Kiln #1 baghouse ductwork unplanned ignition. The pressure resulting caused damage.

While the employee was climbing down a fixed ladder EE slipped on the 3rd rung from the bottom and landed awkwardly on EE's left ankle.

The employee was using a knife to cut a hose (not the company approved safety blade). While cutting the hose the employee cut EE's left forearm.

EE was attempting to unplug a triple gate at the coal mill with no success. EE then introduced EE's hand into the triple gate. The gate activated, nipping the tip of EE's right middle finger.

Employee was working on housekeeping around the plant. While working with the vac-truck operator employee was giving hand signals to the driver during the backing up process. While doing this their feet slipped and they began to fall. during this process their right leg became entangled underneath them and they broke their leg.

WHILE ATTEMPTING TO MOVE A VALVE HANDLE THE TOOL SLIPPED AND STRUCK THEM IN THE CHIN

EE was walking from EE's office to the front break room for the morning meeting. EE heard some voices from the direction of the break room. EE turned EE's head to see who was speaking. At this time, EE's foot went into a depression in the walkway, causing EE's knee to twist awkwardly. EE felt something pop in EE's knee and immediately began to feel pain

While walking across the parking lot, wind blew material into the miners eye.

While walking they slipped on ice and bumped a knee.

While working on 3FM starter, employee had to get down on their knees and lay down to work on the contacts on the side of the starter approximately 10" above grade and 10"-12" inside the cabinet behind a bracket. During normal activity, when returning to a standing position, they lurched and had momentary difficulty standing, they then were fine

EE was installing new dust collector bags in one of the old dust collectors. In order to properly install, EE had to push upwards above their head and twist at the same time. While doing this EE experienced a sharp pain in the middle of EE's back. The employee visited Urgent Care. The doctor gave EE restrictions.

AT 6:15 THE #2 COAL BELT WAS STAGED DOWN. AT 7:15 THE PRODUCTION SUPERVISOR SAW SMOKE COMING FROM THE NORTH SIDE OF THE PLANT. AT 7:25 THE PRYOR FIRE DEPARTMENT WAS CALLED. DURING THIS TIME A HEAD COUNT WAS CONDUCTED. AT 7:45 THE FIRE DEPARTMENT ARRIVED ONSITE. AT 8:00 THE FIRE WAS PUT OUT. AT 8:39 MSHA WAS CALLED TO REPORT THE FIRE.

An employee was in the process of moving some fairly large steel cooler parts (beams) from the forklift forks onto some grating. In the process initial indications are that the beam slid, and EE's finger got caught between two solid objects causing the injury. Cut finger and fracture to finger tip.

WHILE EMPLOYEE WAS REINSTALLING THE LIDS ON A SCREW, THE BUELLS ABOVE LET GO AND DUMPED HOT DUST WHICH SPLASHED ON THE EMPLOYEE. THIS KNOCKED EE DOWN, AND EE CRAWLED OUT OF THE AREA. EE RECIEVED 3RD DEGREE BURNS ON SEVERAL PLACES ON EE'S BODY.

While checking the torch on K1, employee noticed an air lance inserted into the cyclone on K2 hood and when walking to check why it was there the material blew out of the pipe and onto their arm and face.

While blowing down an air slide, Miner was not wearing proper PPE and material blew back into the Miner's face causing a right eye injury.

EMPLOYEE HAD HOOKED SWITCHER AND RAILCAR GLADHANDS TOGETHER. WHEN THE EMPLOYEE OPENED THE BALL VALVE TO APPLY AIR TO THE CAR, THE GLADHANDS BLEW APART. ONE OF THE GLADHANDS STRUCK THE EMPLOYEE IN THE KNEE. THE EMPLOYEE CONTINUED WORKING UNTIL THE PAIN GOT WORSE AND WORKMANS COMP. ALLOWED SURGERY. EE HAD NOT BEEN ON LIGHT DUTY AS EE DIDNT FEEL EE NEEDED IT.

Carpal Tunnel due to cumulative illness.

While removing an inspection door, (knocking a bolt loose with a shovel), the door became dislodged and fell on EE's right foot behind the steel toe breaking the big toe in 3 places.

Employee worked a week and found on the 10th that something was wrong and was taken to the company doctor. Striking a sprocket with an iron hammer and not a dead blow or brass hammer. Metal splinter split off and went into left cheek.

