4-employees were performing preventative maintenance on the aerial tram system used to transport stone from the quarry to the plant. While tensioning the steel cable used to carry the tram buckets, the hydraulic cylinder assembly pulled loose from its anchor point. One employee was fatally injured and another suffered a broken leg, after being struck by components of the system.

Miner reported abdominal strain on 02/06/2012 while installing a door on the fluid bed dryer. Miner was seen by physician were an existing "old" hernia was found. On 02/16/2012 a physician deemed this to be a work related injury.

The victim was a truck driver for John R. Trucking Co. He was found unresponsive in the cab of his truck. 911 was called, CPR was administered by 2 EE's and an AED was brought to the scene. The AED was attached and instructions were followed. Hopewell TWP. EMT arrived shortly after and took over.

EE was operating tramway and had to reach down to secure a door on an incoming tram bucket.Doors on the tramway buckets are designed to latch automatically after they are emptied into the discharge silo;however,unlatched doors occur frequently due to rocks or sand in the latch mechanism. When EE reached to secure door, it caused a muscle strain in his lower back.

Employee was pushing buckets in on the empty side of the tramway, when the dispatcher released two buckets instead of one. Employee tried to stop the extra bucket by grabbing ahold of it and slid about 8 feet, trying to hold it back. Employee complained of shoulder/arm pain and sought medical attention. Employee has been off work since date of injury.

Employee was lifting empty sand bags onto platform by the bagger; as he lifted the bundle he felt his back lock up, his foot slipped and he fell to the ground. He stated that he hit is leg on either the guard or hand rail brusing his leg. He has also complained of back pain and sought medical attention. He did not loose any work time.

Employee was emptying a bucket near Tower 2; he was striking the side of a bucket with a sledge hammer to dislodge material. A small piece of rusted metal flew off the bucket striking the employee in the right lower leg causing a small laceration. The piece of metal had to be removed from the wound.

Employee was walking on lower floor level in the Process Bldg. and a small amount of acid from an abandoned overhead acid pipeline system dripped onto employee's right lower arm. The line has not been used for over 5 years. Employee suffered a 2nd degree burn on his right lower arm

Operator bent down to latch a door on a tramway bucket. As he bent down he felt a pain in his lower back; he closed the door and returned to the control room. The pain persisted so he went to a chiropractor. He returned to work the next day but was not able to complete shift and went to the ER. He was perscribed pain medication and was told to rest.

Employee was moving a 55 gal barrel of kerosene in the Process Bldg with a chain hoist. A clasp was hooked to the barrel edge and as it was moved from the pickup truck, the clasp released and the barrel dropped about two feet onto employees rt foot. Employee had a hairline fracture on top of foot; no medical treatment, just time off for swelling to go down.

Employee was making adjustments to feed system above bulk bagger; he asked other employee to test system. He informed operator he was clear of machine. As machine was started, his leg became caught between stationary and moving part of machine putting pressure on lower leg. Employee had a stress fracture to his right ankle.

Operator was on walkway at creek pump to clean screen. the walkway frame broke & the operator grabbed the railing to keep from falling. At that point he felt a pull in his rt. shoulder. He did not see dr until 3-3-05 & has not lost time. Amendment: After 6 months the dr determined that surgery was reqd to repair a torn ligament. This was not reportable till he reqd surgery 10/13/05

EE WAS SWEEPING FLOOR OUTSIDE TERMINAL AND STEPPED ASIDE TO LET LOADED BUCKET PASS, STEPPING ON ROCK CAUSING THE ANKLE STRAIN. HE FELL TO THE GROUND AND WAS STRUCK IN THE ARM BY THE PASSING BU CKET, CAUSING A PUNCTURE WOUND RESULTING IN TWO STITCHES TO HIS ELBOW.
