Employee pinched right middle finger between a slate bar and belt structure when EE was knocking ice away from the belt line. The employee received 3 stitches.

EE statement: I was tightening a joint of conduit with a 12" pipe wrench and my footing slipped causing my body to twist hurting my shoulder. Pain in my shoulder, Rt shoulder.

A Foremen traveled into the return and found a fall in #9 entry between spad # 4+20 and 5+00.

The injured EE went to hang ventilation curtain, while his operator put up a corner bolt. The EE hanging curtain went back to get nails. The Operator started repositioned the bolter, not realizing his helper came back. When he started moving the bolter he bumped his helpers hip.

The Employee stated that he was moving miner cable and felt a pain in his right groin area. The Employee is waiting for surgery to repair a right hernia.

Employee stated he lowered the drill pot on his left knee. The Employee had no broke bones the Dr. placed him on light duty for a few days.

The intake phone would not work. The Examiner traveled the intake to locate the problem and found a roof fall. The fall was in the South mains in number 6 entry at spad 3+20. The fall was 18 feet wide by 30 feet long and 10 feet high.

An employee was loading weed eaters in the back of a pick up truck, one of the employee's with him shut the tailgate catching the other employee's left pinky in the tailgate.

The Employee stated he was loading roof straps on a roof bolter, as he was placing the straps on the bolter his hand slipped and he caught his forearm on the edge of the straps lacerating his forearm. He received stitches in his right forearm.

Employee was walking air courses and found the roof fall. We called the MSHA hotline at 9:20 A.M. We then cribbed and dangered the area off. Air was flowing over the fall, we pulled our crews outby to start corrective actions.

At 11:50 p.m. the roof fell in the #4 room between spad #'s 2+60 and 3+10. the fall was approximately 18 feet wide 35 feet long and 6 feet high. The area was cribbed off and the area dangered off. The fall did not impede ventilation.

The Employee went to climb inside of the #6 rail ride, his foot got stuck in the mud and he fell into the ride catching his left hand on the SCSR box. He lacerated his palm and received sixteen stitches.

Employee was tramming a buggy around a corner and ran into rope hangers hanging from top, one caught his right nostril causing a laceration. (6 stitches)

EE was walking the MMU 001 belt line and he saw the top working at spad #4+20. EE kept all personnel out of the area and pulled the section Employees outby. The roof fell in on the beltline in #4 entry. EE placed floor to roof support around the fall and moved the section.

After helping his roof bolt partner straighten out a bent cable bolt, he was walking back to his side when he slipped and as he attempted to catch himself he placed his hand on the idle arm as his partner was running the drill pot up. At this time his hand got caught in the mechanism causing a crushing injury to his left index, middle and ring finger.

The Employee stated that he went to push a roof bolt out and felt a sharp pain in his groin area.

Employee was walking and hit his hard hat on a roof bolting knocking it off of his head, he raised up and struck the top of his head on a roof bolt causing a laceration. (required staples)

Employee was roof bolting and got his finger caught between leveling arms and the canopy post. (fractured finger)

The employee stated that he had just placed a roof bolt in the head of the roof bolter and was pulling his arm back when a piece of shale 2'x3'x 2" fell hitting the Employee in the right forearm and hand, fracturing his 5th small metacarpal hand.

Employee was cutting the band off of a bundle of roof bolts using two sets of channel locks, due to the amount of pressure on the band when he cut it loose it backlashed and cut his right forearm. (He received 8 stitches)

The Employee stated he crossed thru a regulator and twisted his lower back.

Employee was bending a roof bolt when the bolt slipped out of his hand and the bolt plate lacerated his left forearm. (5 stitches)

The employee stated he was reaching for a roof bolt and twisted his back.

Employee strained his right shoulder tightening down belt structure, he was treated at the Hospital ER and returned to work, upon the results of his MRI test EE was excused from work by the Dr. for 6 weeks.

Employee was working on a dryer, the employee stated that he slipped and caught his foot and fell backwards injuring his tibia and fibula.

Employee was cutting a rope hanger with a utility knife and accidently cut his finger (7 stitches)

The employee stated that he was installing 4" water line. The tool he was using slipped off the coupler and he hit a fire valve lacerating his left pinky finger, resulting in three stitches.

The owlshift crew called the dispatcher to leave for outside. The dayshift crew called the dispatcher to leave for underground. The day shift crew was held up and did not leave right when they had called the dispatcher. The dispatcher did not let either crew know that each crew was on the rail. The dayshift crew made 770 feet underground and collided with the owlshift crew.

The employee stated that he was cutting a metal band off a bundle of cable bolts that held them together. When he cut the strap, the band sprung up and lacerated his left forearm resulting in 5 sutures.

The employee stated that he and another employee were replacing a rub rail on a continuous miner and when he was pushing on the rub rail he pulled something in his back.

A continuous miner was hung up in a hole. The employee was cribbing the miner up to place cribs under the cats, one of the cribs popped out and hit him in the chin causing him to receive two stitches.

The mine was idle, the outside communications employee noticed he lost power at the #7 mine. EE went in to see what happened and found a roof fall at x-cut #16 spad (8+90). The fall was 8' high x 20'long x 25'wide. EE called the fall in at 1:38 A.M., we then cribbed around the affected area and then started re cable bolting the affected area.

There was a roof fall @ 28 x-cut (20+60) 0n 2 belt #3 entry. The fall was approximately 18' wide x 20' long x 6' high.

EE went outside to get supplies. When he started back in the mine he found the roof fall at 3+25 on 1 belt x-cut 4-5. Fall was approximately 40' long x 18'wide x 6'-8' high.

Employee was the scoop operator, he was trying to pull a shuttle car off the feeder. He ran his scoop bucket on top of the shuttle car and the scoop dropped off the shuttle car and bounced employee inside the cab of the scoop, he hit his upper left rib cavity on the inside of the scoop cab causing a pulled ligament.

