Contractor was changing V-belts on #4 conveyor. EE was using index finger on their right hand and was walking the belt around the sheave. EE thinks the belt hung on the back stop, because it broke loose and EE's left wrist was run between the belts and sheave 2 times before EE could get it free.

Employee failed to lock out breaker and removed gutter cover in (ER6), it is unknown if the employee inadvertently moved a bare wire into the energized tap blocks causing an arc occurrence resulting in the employee being burnt on hands. Employee was transported to hospital by company truck.

EE was at crusher console adding hyd. oil to reservoir. While shutting off valve at nozzle end, the weight of hyd. hose abruptly pulled out slack in between hands & into chain holding console door in open position. The weight of hose on chain caused door to start coming downward. EE tried to pull hose clear of lid when lid caught the EE's right hand in between it & console frame.

Employee was descending from the control tower stairs. Employee tripped near the bottom of the stairs and fell forward hitting his head on the tower leg made of angle iron. Employee incurred a laceration above his right eye, a contusion, and swelling to the right cheek.

Employee and another worker were attempting to load scrap railroad ties on to a trailer. When employee attempted to move a tie by hand, the tie rolled over and the employee's glove caught on a splinter. His hand was caught between two railroad ties smashing his L-3 finger and lacerated the tip of his finger. Restricted duty - no use of left hand.

When he reached ground level, he turned to walk away from the truck and felt a burning pain in his right knee. *Continued pain and went to clinic on 8/27/10. Given anti-inflammatory and physical therapy order for evaluation. Treatment clarifed with clinic on 9/7/10.*

Emp failed to report incident in a timely manner. Initial request for medical attention 05/24/2010. Investigation, employee claims he was lifting pins on 05/06 and 05/07, had pain in shoulder. On 05/13 slipped, increased pain.

A BNSF train was pushing a conveyor train at Mill Creek yard. No spotter was being used. Our employee was on the locomotive of our ballast train loading cars on track #3. One of the switches at the cross-over was set to send the cards from track #2 to track #3. BNSF failed to align the switch back to track #2 and collided with our train.

Employee assisting another employee un-hitching an air compressor on a trailer from the maintenance truck. IW was pressing down on the latch to release the trailer from the ball, when the trailer rolled, causing his L index finger to be smashed between a steel plate on the truck above the hitch and the trailer hitch. IW L index finger received a serious avulsion.

Employee was cutting a tag off of a belt with a box blade. Employee changed blades and didn't put his glove back on his left hand. The box blade slipped as the employee began cutting and resulted in him cutting his hand below the base of the thumb. Resulted in 8 stitches.

Employee walked up hill on a truss, to get past a skid steer. Once past the machine, he dismounted. When he let go with his left hand, his wedding band hung on the expanded metal troughing idler guard causing a laceration. He received 14 stitches, as a result.

THE EE FORGOT TO THROW THE SWITCH CAUSING THE PUSHED RAIL CARS TO CORNE R AND COLLIDE WITH OTHER RAIL CARS. THIS CAUSED ABOUT 2500 WORTH OF DAMAGE TO THE RAIL CARS. THE LOCOMOTIVE OPERATOR EXPERIENCE MILD CERVIAL MUSCLE STRAIN.

ON 11-12-03 EE WAS WALKING BY ANOTHER EE AS THE EE PULLED A PIECE OF GRATING DOWN FROM ABOVE, HITTING EE ON THE HEAD, KNOCKING HIM TO THE GROUND HE DID FEEL LIKE HE WAS HURT AND REFUSED MEDICA L TREATMENT UNTIL 11-20-03, WHEN HE SOUGHT TREATMENT FROM A CHIROPRACTOR 3 TIMES AS HE CONTINUED WORKING. ON 11-28-03 HE HAD NOT IMPROVED AND IS NOW GETTING FURTHER MEDICAL ATTENTION.

NO ONE WAS INJURED. THE ACCIDENT HAPPENED IN THE QUARRY. WHILE MOVING THE SHOVEL THE OPERATOR WAS NOTIFIED THAT THE ENGINE COMPARTMENT WAS ON FIRE, THE OPERATOR THEN SHUT THE SHOVEL DOWN, ACTI VATED THE FIRE SUPPRESION SYSTEM, AND EXITED THEMACHINE, THE FIRE WAS UNCONTROLABLE AND ENGULFED THE SHOVEL ENGINE COMPARTMENT. TOTAL LOSS.