While operating a standup skid loader, the miner was cleaning the basement of the crusher and backed into a column. EE's foot was not inside the skid loader therefore mashing EE's foot. EE was diagnosed with a strained ankle and put on light duty for two days.

Was feeding a steel strap into a rolling machine and the glove on left hand was caught by the roller and pulled in. The ring finger on left hand was crushed up to the first joint and removed by Doctor at the first joint.

While installing guard on drag chain sprocket employee was stomping the guard into place and foot went through the guard causing a contusion on the right shin.

Miner was backing up in an electric cart and had right foot outside of cart. While looking over right shoulder miner made contact with a ballard and smashed foot between the ballard and the front cowl right behind the steel toe of boot. After taking xrays the tech could not tell for sure if there was an issue.

While moving a forklift to a different location the forklift tipped toward the vacuum truck and employee's hand was caught between the ROPS on the forklift and the vacuum truck. Left hand was severely smashed.

EMPLOYEE WAS CARRYING DOWN A RIVET GUN AND A CHIPPING GUN FROM SECOND FLOOR, THE AIR HOSE SLIPPED OUT OF EE'S HANDS AND EE TRIPPED OVER THE HOSE CAUSING EE TO STUMBLE. NOW EE'S KNEE IS SWOLLEN AND TIGHT LIKE EEE STRAINED IT.

The employee was guiding a 400-500# sprocket to the landing area to be picked up by a crane. While rolling the sprocket along the catwalk it began to tip over and the contractor that was helping roll the sprocket said it was tipping and EE couldn't hold it. The employee was trying to get out of the way and the sprocket caught EE's shirt pulling EE to the floor under the sprocket.

ACCUMULATIVE

While stepping off the switcher to disengage the rail car the employee slipped on some ice/snow buildup on the rail and to the side of the rail.

Miner was under feeder weight belt holding it up as it was being replaced when material fell in miner's face. As miner jerked head to the side material went in eye

MINER WORKED ON AN AIRSLIDE FOR 2.5-3 HOURS UNDER OUR FINISH SILO'S AND DID NOT TAKE A BREAK TO COOL OFF OR GET SOME WATER. OUTSIDE TEMPERATURE WAS WELL OVER 100 DEGREES FAHRENHEIT.

The miner had worked 11 hours of EE's 12 shift and had been hydrating well with water and several Gatorade's and a Pedialyte during the day. The heat index that day was around 110 degrees F. It was a very busy day and was overcome with heat exhaustion at the end of EE's shift.

EE was welding on #2 raw mill todd heater draft line when EE experienced heat exhaustion.

EE was walking from the lab back to the control room. EE said: turned the corner from the lab, slipped and fell on right side. EE felt some pain and soreness in upper right arm/shoulder area. The floor was dry and nothing was on the floor.

Employee was operating a vac hose when left arm was sucked into the hose. The arm was swollen from the blood being pulled to the hand.

Employee was pressure washing a loader and some material went around employee's safety glasses and got in eye.

Employee slipped and fell on an icy sidewalk in February. EE continued to have pain and was sent for an MRI. It revealed a fractured scapula on 3-16-21. The follow-up doctor visit has resulted in work restrictions.

Employee was using a cutting torch in the mill. A piece of slag went under EE's face shield and over EE's safety glasses and got into EE's left eye. ER doctor said there was evidence of a burn and gave prescription numbing drops.

Employee had been lifting, pushing and handling mill liners during a repair job. EE's back felt sore so EE reported the injury but continued to work. One week later it started hurting again. EE was sent to the doctor and placed on restrictions for a back strain. Placed on restrictions after completing the 10/28/2020 shift.

Employee stated bumped right knee while working on the screen at the Secondary Crushing system. Employee was diagnosed with a fracture on 11-6-2020 and placed on restrictions on 11-9-2020.

An employee fell while descending a ladder. Employee misjudged the length of the last step to the ground and lost balance. Employee fell backwards into a pump motor and fractured their tailbone.

Employee was cleaning a plugged material line. Wind blew some of the material around glasses and into left eye. A prescription was written making this a reportable incident.

Employee felt a sharp pain in EE's lower back when attempting to lift a full bottle of acetylene onto a welding skid by self.

Employee slipped on wet pavement while hurrying through the rain. Employee stated knee popped as employee caught self from falling. Worked through the pain for a couple of months then knee popped again while carrying conduit. Required meniscus surgery on 12/3/2020.

Employee was struck in the hand while loading mill balls into the #3 Finish Mill. EE fractured EE's left index finger.

