Operator strained muscles in the right side of neck while pulling a water hose, resulting in Physical Therapy. Employer was only notified of injury as of 8/31/17.

Injured EE and co-worker were cleaning up the plant's scrap metal and loading it into a skid steer. Co-worker pushed the scrap metal towards EE and his gloved right middle finger was smashed between the metal of the scrap and the metal skid steer bucket.
