Cut left hand while breaking down boxes

EE was prying on a mechanical lacing clip. EE lost grip of bar, the bar then bounced off of the belt coming towards EE and hit EE in the mouth resulting in a chipped left central incisor.

Employee was trimming the skirt board on C-06 with the grinder and lost control of the grinder resulting in a cut to hand.

EMPLOYEE WAS STEPPING DOWN OFF THE BUMPER OF THE MAINTENANCE TRUCK ON TO LEVEL GROUND AND SPRAINED RIGHT ANKLE.

The employee was moving an 8'x19'x1/2" sheet of steel assisted by a telehandler. The employee tried to slide the sheet of the forks in a pulling motion then felt a pop in the left bicep.

Miner was installing an impact roller on C-12, Miner was applying force to the roller with right hand and bracing self with left hand. When Miner released right hand the roller came in contact with the Miner's left hand causing a break to thumb and hand to become bruised. The Miner was wearing all required PPE including gloves per Company Policy.

Loader operator was closing the door of the cab after entering the machine and closed EE's thumb in the door.

While installing a cheek plate on CR4 employee stepped into an opening on the platform which material is washed into. Employee reported lower back pain.

Employee lifted box from desk top moved to short file cabinet surface of the same level, twisting in the process. Pulled muscle in back, limited lifting for 5 days then released to regular work.

Employee was changing an alternator belt on a surface haulage truck. He was releasing the tension a spring tensioner with a pry bar. In the process he lost control of the pry bar causing the pry bar to strike his left index finger between the pry bar and the frame causing a laceration and minor fracture

Employee was cutting old conveyor with torch, as he cut a piece of angle the angle had unseen stored energy causing it to strike the employee's right hand causing a cut and swelling in the knuckle.

A foreign object entered employee's right eye

He was removing access panels to remove a turbo charger from a wheel loader and heard and felt a pop in his arm. He reported the incident to his manager immediately. He and his manager continued to monitor his condition until 02/12/14 when he informed his manager that the pain had not improved and he wanted to seek medical treatment.

Employee exited equipment improperly, not maintaining three point contact. Upon exiting employee twisted ankle. Employee was diagnosed with a distal fibular fracture and a torn tendon.

An employee tripped over a piece of cut railroad track that is used as weight in a counterweight box. When the employee tripped, he fell forward and tried to catch himself on a 10" pipe. When he reached for the pipe, he hyper-extended his right shoulder and felt a "pop".

Employee began exiting a chute after inspection when his hand slipped and he fell backwards against the side of chute, striking his back against one of the steps. The employee did not report the injury until 06/18/2011, the employee got out of bed, stretched, and felt an extreme amount of pain. Employee sought treatment and was diagnosed with a fractured rib.

Employee reported that while cutting rubber for a screen deck, the knife slipped and lacerated his right inner thigh. Employee went to the hospital where he received 7 sutures, was given a Tetanus shot, and was not given any written prescriptions or restrictions. Employee is to return in 10 days for suture removal.

Employee alleges that he struck his knee while moving around in the cab of a piece of equipment. The employee failed to report the injury until 11/14/2010 and then sought treatment with his personal doctor on 11/29/2010 where he was diagnosed with a torn meniscus, received a Cortisone shot, and was prescribed two (2) days off from work.

At the wet mill doing a belt splice on conveyor 33, repetitively climbing on and off the belt during the repair and twisted his right knee.

Replacing a return roller on C-45 conveyor. Attempting to put the return roller into its holder. Bending & stretching while lifting caused pain in lower back. **Medical with restrictions.**

Stepping down ladder of T-8 haul truck, bolt broke holding a rung on, EE held on with his left hand but fell down a couple of rungs to radiator shroud.

While pulling a conveyor belt (c-45) down with a come-along, the come-along released by itself, this allowed the belt & roller to push EE's hand upward, traping it between the roller & bracket.

Step down from piece of equipment (manlift), proper PPE employed, twisted right foot/ankle when stepping onto Wet Mill Screen catwalk. No lost time - light duty assigned - prescription given.

While driving a 775 haul truck, Employee says he messed his lower back up due to rough roads at south bench near primary crusher. *Restrictions*

Employee lifted railroad ties, then lifting iron bar to place under ties, felt pain in R. elbow. Use of sling & no lifting @ elbow 7 days.

WHILE GETTING OUT OF CRANE, EE SLIPPED ON CRANE DECKING AND GRABBED CRANE HANDRAIL TO PREVENT FALLING, PULLING HIS RIGHT SHOULDER. RESTRICTED DUTIES.

EMPLOYEE WAS CUTTING AWAY OLD METAL & WHEN HITTING IT WITH A HAMMER, MATERIAL GOT INTO HIS RIGHT EYE.