Employee was driving a key into a keyway for a coupling and shaft when they did not hit the key squarely. Employee's hand hit the coupling and fractured the tip of a finger.

Employee climbed down a ladder to access the crane in the Mill building. Employee felt a twinge in their back as they stepped from the ladder to the floor of the crane cab. Placed on lifting restrictions.

As employee was cleaning the shop EE rolled a crusher hammer onto a pallet to get it off the floor. The hammer shifted to the side and pinched EE's left ring finger between two hammers causing a fracture. EE also tore away the skin on the bottom of the fingertip.

The employee was using a jackhammer to attempt to free another jackhammer that was stuck in a pile of material. EE's right ring finger was pinched between the two jackhammers. EE fractured the tip of the finger when it was smashed between the them.

Employee's foot slipped from the bottom rung of a ladder and EE fell against the rail of the ladder bruising EE's ribs, shoulder and neck. Employee continued to complain of pain. On 4-28-20 a doctor placed EE on a 28 day lifting restriction of 25 pounds. EE will also have therapy sessions for the next 4 weeks.

An employee was throwing a breaker when it arc flashed. Employee received minor burns to face and hands.

Employee was busting loose old clinker from overhead steel structure with a pry bar from an aerial lift. Employee pinched finger between the bar & the aerial lift top rail.

Employee was carrying a roll of fiber optic cable to a worksite. While walking on uneven ground employee twisted knee. Employee worked with a knee brace and no restrictions until surgery on 5-30-2019.

An employee burned hand when ee attempted to tighten a hot radiator cap on a skid steer loader.

Employee tripped on bottom step and landed on floor grating. EE scraped EE's knee. It continued to hurt and EE was placed on restrictions on 1/29/19.

Employee dropped a sprocket when removing it from a shaft. The left little finger was pinched between the sprocket and the previously removed guard that was laying on the floor.

Employee was stepping from stairs to the landing and stepped onto a ball of clinker, spraining their right ankle.

While trying to remove blockage and improve product flow an employee reached up and inserted hand into an energized rotary feeder. It resulted in the partial amputation of three fingers of the right hand.

While cleaning a cooler tipping gate, an air cannon discharged and blew dust into EE's left eye. Foreign material was removed and a prescription was filled. Employee was wearing safety glasses.

Employee was unplugging wet buildup at the Kiln 3 #16 dust screw. EE received alkali burns to EE's hands.

Employee stated ee stepped off a ladder with weight on right leg. EE foot slipped and then grabbed and ee felt the top of calf pop.

Employee received restrictions due to sustaining 2nd degree burns from hot material contacting right shin. EE poked material that had plugged a chute and it came out towards ee rather than staying in the chute.

An employee strained a knee while moving a training dummy during a first aid training scenario. While moving the dummy to the ground employee felt pain in left knee. Surgery was required on 7-13-18 which made the injury reportable.

Employee strained back while shoveling a pile of loose material into the bucket of a skid loader in front of FM3

Employee experienced muscle cramps while working in hot temperatures. Employee received 3 bags of fluid through an IV at the local hospital.

The employee dropped a bar that is used to secure the mill door closed on the #2 Raw Mill. As the employee attempted to catch the bar it smashed the employee's finger between the bar and a liner bolt. Employee lost finger nail on right middle finger. Employee was placed on restrictions for the right hand.

Recordable hearing loss discovered during annual testing.

EE was exiting bobcat and stepped and turned to a locker raising their head up, right cheek came in contact with a stored 2x6 and damaged a tooth. The dentist pulled the tooth and placed the EE on restrictions for 2 days. PPE was not a factor. This is a new EE and EE is very tall. The stored lumber was always previously considered "overhead" and out of the way.

Employee required stitches after contacting an overhead beam. Employee was also placed on a lifting restriction for one shift.

First day of restricted duty 5/30/17. Employee walked outside of east door of compressor room and twisted left knee. There was wet material on the concrete from last nights rain.

Employee received foreign material to eye while standing under a fender of the water truck and using a hammer to tap on a socket in preparation of removing the fender.

Employee was under a front end loader and reached to install a bolt in the starter. Dirt fell down into the eye of the employee. EE received a scratch to the cornea and a prescription was necessary.

Employee was dressing up a piece of metal when the hand held grinder kicked back and struck employee in the lower lip requiring 7 stitches.

Employee received a burn to left eye from the welding slag while sitting beside a coworker. Received a prescription.

