During the night shift, while operating the fuel boat, employee believes they pulled a muscle in their left shoulder.
Employee was lifting a screen at the tipple and strained his right shoulder. This did not become reportable until 7/16/08 as the company did not know until then that there was an injury. He did not report the injury until 7/16/08. (Misc Mail 5/8/09 #2-had surgery on 1/5/09. He return to work on 4/27/09 with no restriction total misssing 111 days)
This was not an accident. Employee drove into work, did not exit his truck but was found in his truck. He died of a heart attack.
Contracted employee missed last step and landed wrong. He has a right knee strain.
EMPLOYEE NOTICED THE TOOL BOX LID WAS LEFT OPEN. HE WENT TO CLOSE THE LID, TRIPPED AND CUT HIS LOWER LEFT LEG. TWENTY STITCHES REQUIRED TO CLOSECUT.
EE MISSED TOP STEP OF LOADER AND FELL TO THE GROUND.