Swinging 3 lb. hammer replacing U-Bolt on Wet Mill Screens Spray Bars. Right wrist is hurting, sharp pain. Went to Hospital (ER) - 3 days of light duty. No lost time.

EMPLOYEE WAS CUTTING TAPE OFF OF A 5/0 CORD ROLL & CUT HIS LEFT HAND WITH A UTILITY KNIFE.

In #1 dry mill electrical room, ee was testing voltage in main breaker for transformer that feeds the substation. when using voltage meter, electrical flash caught ee on left hand. he was transported to hospital er by fire dept. rescue squad. some skin removal and perscrip given.

AT THE PRIMARY CRUSHER, EE SMASHED & CUT SMALLEST FINGER ON HIS RT. HAND MOVING A ROCK.

WORKING AROUND CRUSHER #3 AT DRY MILL. A PIECE OF DIRT BLEW OFF THE CRUSHER WHILE OPENING IT AND WENT IN HIS LEFT EYE. (FOREIGN BODY REMOVAL - SOME RESTRICTIONS).

WHILE WASHING OUT HEAD END OF C-19 WITH HOSE, EE SLIPPED AND FELL HURTING LEFT WRIST & LEFT UPPER RIB CAGE.

EE WENT TO WASH OUT C46 TAILPULLEY. EE ENTERED THE AREA WITHOUT DOING A PROPER SAFETY INSPECTIONBEFORE ENTERING THE AREA. WHILE WASHING THE TAILPULLEY, EE WAS NOT PAYING ATTTENTION TO THE AREA HE WAS IN AND FELL INTO A HOLE IN CATWALK WHERE A BOARD WAS MISSING.

SLID DOWN THE FRAME OF C-19 BELT & STRUCK KNEE ON CHANNEL IRON. NO KNEELING - 4 WEEKS RECIEVED RESTRICTIONS FROM DOCTOR ON 8-22-03

WHILE MOVING A 55 GAL DRUM, PULLED BACK ON DRUM & PINCHED PINKY FINGER ON LEFT HAND. **SPLINT REQUIRED**

EMPLOYEE WAS ATTEMPTING TO MOVE A WEAR PLATE IN SHUTE OF WEST WETMILL SCREEN. THE PINCH BAR HE WAS USING SLIPPED OUT TWISTING HIS RIGHT ARM. HE WAS TAKEN TO E.R. SLING FOR IMMOBILIZATION WAS USED.

WHILE CLEANING THE JUNKYARD BEHIND THE SHOP, HE WAS PICKING UP A PIECE OF WOOD & AN OAK PALLET. THE TWO PIECES SEPARATED & THE PALLET SWUNG DOWN & BROKE HIS FINGER BETWEEN THE TWO OBJECTS.

WHILE REPLACING A PULLEY IN GALLEY #2, THE PIPE HOLDING THE BELT BACK BROKE, PIPE FLEW, STRIKING LEFT KNEE AREA. THIS CAUSED A CONTUSION AND STRAIN. DOCTOR GAVE 4 DAYS OF LIGHT DUTY WORK.

WORKING UNDER A TRUCK FIXING THE EXHAUST SYSTEM. RUST FELL INTO RIGHT EYE.

WAS WORKING TO GET SNUB PULLEY INTO POSITION, IT HAD BECOME PINCHED UNDER THE BELT. WHEN EE REACHED IN TO GET THE PULLEY, IT MOVED TOWARDS HIM & PINCHED HIS FINGERS ON LEFT HAND. SPLINT REQUIR ED.

WHILE DIGGING OUT #3 SECONDARY CRUSHER, WENT TO THROW A ROCK AND HIT THE SIDE OF THE CRUSHER, PINCHING HIS FINGER.

WHILE BACKING OUT OF A HOPPER, STRUCK LOWER BACK ON CORNER OF THE CHUTE.

WHILE HEADING DOWN STEPS TO RETRIEVE TOOLS, TRIPPED ON GRATING & FELL DOWN THREE GRATED STEPS. RECEIVED EYEBROW LACERATION FROM HIS GLASSES STRIKING HIS FACE. RIGHT EYEBROW. R. WRIST SORE. *ST ITCHES REQUIRED.*

CUTTING A SKIRT RUBBER WITH A STANLEY KNIFE. WAS PULLING THE KNIFE TOWARDS HIS RIGHT LEG. KNIFE WENT THRU RUBBER AND THEN STRUCK HIS RIGHT LEG.

WAS CLEANING OFF BOLTS AT THE END OF DISCHARGE END OF THE EANGLE SCREW. GUST OF WIND BLEW SOME DIRT INTO HIS EYE.
