While Employee 1 held the spring-loaded latch in a raised position, the tire bar slipped as Employee 2 positioned the new roller, creating a pinch point that caught their left thumb between the roller and latch.

On November 20, 2018 employee was changing the hydraulic fluid on the Komatsu WA 450-6 loader. EE was standing on the rear fender. EE slipped off the fender when EE backed up too far and wasn't paying attention. EE landed on feet, jarring lower back.
