EE was holding the end of a grease hose on to a grease fitting when grease shot out and went through EE's glove and into the end of EE's finger. EE received 2 stitches in the finger where the doctor lanced it open to clean the wound.

EE was changing oil on a rock truck and was retrieving a ladder from under the truck. EE picked up the ladder and turned and hit the side of their head on the bumper.

Operator was loading trucks and noticed that high idle quit working then am/fm radio quit then CB quit, when they looked back they saw smoke coming from the engine compartment and tried to shut the machine off. The wire going to the engine shut off had burnt into also so they could not shut it down. Operator got off the machine and walked away

EE was standing on a D-11 Track pulling on a Hydraulic hose that is on a hose reel on our lube truck, when EE pulled on it the reel did not lock in place and it jerked the hose back and caused the EE to lose balance and slip and fall onto the push arm. Received 4 cracked ribs.

Employee was standing on front platform helping a mechanic bolt a light bracket on the lift cylinder of a D11 bulldozer after they had replaced Hyd. hoses. The employee lost balance and handhold causing EE to fall. Employee received a cut on the head that required stitches.

The technician was installing a roll pin into repair link for chain and a small piece of metal broke off and was embedded into upper left arm. The wound caused bleeding which an EMT was able to get stopped before technician was transported by ambulance to the local hospital for care.

Employee had hand on the roller of a nose cone, from a D11 side frame, while hoisting the nose cone to the ground for maintenance. While guiding the nose cone to the ground, the right side landed on the ground first. The left side had not yet landed which caused the strap to rotate unexpectedly pinching the employees thumb.

EE was in a hurry to get fire extinguisher from in back of his cab and slipped and fell from the cat walk. EE landed on his foot breaking bones in his ankle.

Employee was operating another's 2011 pickup alone. Truck required servicing to clear mud from transmission linkage preventing transmission from being put into park and normal operation. Employee stopped truck, crawled underneath to service linkage and the transmission engaged in reverse fracturing employees left arm while he attempted to get out of the way.

An EE was grading the haul road and was approaching the pit where our 345B trac hoe was working, he noticed that fire was coming out of the engine compartment. He informed the operator of the hoe and the hoe man stopped the engine and dismounted. There was no injuries.

Was in his rock truck getting loaded and doing cab cleaning. Noticed that the cover was missing off the air vent, thought it might have fell down into the dash. When he stuck his finger down in to feel for it, the fan blade took the tip of his finger. Received 3 stitches.

EE was fueling a rock truck at the end of his shift and stumbled over the hose and twisted his knee. He worked the rest of that week, and all of the next week, but on Monday morning of the 23rd he said he had to go have it looked at.

On the morning of the 13th EE was climbing down off D11 dozer, when he slipped and hurt his knee. Worked all week on it and on Monday the 20th reported it. Took off Tuesday the 21st and is still off.

Operator pulled his drill to the edge of drill bench to start drilling. When he put his mast up and turned the carousel to extract a drill stem, the stem became lodged. The operator pulled the drill up to allow room for the mechanic to work on it and the drill slipped off edge and fell on its side. No one Hurt !!!

Employee was working on 4400 TK bed. He was carrying a piece of steel, slipped on wet bed (from dew) and tried to catch himself from falling with his left hand. It bent his little finger backwards. Doctor said it tore tendons and for him to take two days off.

EE was standing on the lower level bumper area on the front of the Cat 777 End Dump and had placed his lunch bucket on the top platform and apparently blacked out and fell approx. 4 feet to the ground. This "accident" is under investigation as to possible medical conditions of ongoing concern. EE could have been on the ground and fell over.

EMPLOYEE WAS TURNING THE WATER TRUCK AROUND AT THE END OF THE ROAD WHEN A COAL TRUCK CAME UP ON HIM WHILE EMPLOYEE WAS SIDEWAYS IN THE ROAD SO EMPLOYEE COASTED BACK TO GIVE THE TRUCK DRIVER MORE ROOM AND HIS BACK TIRES FELL OFF THE ROAD AND INTO A SUMP HOLE AREA AND ROLLED OVER ON IT'S SIDE.
