While employee was moving from the top of one rail car to the other, mis stepped and fell between cars, approx. 3ft. pulled self back up onto the railcar. continued on with work for remainder of shift. Reported to manager in morning.

Undetermined. Team member (T/M) felt irritation in left eye Sat. Nov. 19 approx. 4:00pm. Could not attribute to anything but performing grinding on Fri. Nov. 18th. approx. 5pm. Investigation showed t/m was following all safety expectations including elevated PPE requirements. T/M also stated there was no indication of irritation during or after completion of work.

Working from 5 step rolling ladder, Team member (t/m) attempted to close hopper hatch to control release of fine material. T/M indicated hatch may have made contact with face, but cannot remember. We believe t/m may have jumped from ladder, causing foot injury, then while attempting to flee area, made direct contact with a structural crossmember injuring EE's face.

Team member was raking lime coating out from kiln when rake got caught on grate. This sudden stop of motion caused T/M to lose balance and fall onto buttocks. A hot piece of lime pebble flipped up and down backside T/M's undergarment, causing a small burn to buttocks.

Employee was clearing out a plug up from the kiln feed hopper through a side hatch cover. As the employee was performing this task, hot debris came out of the hatch cover dusting the employee resulting in burns to the employee's forearms.

EE was moving the hood rake to sweep the floor upon moving the rake, loose dust from above fell down and got in between the safety glasses effecting only the right eye. EE was treated for the injury by a Dr. on 2/1/18 and received a Rx for eye drops to treat the eye.

The kiln operator received an interrupt alarm that #2 Kiln stone feeder weight conveyor belt had stopped. The Utility attempted to reset the belt at the ground level MCC VFD with no success. By the time the Utility made it up to the belt to investigate the issue it was already completely engulfed in flames.

EE was tasked with opening a lime chute slide gate. The gate was stuck and EE had to use a sledgehammer to work the slide back & forth to get it unstuck. While doing this EE's right wrist started to hurt. The pain continued for a few days when EE was treated @ the local Occ. Health for a wrist sprain. Dr. Rx modified duty, splint, and Rx meds.

EE had finished setting a handbrake from the railcar platform. During the transition from the end ladder to the side ladder his left foot slid across the foot rung causing him to lose his balance and grip on the ladder. He fell back 3ft and as he hit the ground he put his right hand out to break his fall. The employee suffered a broken wrist.

Maintenance was patching holes by spot welding to the bottom of an enclosed belt conveyor. The heat from welding transferred through the 3/16" thick steel substrate to the UHMW lining and to the rubber cleated belt with sidewalls causing it to ignite and catch fire.

EE strained his back while removing debris from the kiln. Pieces of quad foil that had broken off from the main structure were lifted into wheel barrows and transported to the kiln entrance. EE was finishing unplugging a vac hose. He walked over to disengage the clutch after he did this he backed up to get a better view of the vac hose. As he backed up he tripped over the hitch trailer for the vac unit which was sitting on the ground next to the unit. As he fell he put his hand down to catch himself injuring his wrist.

EE was carrying box of light bulbs from the warehouse to the back of his truck. As he was carrying the box, he tripped over a screw that was in the isleway outside of the warehouse between the electrician shop and the warehouse door. He fell to the ground and said he felt the wind was knocked out of him. Become recordable injury when on 8/25/10 a CT revealed fractured ribs.

EE was moving a piece of plywood, approx 25 pounds, he sat the plywood down and stated that he had received an electrical shock. The area was looked over and it was found that there was no cords in the area, he stated that he wanted it recorded that he thought he had received a shock.

Note:Report is being filed as a precaution. A fire developed in the exhaust stack of the 3744 Locomotive, that lasted 15 mins. and was blown out by throttling engine up. Approximately 10 min. period of time past without a fire. Then a fire started again in the exhaust stack area and was extinguished by local fire dept. using 1 extinguisher. The second fire burned for 15 mins.

At approx 0600 the chute coming off of E231 plugged. At some point it appears that the V-belts on C132 had burnt off & the belt had stopped allowing hot material to sit on the belt & build more heat. The material was able to get to C132 due to a rotted gate in the C231 to C132 discharge chute. The Fire was found and reported @ 0615 by Kiln Operator.

EE was doing railcar moves as the switch man. He was riding on the locomotive.As he dismounted the locomotive he slipped in mud,fell to the ground.EE has a sore shoulder and abrasion on his shoulder,no medical treatment was given. The shoulder continued to be sore for a long period of time.EE finally saw a doctor and now has received surgery on 9/19/08.

EE was opening a 3 ft by 8 ft fiberglass top railcar lid. He raised the lid and it became stuck. The employee went around hitting the door with a hammer to break the ice free. He then went back to the lid & gave a hard jerk; the lid became stuck again. At that time the employee felt a twinge and pain in his lower back. Rx Pain Drugs and Modified Duty.

The maintanence crew was replacing an electric motor. The motor was lined up with two of the bolts and the motor was suspended approximately 2 inches from the base. The EE proceeded to reach between the motor and the base to line up the support bolts. As his hand was under the motor, the motor fell crushing three of his fingers. Rx Drugs, Stiches.

EE was opening railcar lids and was using an air wand to clean out a car to load. When the employee went to lift the air wand out of the car he lost his footing and fell across the open access door. He struck his left shin on the catwalk of the railcar. Employee recieved medical treatment at hosp. - 9 stiches and Rx Drugs.

EE went to spray out the top of the chute from 5/7 ft away from the chute door with a water gun. As he was spraying a back pressure blowout occured, shooting hot mud & lime towards the EE and hitting him in the face. The EE was treated for 1st & 2nd degree burns & was given Rx medical ontiment & Work Restrictions.

Employee sustained an injury to his knee. The employee was climbing into a bobcat to perform cleanup activities around the Truck Load Out building. As he entered the bobcat his right foot slipped out from under him causing the right knee to strike the metal frame of the bobcat. The employee's knee had swelling and a bruise. The doctor gave one day of no work.

While going to check the oil on the locomotive, there was oil spilled previously and followed by dry lime to soak it up. It rained previously a couple of times turning it into a slick mess. When the employee went to check the oil he stepped in it and slipped off the side hitting his knee on the deck as he went off the platform. Employee recieved Rx Drugs and Restrictive Work.

EE had just completed changing an air lock key from an elevated platform. When the EE started to descend the ladder from the platform, the ladder twisted under his feet causing him to fall approximately 7 feet to the concrete floor. The EE sustained a lost time injury to his back, ribs, and knee.

The employee was on top of a railcar, grabbing a lime sample with a 5 gallon bucket. The employee lost his balance and fell through the access door of the railcar. The employee fell approximately 3 feet onto the top of the lime in the railcar. The employee breathed in the lime dust and received chemical burns in his esophagus.
