The employee's leg was struck by a shifting slurry hose while disconnecting a coupling. They went to a doctor and was diagnosed with a tear in their ACL. The injured employee had surgery on 9/4/25 which made this injury reportable.

Haul truck driver reached between the seat and counsel to get a part, when EE pulled EE's hand out it was cut between the middle finger and ring finger between the knuckles, causing EE to get stitches. Operator returned to work the next morning.

EE was assisting with carrying an electrical cabinet. While re-positioning after walking through a doorway, EE twisted right arm and felt a pop in right elbow. The injury was initially treated as first aid with no restrictions. On 8/16/2023, EE visited a medical clinic and work restrictions were given.

EE suffered soft tissue damage, in the right shoulder, resulting from a fall to the same level, while walking on uneven ground in the pit. The injury was treated as a first aid. EE was prescribed Physical Therapy. EE began Therapy on 1/22/2021.

EE was removing a 1.5" aluminum fitting from a steel pipe collar. The fitting broke and separated from the threads inside the collar. The momentum cause the EE to strike an elbow on a 4" gate valve located behind EE. The injury was treated as a First Aid. EE was evaluated and had X-Rays on 12/17/19. The X-Rays revealed a chipped bone.

Employee was cutting a pipe overhead using a grinder with a cut-off wheel when the grinder licked out and cut arm. The employee was taken to a medical facility where 11 stitches were used to close the laceration.

The employee was exiting and dismounting the machine. While going down the steps of the machine, the employee slipped on the last step and fell to the ground landing awkwardly and spraining his left knee in the process. Employee was taken to a medical clinic where he was placed on light duty.

Employee was closing a bottom discharge gate on a rail car. The gate was difficult to move. The gate suddenly broke free and the employee caught his finger between the bar he was using and a railroad tie. The incident was treated as a first aid. The employee sought medical attention on 10/31/12 and was diagnosed with a fracture on the tip of the finger.

Employee was using a 60" bar while closing a bottom discharge slide/gate on a rail car. The slide was difficult to move. The bar slipped from the slide "knuckle", causing the employee to fall forward, mashing two fingers on his right hand between the bar and RR track rail.

Employee was driving a locking pin into a high pressure hose coupling. Employee struck the ring finger of his left hand with the handle of the hammer he was using, resulting in a laceration of the finger. The laceration required 2 stitches to close.

Employee was at the fuel station to fill the fuel tank on a D8 dozer. The employee was standing on the dozer blade push arm, when he jumped backwards to dismount. His ring caught on the latch for the batt box cover. The skin was pulled from the ring finger of his left hand. The finger was surgically removed later at the hospital.

Injured while attempting to move a large rock away from a power pole. Injured was pulling the rock away from the pole, when the rock rolled back toward the pole. The injured person jerked his hand from the rock and cut the second finger of his right hand on a sharp edge of the rock in the process. Injured was not wearing gloves.

Victim suffered a broken bone and laceration of the ring finger and a laceretion on the middle finger of the right hand. The injuries occured while the victim was dumping a scrap metal box in the maint shop and was struck by the metal roller frame that supported the scrap metal box.

Employee was closing the plant entrance gate to secure the plant for weekend shutdown when he stepped on the edge of the pavement twisting his ankle. Employee did not seek medical attention until 7/3/2006. He was examined and released with no apparent damage. Employee returned to the Dr. on 7/14/2006 and was started on therapy at that time to reduce swelling of his ankle.

The victim and a co-worker were attempting to load a steel beam, 8"x10"x16', onto a trailer with the aid of a backhoe. As they were postioning the bachoe to connect the lifting chains, the backhoe struck the steel beam causing it to roll onto the victims R. foot.
