EE was spraying De-icer onto the scalping tower belt. When EE turned off the nozzle the hose split spraying De-icer into EE's eyes.

The injured EE went to hang ventilation curtain, while his operator put up a corner bolt. The EE hanging curtain went back to get nails. The Operator started repositioned the bolter, not realizing his helper came back. When he started moving the bolter he bumped his helpers hip.

The intake phone would not work. The Examiner traveled the intake to locate the problem and found a roof fall. The fall was in the South mains in number 6 entry at spad 3+20. The fall was 18 feet wide by 30 feet long and 10 feet high.

The Employee stated he was loading roof straps on a roof bolter, as he was placing the straps on the bolter his hand slipped and he caught his forearm on the edge of the straps lacerating his forearm. He received stitches in his right forearm.

Employee was walking air courses and found the roof fall. We called the MSHA hotline at 9:20 A.M. We then cribbed and dangered the area off. Air was flowing over the fall, we pulled our crews outby to start corrective actions.

At 11:50 p.m. the roof fell in the #4 room between spad #'s 2+60 and 3+10. the fall was approximately 18 feet wide 35 feet long and 6 feet high. The area was cribbed off and the area dangered off. The fall did not impede ventilation.

The Employee went to climb inside of the #6 rail ride, his foot got stuck in the mud and he fell into the ride catching his left hand on the SCSR box. He lacerated his palm and received sixteen stitches.

EE was walking the MMU 001 belt line and he saw the top working at spad #4+20. EE kept all personnel out of the area and pulled the section Employees outby. The roof fell in on the beltline in #4 entry. EE placed floor to roof support around the fall and moved the section.

After helping his roof bolt partner straighten out a bent cable bolt, he was walking back to his side when he slipped and as he attempted to catch himself he placed his hand on the idle arm as his partner was running the drill pot up. At this time his hand got caught in the mechanism causing a crushing injury to his left index, middle and ring finger.

The employee stated that he had just placed a roof bolt in the head of the roof bolter and was pulling his arm back when a piece of shale 2'x3'x 2" fell hitting the Employee in the right forearm and hand, fracturing his 5th small metacarpal hand.

The owlshift crew called the dispatcher to leave for outside. The dayshift crew called the dispatcher to leave for underground. The day shift crew was held up and did not leave right when they had called the dispatcher. The dispatcher did not let either crew know that each crew was on the rail. The dayshift crew made 770 feet underground and collided with the owlshift crew.

The owl shift crew called the dispatcher to leave for outside. The day shift crew called the dispatcher to leave for underground. The day shift crew was held up and did not leave right when they had called the dispatcher. The dispatcher did not let either crew know that each crew was on the rail. The day shift crew made it 770 feet underground and collided with the owl shift crew.

The employee stated that he was cutting a metal band off a bundle of cable bolts that held them together. When he cut the strap, the band sprung up and lacerated his left forearm resulting in 5 sutures.

A continuous miner was hung up in a hole. The employee was cribbing the miner up to place cribs under the cats, one of the cribs popped out and hit him in the chin causing him to receive two stitches.

The mine was idle, the outside communications employee noticed he lost power at the #7 mine. EE went in to see what happened and found a roof fall at x-cut #16 spad (8+90). The fall was 8' high x 20'long x 25'wide. EE called the fall in at 1:38 A.M., we then cribbed around the affected area and then started re cable bolting the affected area.

Employee was the scoop operator, he was trying to pull a shuttle car off the feeder. He ran his scoop bucket on top of the shuttle car and the scoop dropped off the shuttle car and bounced employee inside the cab of the scoop, he hit his upper left rib cavity on the inside of the scoop cab causing a pulled ligament.

The operator thought his helper motioned him to advance forward, when the operator dropped the ATRS on the bolter the front end of the roof bolter dropped and the drill station canopy hit the helper on the head.

The intake phone would not work so the Foreman traveled the intake to locate the problem thats when he found a roof fall on the 1st East submains in the intake @ spad # 10+50. The fall started in #7 and fell to the edge of #6. We have two intakes. The fall was app 40' long, 18' wide,8-10' high.#7 is not our primary escapeway.

The #7 Mine uses bed ash to make roads with, it is similar to concrete. EE got bed ash on his pants and later got wet, he started operating a scoop. A short while after operating the scoop his legs broke out in a rash. He later went the Dr and the results were second degree burns.

Employee was helping bolter operators, when he reached over the bolter to mark an area on the roof with paint. The bolter operator on the opposite side moved the bolter inby to the next row to be installed and the helper had his foot in front of the bolter tire and the tire rolled on his foot.

The continuous miner operator was positioning the mining machine to remove rock from the top of the miner, when he thinks he must have tripped falling down onto the remote control box causing the tail boom to swing and the rock to slide off on him. The miner operator was not exactly clear on what occurred.

The employee was pulling a drill steel from the mine roof and his hand was on bottom of steel. When the steel came loose from the roof the employee's right pinky finger was smashed between the bottom of the steel and the roof bolter drill head. The employee's received a laceration which required 6 sutures, and a broken 5th Distal Phalanx.
