Employee did not report the injury in a timely manner. EE has been treating with EE's personal doctor who is requesting surgery. We accepted the claim today. Employee fell from track hoe on to EE's left shoulder.

Employee smashed his finger between a sledge hammer and a table, fracturing it.

Employee fell and hit his head.

Unloading steel and caught his finger between his steel toe boot and the steel while setting it to the ground.

The starter had locked up on an air compressor. Employee grabbed the V-belt to move motor. The engine kicked back and caught his right hand in alternative sheave.

EE was using a utility knife to cut belting. The knife slipped and cut his right wrist.

Employee was using a knife to cut conveyor belting. The knife slipped off belting and cut his right leg above the knee. There was no lost time or restricted duty but two stitches were required.

Employee was shoveling in surge tunnel and injured his right arm.

The employee was operating the dewatering truck on a state highway returning from a shot at Hartshorne Quarry. Another driver lost control of her vehicle and crossed center lane and struck dewatering truck and turned it over.

Employee was replacing V-belts on 8x20 belt. The pipe wrench slipped and struck his face under his left eye. No lost time, but sutures were required

Employee was working with other employees to replace bearing on S-1 conveyor. Chain hoist slipped and bearing fell and smashed right index and middle fingers on his right hand.

EE was removing a guard from under a 8x20 screen. The guard fell and cut his left index finger. No lost time - sutures were required.

Employee was replacing belting on side of screen and cut his hand with a box blade knife while cutting bolt holes. No lost time, no restricted duty, thirteen stitches were required.

Employee was grinding on metal screen when grinding wheel slipped and cut through his glove and cut his wrist. Six stitches were required.

Employee was helping to hold a hammer wrench while working with other employees to loosen bolts on the shorthead. The hammer wrench spun around and struck the thumb on his left hand. His thumb was fractured. There was no lost time or light duty.

Employee was using a cutting torch to heat up a bearing at the J-1 conveyor when the base of the torch exploded, burning a spot on his left arm.

Employee was getting out of his loader when he fell and injured his right arm.

Employee was picking up a piece of metal. When he set it down on a block, it bounced up and struck his right hand. His hand was cut, requiring four stitches. There was no lost time and no restricted duty.

Employee injured his lower back while installing troughing rollers on F2 conveyor.

Employee was checking voltages and fuses on an electrical switch at the main plant. While checking the fuses, he had the meter setting on Ohms. When he started checking voltage, he forgot to change the meter's setting. When he checked the voltage, the phase to phase contact caused an electrical flash when the meter's probes came in contact with the current.

EE had finished cleaning loader windshield and stepped onto loader platform. He slipped and fell from the loader, landing on the ground. His right foot/ankle was broken.

EE injured his lower back and right hip while operating a loader at the plant.

Employee was moving a screen and smashed the little finger on his right hand. There was no lost time or restricted duty but he did receive three stitches.

Employee was cutting a piece of rubber when the knife slipped and cut his right thigh. There was no lost time or restricted duty. Sutures were required.

Employee was drilling a hole in a piece of metal. The drill bit caught and spun the drill around into his left wrist. His wrist was cut and required 7 sutures.

EE was working with other employees to pull the shaft out of the VSI. He was walking on the catwalk and caught his foot on a piece of metal guard. He fell and injured his right arm.

EE was replacing V-belt on short head crusher. His glove hung in the belt and when the crusher broke over it pulled his hand in between the belt and sheave. His right had was bruised and his thumb was cut.

Driller was loading empty barrels into the back of the work pickup truck. While in the process he felt a twinge in his lower back. Over the course of the next week and a half, his back started to get worse by the day. Finally he went to the doctor on 12/9/04 who said his back was strained and out of place so he popped his back and prescribed muscle relaxers.

EE was working to replace the sides on a caterpillar 980G loader bucket when the new piece they were fitting in place fell and landed on his right foot.

Another employees was using a backhoe to put a liner in a crusher bowl at the plant. The backhoe shifted and the outrigger landed on employee's foot. His right foot suffered a contusion.