Employee was moving a sheet of 3/4" plywood. It hit some build-up on the floor and stopped moving. The employee felt pain in shoulder. *The injury occurred 2/14/17 but wasn't reportable until the EE started missing work last week to have surgery on the shoulder.

Employee was climbing down the ladder of a 988 loader while doing pre-shift examination. Employee failed to maintain 3 points of contact and fell 3 or 4 feet to the ground, landing on tail bone and spraining right wrist.

Employee stated left hip popped a couple of times while attempting to remove foot from a grease pit.

Employee felt pain in right knee while shoveling coal.

Employee strained right shoulder while pulling on a chain fall.

Employee mis-stepped as EE was crossing over a beam. EE's foot came down partially on the ground and partially on an access door, twisting ankle.

Employee felt discomfort in back while lifting on the lever to uncouple a rail car. Employee's first visit to the doctor was on 10/5/16 and the doctor took EE off work for 2 days.

Employee felt shoulder pop while handling warehouse material. First day of lost or restricted duty was 09/21/2017 due to surgery.

Employee overheated while working at the #2 cooler

Employee was cleaning around Kiln 3 burner floor and overheated.

Employee slipped and fell to the floor, landing on radio which was in chest pocket. The employee has bruised ribs.

Employee experienced muscle cramps while working in hot temperatures. Employee received 1 bag of fluid through an IV at the local hospital.

Driver was climbing up ladder on side of trailer. Driver's hand slipped off rung of ladder, losing three points of contact and fell from ladder to the ground. Fibula was fractured.

Employee overheated while cleaning the elevator pit.

Employee received a small laceration to right palm when a compartment lid closed unexpectedly while working on a loader. Employee received 4 stitches.

Contractor was rolling #2 mill, subcontractor was hooking cable to bolt on to manually turn mill. Mill was backspinning & cable got stuck. Subcontractor attempted to slide cable back on to bolt, while trying to prevent backspin of mill, when cable tightened and pinched subcontractors left hand/fingers between cable & mill.

First missed shift of work was 4-20-2016. A step broke as the employee was getting off the loader and EE jarred knee.

An IMI employee was helping to dump mill balls when his finger was caught between a full barrel of mill balls and the guard rail.

Employee stated that he slipped going down stairs towards Raw Mill #2 feed belts. He caught himself before he fell. He felt some pain in his lower back. Employee began physical therapy on 1/20/2016.

Employee became dehydrated after a long hot night of work.

While working to clean a small pile of material in a dust hopper the employee was covered by previously unseen hot material from the bags above. The employee has had skin grafts to both legs for burns.

Employee was standing in front of a door to a hopper. Material fell from the top of the hopper and came out the door. The employee turned his back to the material and it covered his back. The harness being worn by the employee held the material in place long enough for it to burn his back.

Employee was cutting abandoned overhead pipe. He started cutting the pipe and it came down and knocked the ladder out from under him and he fell to the floor.

Employee was assisting in loading a trailer with scrap metal. The scrap was being lifted by crane. A piece of scrap got caught on nearby 10" I-beam. This lifted the I-beam off the ground. When the I-beam fell out landed on the employee's ankle.

Employee felt discomfort while using a drag/rake to clean dust and chunks from the floor of the mill building.

Mud fell from the hose of a cutting torch that was being positioned for work at the primary crusher. The mud went around the employee's side shields and into his right eye.

Prescription for eye medicine. After receiving dust in his eye, the employee was able to flush out the eye thoroughly but was still sent to the doctor for verification of removal of the object. A prescription was written for antibiotic drops.

Employee stated he stepped on a pipe and turned his ankle.

While driving a water truck with the window down the employee got something in his eye. The doctor prescribed medicine for the irritation. He returned to duty.

Employee tripped over wire left by contractors by the East rail stairs

Employee stated he slipped on ice in the parking prior to the beginning of his shift.

Employee working on #3 kiln clinker breaker hammers, wind gusting in the door & blew foreign particle into right eye.

Employee installing 6" steel transport line for #2 FM FK pump. While welding line in place, employee had to lift off welding hood to inspect work. While inspecting the weld, a piece of slag popped off weld going around safety glasses & into left eye.

Employee stated he received a static electric shock from a vac hose while moving it which caused him to jerk away from the hose. He felt a pop in his shoulder.

