Employee was changing two troughing idlers. The first one was set into place. When the second was set, it caused the first to flip over striking the employee's arm. Injury was a laceration to the right wrist that required stitches.
Employee caught his right middle finger between the basket of the JLG Lift that he was operating and a conveyor frame, fracturing the tip of the finger.
Slipped and fell while walking over hard packed snow.
Mechanic was trying to secure a tarp on an excavator. The wind was whipping and he lost his balance and fell on the ground. He landed on a previously injured knee (right). It was reported to the company immediately. Safety staff picked him up to be seen by a doctor. It was diagnosed as a fractured patella. He will see a specialist on Thursday at 9:15 a.m.
While changing screen cloth he was using a bar to position the screen panels. The screen above slid down and struck the bar bending his wrist backward. Original diagnosis was a sprain/strain of the wrist. The employee was diagnosed with a fracture on November 27, resulting in lost time.
The employee was removing a liner bed from a conveyor when it slipped. He tryed to catch it and felt a "pop" in his wrist and elbow.
An employee was attempting to remove a rock from the tailpulley of the C-3 Conveyor. The injured employee reached into the tailpulley just as another employee jogged the belt. Lock-Out Tag-Out was not used.
Claims that he injured his back while carrying boxes of parts into the parts storage trailer. This claim is currently being litigated.
CLAIMS BOUNCING AROUND WHILE OPERATING BOBCAT SKIDSTEER LOADER INJURED HIS BACK. THIS CLAIM NOT REPORTABLE UNTIL 9/24/03 WHEN EE MISSED WORK. CLAIM IS CURRENTLY BEING LITIGATED.
CLAIMS HE FELL BACKWARDS WHILE GETTING OUT OF BOBCAT AND INJURED RIBS.
WAS REPLACING A GURARD ON A CRUSHER. A BRACKET SLIPPED DOWN AND STRUCK HIM ON THE FACE. REPORTABLE BECAUSE OF CHIPPED TOOTH.
ROCK CAME OUT OF CRUSHER AND HIT MINER IN MOUTH. MINER WAS USING ROCK BREAKER TO DRAG STONE INTO CRUSHER WHEN ROCK CAME OUT. CHIPPED THREE (3) TEETH.
WAS HANDLING BUNDLE OF ELECTRICAL CABLE WITH ANOTHER EE. OTHER EE DROPPED HIS END. THIS CAUSED INJURED TO JERK ABDOMEN AND LEFT ELBOW. THIS ACCIDENT NOT REPORTABLE UNTIL LOST TIME OCCURRED ON 6/4/03.
CLIMBING DOWN OFF OF SEMI TRAILER, GLOVE CAUGHT FRACTURING LEFT PINKY FINGER.
SLIPPED ON ICY CATWALK, INJURING LOW BACK AND RIGHT SHOULDER. LOST TIME MAKES THIS REPORTABLE.
