT ARM MUSCLE WHILE PULLING ON WRENCH TO LOOSEN BOLTS.
EE WAS REPLACING A SCREEN CLOTH ON SCREEN. EE WAS WEARING GLOVES. AS EE WAS LOWERING DECK INTO SCREEN FRAME, GLOVE CAUGHT SCREEN AND WAS PINCHED BETWEEN SCREEN DECK AND FRAME CAUSING SIMPLE FR ACTURE OF RIGHT INDEX FINGER. SPLINT ON FINGER, ABLE TO PERFORM NORMAL JOB DUTIES.
EE OPERATING BRODERSON 15 TON CRANE WITH 14 - 51 FOOT BOOM. OPERATOR RETRACTING OUTRIGGERS, RETRACTING BOOM AND RAISING BALL HOOK AT SAME TIME. BALL STRUCK STOP BLOCK BUT CONTINUED OVER BLOCK, BOUNCING OFF BOOM AND FALLING THROUGH GLASS TOP OF OPERATOR'S CAB STRIKING EE ON HEAD. EE WAS WEARING HARD HAT. STOP BLOCK SAFETY SWITCH CONFIRMED TO BE FUNCTIONAL ONE DAY PRIOR TO MISHAP.
EMPLOYEE WAS CUTTING A RUBBER BELT TO FIT IN 03 SCREEN HOPPER FOR SPLILLAGE PREVENTION. WHILE CUTTING BELT, KNIFE SLIPPED AND EMPLOYEE CUT INTO RIGHT LEG, REQUIRING SUTURES. UNSAFE ACT OF CUTT ING TOWARDS SELF. CORRECTIVE ACTION TAKEN TO PREVENT REOCCURRENCE WITH COUNSELING.
EE WAS MOVING AN ACCESS DOOR FROM A MATERIAL ELEVATOR. AS HE WAS REMOVING THE LAST CLAMP ON THE DOOR, IT FELL STRIKING HIM ON THE RIGHT FOOT JUST BEHIND THE STEEL TOE OF HIS BOOT RESULTING IN DOUBLE METATARSAL FRACTURE.
EE WAS SETTING UP AN AUTOMATIC WELDER. AS HE WAS REPOSTIONING THE WAND ASSEMBLY, THE ELECTRODE TOUCHED THE SIDE WALL OF THE CRUSHER AND FLASHED. EE SUFFERED A WELD FLASH OF BOTH EYES. EE MUST DE-ENERGIZE WELDER PRIOR TO ADJUSTING WAND.