Employee noticed injury to his eye when he woke up the morning of Thursday 11/21/13. He was unaware of any incident that lead to this injury. When notified safety officer and foreman noticed a spot on his right eye. He was driven to the Mayes County Medical Center for further examination. He was then sent to Claremore-Pryor Eye Clinic and given antibiotic eye drops.

Employee was on top of the railroad car with his harness intact and roped. He lifted the pole into the air, at that point the power line arced a discharge towards his pole. He stood straight up and fell off the railcar. Received 3 burns, chest, right arm, left leg. The electricity exited his body through his left ankle.

Second surgery on a carpal tunnel claim. Employee was a maintenance man for over 20 year and was suffering from pain and numbness in his hand.

Employee states that he slipped on some clinker which caused him to sprain his left knee.

Employee felt discomfort in his shoulder while pulling on a chain hoist. This current surgery is a second surgery related to this 2013 accident. Employee has been working with a full medical release until the date of this current surgery.

Employee strained his back while carrying five-gallon buckets containing samples of cement.

Employee walked 139 steps up and down about 7 to 9 times in one day. Employee reported the pain on 5/18/13. Details from workers comp report show repetitive motion injury. Symptoms and treatment follow bursitis type inflammation. Doctor notes unavailable at this time.

Employee placed himself between a forklift and an unstable load that was being lifted. Suffered 3 broken ribs and a bruised lung.

A tire blew out on a 988 loader sending loose rock and dirt flying. The debris struck the door window of the haul truck being loaded. The window shattered sending glass dust onto the haul truck operator. The operator was wearing safety glasses at the time. Employee has a small cut on eye and about a dozen small lacerations on body/face.

Employee was removing a fan impeller in limited space. When the impeller came loose the employee smashed his left ring finger. A fracture was found at the tip of the finger. The finger tip also had a cut and a band aid was placed on it.

Employee slipped on wet stairs. He stated he landed on a chain fall that he was carrying and hurt his hip and back.

An employee was climbing down a ladder when she got in a hurry and slipped. She slid down the ladder landing on both feet and collapsed backwards. She struck her head causing a laceration that required 6 stitches.

employee claims that he tripped over vac-hose and fell on his hands. He fractured his right wrist in two places

Delivery driver was closing doors on truck when a wind gust knocked him down. Driver was knocked out for 2 to 5 minutes.

Employee was lifting a 500 pound cradle when a lifting point that had been welded to the #2 finish mill broke. The cradle fell in an arcing motion and struck the left hand. It chipped two fingers at the knuckle and caused a tendon in the middle finger to tear loose.

Employee was cleaning out a mud ring from the #3 kiln. A section broke free forcing him into a crouching position, injuring his right knee and ankle. 1st visit to E.R. showed nothing. No signs of improvement at follow up visit. 3rd visit showed a little improvement, referred to a specialist. Physical therapy and restricted duty have been prescribed as of 7/13/2011.

Employee claims he tripped over his own feet. During the fall he claims that he tried to grab a door frame to catch himself. He continued to fall which caused his right arm to hyperextend and pull his bicep muscle. He was seen on 4/18/11 by an Ortho who diagnosed a bicep tear.

Employee was carrying a 5 ton chain hoist (approximately 50 pounds)downstairs. He was not using the handrail and was stepping in a sideways motion to "baby" his left knee that was injured previously. Initial doctor visit and x -ray on 3/17/11. Follow up and referral to second doctor on 3/28/11. MRI results on 4/12/11 of a tear in the acl.

Employee claims that while lifting and transferring 50 pound items to another person he felt a pop in his left shoulder. Reported on 1/14/11 to supervisor but no doctor visit. 3/4/11 seen by doctor, heal with rest. 7/8/11 follows up, persistent popping. 8/11/11 seen by Ortho without reporting. 8/16/11 discussed proper procedure for doctor visits, first notice of physical therapy.

A bar used to hold a door on the #3 finish KVS mill was being removed. The bar weighing about 50 pounds, slipped from the grip of the left hand and smashed the tip of the right middle finger, causing a split in the skin.

Employee sought medical treatment before notifing the employer that he was having problems with his wrist. Employee claims he has numbness and tingling in both his wrists. No defining moment of injury was ever reported.

employees alleges while trying to lift vac hose overhead to be placed on top of a classifier, he felt a pull in his right shoulder.

While lifting an overhead style gargae door measuring about 12' wide, the employee heard and felt a pop and experienced a sharp pain in his left shoulder.