Employee was working with other employees to change a screen on the #21 shaker. The screen got stuck. He was using a pry bar on the screen when the screen came loose and fell on the index finger of his left hand. He suffered an open fracture of his left index finger and is losing time.

EE WAS WORKING WITH OTHER EMPLOYEES TO CUT THE BEARING OFF THE SHAFT ON THE #2 CONVEYOR. THEY HAD HUNG A TORCH ON THE I-BEAM OVERHEAD FOR LIGHT. WHILE THEY WERE PUTTING A NEW BEARING ON, THE TORCH FELL AND STRUCK INJURED, KNOCKING OFF HIS HARD HAT AND SAFETY GLASSES. THE TORCH STRUCK HIS RIGHT EYE.

Employee stepped out of a Caterpillar 769D truck to dump a bin. He slipped off the catwalk and fell to the ground. His back was injured.

THE RETAINER PIN THAT HOLDS THE STRIKER PIN IN PLACE ON THE ROCK BREAKER BROKE. EMPLOYEE ATTEMPTED TO MOVE THE APPROXIMATELY 250 LB STRIKER PIN BACK INTO PLACE AND STRAINED HIS BACK. HE SHOULD NOT HAVE ATTEMPTED TO MOVE THIS HEAVY OF AN OBJECT WITHOUT ASSISTANCE. A HOIST SHOULD HAVE BEEN USED. HE IS LOSING TIME AS OF 10/15/03.

EMPLOYEE WAS TIGHTENING BOLTS ON THE 4 1/4 CONE CRUSHER. HE WAS USING A WRENCH AND SLEDGEHAMMER. HE INJURED HIS RIGHT SHOULDER WHILE SWINGING THE SLEDGEHAMMER. HE DID NOT SEEK MEDICAL ATTENTIO N UNTIL 9/30/2003. HIS WORK ACTIVITIES WERE NOT RESTRICTED UNTIL HIS SECOND DOCTOR VISIT ON 10/3/2003.

EE WAS DISMOUNTING FROM A CATERPILLAR 990 LOADER. HE SLIPPED AND STRUCK HIS ELBOW ON THE LOADER'S STEPS.

EMPLOYEE WAS DISMOUNTING A LOADER. HE SLIPPED ON MUDDY LADDER AND STRAINED BACK.

EE STRAINED HIS BACK WHILE TRYING TO OPEN THE ENGINER COMPARTMENT ON A LOADER

EE WAS USING A DOLLY TO MOVE A TOOLBOX. IT ROLLED BACKWARDS & TOOLBOX STARTED TO FALL. HE CAUGHT IT & STRAINED HIS LOWER BACK. ACCIDENT BECAME REPORTABLE ON 1/20 WHEN DOCTOR TOOK HIM OFF WORK.

EMPLOYEE STRAINED HIS RIGHT ARM WHILE PULLING ON A WATER HOSE THAT WAS BURIED IN THE MUD.

EMPLOYEE STRAINED HIS BACK WHEN HE STEPPED OFF A BELT.

EMPLOYEE WAS LEANING OVER A STEERING ARM ON A CATERPILLAR 988F LOADER TO HOOK UP A GREASE LINE. HE FELT A BURNING/TEARING PAIN IN HIS STOMACH.

EE WAS PLACING PRIMERS IN BORE HOLES WHEN HE BECAME HOT AND DIZZY AND SUFFERED WEAKNESS.

EE WAS WALKING INTO SHOP TO GET A WRENCH. HE STEPPED ON A HOSE AND TWISTED HIS ANKLE.

A GUARD CAME OFF THE #30 CONVEYER AND HIT HIM INTHE BACK. (ACCIDENT WAS NOT REPORTABLE UNTIL 9-12-00).

EE WAS WASHING THE TAIL PULLEY'S ON A CONVEYOR. WHEN HE PULLED ON THE HOSE, HE SLIPPED IN MUD AND STRAINED HIS BACK.
