Cat 777 haul truck was going up the ramp to the dump site, the truck started backwards made a turn and landed on its side. The driver said he could not remember what happened, it is probable that he dozed off, he did not attempt to hit the brakes for 75'. Driver had a few scrapes on his left arm, he was treated at the er and released to full duty with no restrictions.

IW was installing a hydraulic hose on a D11N dozer. IW was under the dozer on his back, he layed the hose on the hydraulic box and it fell, striking him above the right eye. He received laceration of about one inch. The laceration was treated with four stiches.

EE BEGAN TO REPLACE ALINE ON UNIT #280 KOMATSU ROCK TRUCK, HE FAILED TO RELEASE PRESSURE IN THE LINE BEFORE STARTING THE REPLACEMENT. THE PRESSSURE FROM THE LINE BURNED AND INJURED THE SKIN.

EMPLOYEE WAS IN THE CONTROL ROOM WHEN HE FAINTED. FALLING TO THE FLOOR. HE CAME BACK TO CONSCIOUSNESS QUIKLY AND IMMEDIATELY STATED THAT HE WAS HAVING SEVERE CHEST PAINS. AT THAT TIME THE VINT ON CO. EMS WAS CALLED AND UPON ARRIVAL EXAMINED HIM AND ASSESSED HIS CONDITION WARRANTED FURTHERATTENTION. WAS TRANSPORTED TO HOSPITAL.