Employee was in a bent over postion and using a jerking motion to try and move forks on a forklift.

Employee was complaining of ringing and hearing loss.

Employee was changing a brush on chain temperature thermocouple on #3 Kiln. Hand was caught between spring loaded mechanism. Broke right index finger.

Employee was assembling a set of shelves. Another employee dropped a crossmember which rebound from the floor striking him in the mouth.

Employee alleges he was working on #3 Finish Mill gyp feeder and strained his back while swinging a sledge hammer to break loose material.

Employee was cutting insulation using a utility knife when the knife slipped and the blade cut part of his left thumb.

Employee claims he used a cheater bar to work on a 992 loader. The bolt suddenly loosened causing all of the employees weight to shift to his left knee. The employee now (10/12/2010) is having his knee replaced.

Employee alleges that he strained his stomach while pushing on an elevator counterweight. Originally was seen at a local emergency room and diagnosed with strain and returned to work.

EE was in compartment 1 of the baghouse working on the floor on his knees snapping bags into the tubesheet, when his back gave out and he fell forwards onto his right side.

Individual had intermittent pain in right thumb and continuous tingling sensation in hand. Pain is now constant in the thumb. His right hand is dominant and in the operation of the crane it is used more frequently to move material where the left hand is used to control the bridge & trolley.

Injury was out checking the kiln and realized that he had something irritating his left eye. He went to the doctor the next day when the irritation continued and had a small particle removed. He did not miss work and was not on restricted duty.

Employee was swinging a hammer on the gypsum bin on the #3 finish mill feeder weight floor and he allegedly felt shoulder disconfort. This was considered a first aid case and he did not miss work and was on full duty. On 9-17-09 he had a medical procedure that requires this to be MSHA reportable. He was a member of a volunteer fire department during this entire time.

Injury went to the finish mill room to get a sample of cement from the number 2 finish mill and alleges that she tripped on the floor as she turned to walk back to the lab with the sample. She claims that she tried to catch herself with her left arm and as a result injured her left shoulder. This is a lost time accident.

Employee was using a torch to cut off a piece of 3/8 plate steel and the end piece fell and hit his toe. An X-ray was taken and it revealed that a small hairline crack was present on his big toe on the right foot. He was wearing all PPE. He was released to full duty and did not miss any work.

Injury turned in that he had carpal tunnel syndrome. The company investigated this claim because his job duties do not correspond with this condition. This became MSHA reportable on 8/18/09 when he missed work.

Employee is a loader operator and over time feels that operating it hurt his back. He reported it on 9/17/08 and it became a lost time on 9/18/08.

Employee smashed middle 2 fingers on left hand between counter weight and support pole when counter weight was nugged by front end loader.

Employee claims he hurt his back while shoveling up rock at the crusher. Pain has persisted and the injury began a lost time on 9/4/08

Employee claimed hs was having pains in his arms and was sent to a local physician and given some pain meds. The problem has persisted and follow up testing with a specialist revealed that he has carpal tunnel in both hands. The treating physician's office informed us of this on 5/20/08.

He bumped his elbow on a steel tube while descending/climbing down into the secondary crusher where he was working thus causing soreness to his right elbow, right forearm and hand.

EE was installing liner plates in the secondary crusher when his right hand was struck with a sledge hammer (not the hammer itself) thus bruising right hand pinky finger.

While removing a bar (or bow) that secures a mill door employee strained his back. The pain has not subsided so he had further testing done in early April. Employee began physical therapy for the injury on 4/10/08 making it a reportable injury.

EE and another EE were lifting a wear plate to carry it when the other EE dropped his side causing injured EE's back to jerk trying to withstand the weight of the metal.

Employee was using cutting torch, bandsaw and grinder performing work in the kiln installing a platform when a piece of foreign debris entered his right eye.

Walking to tool bucket, he stepped on a plate which slipped and fell out from underneath his feet. Left leg went in a hole and he caught self with left arm/shoulder.

Employee cut right ring finger while cutting vacuum hose. Injury required 6 stitches.

Employee was desending a staircase, when his foot slipped, causing him to fall to the ground. He dislocated his left foot in the process.

The employee was reaching for a powerpack as a co-worker handed it down to him. He claims he pulled his right shoulder when the other employee dropped the powerpack to him. He reported it, but thought it would be OK. However, the pain has persisted. An MRI on 8/13/07 showed he has had some trauma in the shoulder and he was given 2 shots.

