Operator strained muscles in the right side of neck while pulling a water hose, resulting in Physical Therapy. Employer was only notified of injury as of 8/31/17.

Injured EE and co-worker were cleaning up the plant's scrap metal and loading it into a skid steer. Co-worker pushed the scrap metal towards EE and his gloved right middle finger was smashed between the metal of the scrap and the metal skid steer bucket.

EMPLOYEE WAS USING BAR TO PUSH ROCKS DOWN JAW CHUTE. ROCK HIT THE BAR CAUSING THE BAR TO SLIP OUT OF HIS HAND AND HITTING HAND AGAINST JAW CHUTE.
