After performing maintenance to the plant hopper the victim was climbing down off of the hopper. When he reached the ground he stepped on a piece of over sized gravel with his right leg. This caused his leg to twist and broke both of the lower bones just above his boot.

THE DRIVE BELT CAME OFF THE PULLEYS ONT HE DIESEL FUEL PUMP. AFTER REMOVING THE GUARD AND REPLACING THE BELT IT WAS SLIPPING WHEN TURNED ON. WHILE ATTEMPTIES TO TENSION THE BELT THE OPERATERS GLOVE WAS CAUGHT AND PULLED IN TO THE PULLEY. THE LEFT INDEX FINGER WAS BROKEN AND LACERATED JUST BEYOND THE NAIL.