Worker was attempting to reposition forks. (Forks are hard to move). He let it down with his right hand to push over with his foot, it swung back and smashed finger on left hand.

Employee was stepping off a mill to the catwalk and stepped on an airhose. He twisted his left knee. Pain persisted so an MRI was done in April, a minor tear was found. Employee has personal health issues so surgery was put off until 5/3/07 (just prior to the shifts long weekend). Employee was allowed to return to work with restrictions.

Employees were charging a ball mill and setting a drum full of balls into a cradle dump when the drum slipped out of choker and struck the employee. He ended up needing surgery on 4-06-07 and this became a lost time on 4-09-06

Employee was doing overhead work jackhammering hardened product. He slipped when material broke free and jackhammer hit his left hand causing a slight fracture at the last joint of his 4th finger.

Employee was attempting to remove a split tire from a conveyor belt when her glove got caught by the wire from the steel belt. This ended up with her arm getting pulled between the head pulley and a roller. She received friction burns (3rd degree) and possibly a hair line fracture to the wrist (not confirmed).

Employee was getting ready to turn off pony motor when coupling came apart. Coupling or guard struck employee in the hand cutting his finger.

An employee was attempting to relight the preheat torch when the explosion occured, due to a build up of flammable gas. Initally we were told no 7000-1 was needed then field office called on 6/23/06 and requested one confirmed 6-27-06.

EE was kneeling on a metal gravity platform welding/cutting for approximately a 10 hour shift shortly after shift ended knee became inflamed.

EE WAS THREADING RAW MILL ENTRY COVER THROUGH OPENING AND HE PUSHED THE COVER TO STRAIGHTEN IT UP SO HE COULD SLIDE IT THROUGH THE OPENING. THE CHOKER SLIPPED, CAUSING THE COVER TO SHIFT AND STRIKE HIS LEFT SHOULDER (BROKEN COLLARBONE).

Employee was lifted in a man basket to reach a water valve located in a pipe. the valve was hard to turn. The employee put forth more effort to turn the valve closed. After closing the valve the employee complained of being weak and short of breath. At that point a mill personal thought the employee may be having a heart attack.

Employee injured shoulder back in February and pain persists. I just received the (MRI) results, which show a Rotator Cuff Tear and surgery is required. He is scheduled for surgery on 4-20-2006. This will become a lost time on 4-21-06 (which would be his first scheduled shift for that week).

Employee looked at welding arc. burning the corna of both eyes.

Employee was dumping sample remains into a drag hopper when some of the product (clinker) blew in his eye. Employee was seen at the ER and was given prescription medication (as in indicated by invoice I recieved today 12/30/05). On 12/15/05, supervisor informed me employee was seen by ER doctor and returned to work.

Employee was trying to get access to a valve to open it. While using a breaker bar to bust up built up material around the valve he smashed a finger, which resulted in a laceration of same.

Employee twisted her ankle aggravating an old injury. Pain has persisted and employee was sent for follow-up diagnosis. Employee is now in physical therapy to work the tendons and relieve the pain.

Worker was removing a 4160 starter from the housing with a co-worker, when the starter rotated on its pivot point and smashed his hand at the hand hold.

Employee slipped and fell off roof after hooking roof to crane for removal.

Employee was trimming weeds and got something (piece of grass or weed) in his eye.

Employee was raising electric cart to perform repair. Cart fell hitting employee on the foot fracturing 2 of his toes.

WORK WAS BEING DONE IN THE MILL BUILDING, DUCT WAS BEING INSULATED WHILE OPERATIONAL, OUR INSULATOR SUFFERED MILD HEAT EXHAUSTION

Employee alleges that he had released the handle so that he hopper would raise foreward and as he was looking in another direction the hopper rocked back causing the handle to strike his little finger on his left hand.

An ee was using a beater hammer to knock a gear off of a shaft while injured was sweating the gear. A piece of steel came off of the hot gear and embedded into ee's right cheek.

EE IS UNSURE HOW INJURY OCCURRED. HE HAS A MUSCLE STRAIN IN HIS MIDDLE BACK. IT WAS A GRADUAL ONSET. EE IS UNSURE WHEN INJURY ACTUALLY HAPPENED - ACTUAL DATE OF ACCIDENT IS UNKNOWN. (A-C EQUIPMENT SERVICES: KILN REPAIR)

Employee alleges that he was hit in the face with some dust while beating the dust lines to the dust collector. Dust entered the Right eye.

