On 7/10, EE felt soreness in right shoulder when using a 2# hammer. EE then used a 20# hammer & felt more soreness. On 7/18, EE reported injury and requested to be seen by DR. EE was diagnosed with shoulder pain and returned to work to regular position. On 9/14, EE requested to be seen by ER for shoulder pain. EE was released to return to work on 9/17 with restrictions.

Employee was removing a stick cylinder from an excavator, hydraulic pressure caused pressure release getting employee sprayed with hydraulic fluid on the right forearm.

EE was replacing impeller bars in Crusher. While exiting crusher, the wedge locking the rotor in place came loose. The rotor appears to have been out of balance due to a missing set of impeller bars. The unbalanced rotor begun to spin & the EE's boot caught between the wall & the rotor pulling him downward. EE suffered left leg lacerations & two crushed toes on right foot.

Employee was dismounting a 980G Loader and did not notice his left shoe lace was untied. Shoelace got hung up on the 2nd step from bottom and employee fell landing on his lower back and rolled up onto his shoulder.

Welder and a helper were assigned to weld patches over holes in the possumbelly of the SE shaker. The helper was holding the patches in place as they were tacked. Helper experienced flashburn as a result.

DRILLER WAS PUTTING A NEW SAVON SUB ON THE ROTERY HEAD. WHILE TRYING TO TIGHTEN THE SUB INTO THE HEAD, DRILLER WAS HOLDING BREAKOUT WRENCH (48" WRENCH) ON THE SUB AND RUNNING CONTROLS WITH HIS OTHER HAND. WHEN HE HIT THE ROTATION LEVER THE WRENCH KICKED BACK AND SMASHED HIS LEFT INDEX FINGER BETWEEN THE WRENCH AND THE MAST.

1 1/8" WRENCH WAS USED TO UNLOOSEN A BOLT ON SNUBBER PIN TO REPLACE RUBBER BUSHING. WHILE PULLING UP ON THE WRENCH TO BREAK IT LOOSE EMPLOYEE FELT A POP IN HIS BACK. ALTHOUGH HE WAS INJURED ON 1/4/03 IT DID NOT BECOME RECORDABLE UNTIL 1/16/03 WHEN THE EMPLOYEE REQUESTED TO GO THE CLINIC.