The operator thought his helper motioned him to advance forward, when the operator dropped the ATRS on the bolter the front end of the roof bolter dropped and the drill station canopy hit the helper on the head.

A roof fall occurred @ 30+40 in a three way intersection on the south mains intake, #7 entry there was already some cribs set, the fall fell from the rib line to the cribs. the fall was 6' wide by 27' long 2 to 6' high.

We found a roof fall @ spad #29+00 in a cross cut 4-5 in the South Mains, the fall was approximately 26' long x 18' wide x 6'high.

The intake phone would not work so the Foreman traveled the intake to locate the problem thats when he found a roof fall on the 1st East submains in the intake @ spad # 10+50. The fall started in #7 and fell to the edge of #6. We have two intakes. The fall was app 40' long, 18' wide,8-10' high.#7 is not our primary escapeway.

We were doing our weekly exam and found a roof fall in our East mains intake in # 7 entry it was a three way intersection at spad # 30+40, the fall was 18' long by 8' high #7 entry is not our primary escapeway.

The #7 Mine uses bed ash to make roads with, it is similar to concrete. EE got bed ash on his pants and later got wet, he started operating a scoop. A short while after operating the scoop his legs broke out in a rash. He later went the Dr and the results were second degree burns.

Employee was helping bolter operators, when he reached over the bolter to mark an area on the roof with paint. The bolter operator on the opposite side moved the bolter inby to the next row to be installed and the helper had his foot in front of the bolter tire and the tire rolled on his foot.

Mine was idle and a foreman found the roof fall at 1:35 pm, at the intersection of station 2+40, along track South Mains.

Employee was installing roof bolts when he had his right thumb caught between the drill chuck and the mine roof. The employee received a laceration.

Employee was moving a box and turned and struck a metal roof strap. The strap lacerated his right ear. The employee received sutures.

Employee was walking and struck his head on a roof bolt. Employee received slight neck pain.

Employee was tramming scoop when his canopy struck and tore a roof strap loose and it rotated and struck him on the back of his neck. Employee received 4 stiches.

Injured Foreman was traveling from the active face area, when he was struck by the shuttle car.

Employee was standing on a curb outside of bathhouse near parking lot and another employee accidently stepped on his foot. Employee lost his balance and fell from curb fraturing his right ankle.

Employee was assisting bolter operator loading supplies. The employee had his left hand in hole provided in straps, when strap was being loaded his left ring finger was through hole in strap, striking another strap and causing a laceration.

Employee received injury to his right ring finger while using a slate bar to dislodge material from a chute area. The employee smashed his finger between the slate bar and magnet. The employee received four sutures.

Roof fall discovered at 7+80, # 7 entry on the 6th East Panel. Roof abnormality, additional supports were installed.

The continuous miner operator was positioning the mining machine to remove rock from the top of the miner, when he thinks he must have tripped falling down onto the remote control box causing the tail boom to swing and the rock to slide off on him. The miner operator was not exactly clear on what occurred.

The employee was pulling a drill steel from the mine roof and his hand was on bottom of steel. When the steel came loose from the roof the employee's right pinky finger was smashed between the bottom of the steel and the roof bolter drill head. The employee's received a laceration which required 6 sutures, and a broken 5th Distal Phalanx.

I was exiting the side of the mini-trac and caught my foot as I caught my foot I lost my balance and fell on my shoulder. No rules or regulation was broken. Bruise left shoulder.

Employee had gotten off scoop to clean up area and was walking and looking down just as he looked up a piece of wire screen struck EE's safety glasses, they slid down and cut his left cheek.

Cutting mine belt when knife slipped out of belt striking finger, No rules or regulations were being broken.

Roof fall, East submains 3-2 xcut, 29x 17+40. Length 30', width 18', thickness 9'. Damaged return wall.

EE was pulling a roof bolt machine with a scoop on the section using a chain. The chain broke and one end came threw and opening on the scoop canopy and struck EE in the face.

Roof fall was found at 9:15 am while traveling return air courses. Fall was approx 180' long starting at spad #3+00 to #4+80 in #1 entry of 4 north panel.

Roof fall in 4-way intersectiion 18' x 18' x 7' height. East submains #5 entry 39 xcut (23+40).

Roof fall east sub mains #6 entry 27x 16 + 20 intersection 27x to outby rib 28x. Length 50' x width 18' x height 7'.

Employee got a metal fleck in his eye while grinding.

While setting concrete forms, employee jumped aprox. 2.5 ft. into excavation to assist. He stated he hurt his back when he hit the ground.

Roof fall at Entry #5 at station 8+85. Fall was 22'long x 18.5'wide x 5.5'high. No equipment was involved in the fall.

Roof fall at station #7+80 entry #3. Fall measured 19.5' length x 19.5' wide x 6.5' thick. No equipment was involved in the fall. The area was cribbed & dangered off.

Employee was moving Fletcher roof bolter in outby area of mine. The canopy of the bolting machine caught a roof bolt & a small piece of metal projected into his left eye. Employee had safety glasses in his possession, but was not wearing them.

Roof fall at break 11 in #6 entry in 1st north panel. Fall was 19' long, 19' wide and 5.5' thick. No equipment was involved in this fall. The area was cribbed and dangered off.

Employee was drilling a hole in the roof to install belt hanger, he was getting ready to use pusher to drill. He had looked toward the controls when a rock fell and struck his left hand. This injury did not involve any aspect of compliance with rules or regulations, job skills or attitude.

Roof fall at station number 14+10 in #2 entry 1st north panel. Fall was 7' high, 20' wide and 65' long. No equipment was involved in the fall, the area was cribbed and dangered off.