Employee alleges that he struck his little finger (right hand) against a valve while pushing a bar through a port hole to unplug the port hole of material that was caked in side the dust tank.

Employee alleges that he slipped on icy surface while walking. Employee struck his left hip and left arm on the pavement and continues to have problems with left hip.

Employee alleges that he was removing a sprocket from gear box off of #3 finish mill elevator. Employee hit sprocket with hammer causing sprocket to chip. Employee alleges chip struck his right leg puncturing a vein. Employee was given two shots and medication.

Employee alleges he was using a box cutter to cut the tape off of a flex hose so that he could use the 4" flex hose. The tape was holding a 3" hose and 4" hose together. Cutter slipped and cut top of left hand. Cut required sutures.

Employee alleges that a piece of dust fell from the kiln, at the tire door, and went into his eye. Doctor prescribed medication.

EMPLOYEE ALLEGES THAT HE HAS PAIN BETWEEN SHOULDERS. HE STATED HE DOES NOT KNOW IF OR WHEN HE MAY HAVE HURT IT. HE ALLEGES IT HAS BEEN HURTING FOR THE LAST 24 HOURS.

Employee alleges that he injuried his right knee (knee popped) while pushing a wheel barrow loaded with coal refines. (approx. weight 50 lbs).

EE was welding, raised welding hood then chipped weld and got piece of slag in his left eye. ee did not realize this at the time, ee determined this after his shift ended at 7 am and was taken to the emergency room the morning of 3/31/04

He was plugging into a 100 ton hydraulic jack and the pop off valve went off and sprayed hydraulic fluid into his eyes.

EE WHILE WALKING TRIPPED & CAUGHT HIS FALL TOWARDS THE GROUND WITH HIS RIGHT ARM, THUS INJURING/BRUISING HIS RIGHT SHOULDER. X-RAY AT E.R.- BRUISED, BUT NO WORK RESTRICTIONS. EE NOTIFIED EMPLOYER ON 5/14/04 THAT HE WAS STILL EXPERIENCING DISCOMFORT IN RIGHT SHOULDER FROM THIS INCIDENT & REQUESTED FURTHER TREATMENT. GIVEN MRI ON 5/19/04.

EMPLOYEE STEPPED OFF A FORKLIFT AND DIRECTLY ONTO A PIECE OF WELDING WIRE THAT WAS LAYING THERE IN THE GROUND AND THIS WELDING WIRE PUNCTURED THE BOTTOM OF EMPLOYEE'S SHOE AND THEREFORE ALSO PUNCTURING THE BOTTOM OF HIS RIGHT FOOT.

EE HAD BEEN CUTTING, & HAD HIS FACE SHIELD ON, & HE WAS FINISHED FOR A MINUTE, SO HE PUT THE SHIELD UP IN THE STORED POSITION. WHILE IN THAT POSITION, DUST SETTLED ON IT, THEN HE LOOKED UP AT HIS FELLOW WORKERS, & THE DUST SLID OFF OF HIS SHIELD & BEHIND HIS SAFETY GLASSES DIRECTLY INTO BOTH EYES. EE STATED HIS EYES BURNED.

PULLING A 4,000 LB. BEARING, IT GOT HUNG UP. THEY WERE TRYING TO GET IT LOOSE AND THE BEARING FELL ON HIS LEFT FOOT.

EE AND CO WORKER HAD GONE DOWN TO THE GROUND LEVEL TO TAKE THEIR LOCKS OFF A DISCONNECT IN THE MOTOR CONTROL CENTER. EE WAS TO GO BACK UP ONT HE LANDING WHERE THE MOTOR WAS AND THEN TO SIGNAL HIS WORKER TO THEN CLOSE THE DISCONNECT ALLOWING EE TO BUMP THE MOTOR FROM A RUN OFF JOG STATION NEARBY. HE STAND AT THE RUN OFF JOG AND LOOKING DOWN AT ANGLE WATCHING FOR HIS WORKER .

BUZZI UNICEM EMPLOYEES WERE CLEANING OUT A KILN & HAD STEEL BALLS SCATTERED ON FLOOR. EE STEPPED OUT OF KILN & DIDN'T SEE THE BALL ON THE FLOOR.

A MAN INSIDE KILN WAS HAMMERING A PLATE TO RELASE IT WHEN A PIECE OF STEEL SLIVERED OFF AND STRUCK EUGENE IN THE CHEEK.
