Miner was attempting to move conveyor with chain slings and excavator. Chain slipped on conveyor causing it to swing. Miner attempted to stop swinging conveyor and was pinned between suspended conveyor and adjacent fixed conveyor.

Miner was changing screen cloth when screen shifted forward unexpectedly anc cut middle left finger.

Miner was stepping out of bed of service truck onto bumper, slipped and hit the ground on right shoulder. Miner was taken to doctor to be checked out and was issued prescription for anti-inflammatory.
