American Asphalt - CHASE QUARRY Metal/Non-Metal

Employee was tightening drive belts on crusher using a 3/4ton come-along connected to a 1"x4'nylon strap rated for 6,400lbs. Strap showed no wear prior to use. The strap broke in the middle causing the come-along to go towards the employee and hit right hand. Ice was applied and employee was taken to a medical clinic. X-rays showed that the right thumb was broken.

Slipped climbing down ladder, contusion and sprain right knee.

Working on excavator hydraulic line released pressure and struck EE in left cheek and fluid sprayed in left eye.

Moving screens from rack. Screen fell lacerating right hand & puncturing left hand.

Slipped climbing down ladder and fractured left thumb.

Watering roads in quarry, dust or dirt in left eye.

Employee was cleaning mud from tracks of excavator and became lightheaded, short of breath and had chest pains.

Cutting with a torch, metal got in his eye. He was wearing welding hood and safety glasses.

Cutting skirting, knife slipped, cut right thigh. Stitches required.

Exiting loader, caught foot in loader step under cab.

EE was hitting air gate with hammer missed the gate and hit the top of his left hand.

EE was cleaning crusher with grinder, dust blew back in his face, into his left eye.

EE was stepping down off work deck and twisted his right ankle.

Pushing rock down chute to crusher. ROck fell and struck his right hand finger.

Replacing drive belt, right thumb got stuck under the belt.

EE was lifting a gas can off of a truck and felt something pop in his back.

Cutting conveyor belt, knife slipped, cut palm of left hand.

Slipped on ice and fell.

Employee was dismounting excavator, slipped and twisted left knee.

PULLED A GATE ON THE CRUSHER. WHEN HE DID HE HEARD A POP IN HIS RIGHT ARM.

CLIMBING ON SCREEN DECK FOOT SLIPPED OFF DECK, CAUGHT HIMSELF WITH RIGHT HAND, TWISTING RIGHT WRIST.

CUTTING NEW SKIRTING FOR CONVEYER, CUT HIS RIGHT HAND THUMB.

CLMT WAS FUELING DOZER - HE SLIPPED - FELL TO GROUND - TWISTED RIGHT KNEE.

PICKED UP WATER VALVE THAT WEIGHTED 140 LBS AND STRAINED HIS BACK, ARM AND LEGS.

WALKING ON SCREEN DECK CATWALK HE SLIPPED & HIT HIS KNEE.

WALKING BEHIND WELDER, EE HIT RIGHT ELBOW ON VISE ATTACHED TO WELDING TRUCK

DUMPING SAND IN SAND PILE, BERM GAVE WAY DUE TO HEAVY RAIN, TRUCK ROLLED OVER LANDING ON ITS ROOF.

FELL FROM THE TOP STEP OF LOADER (HE MISSED THE FIRST STEP).
