Maple Grove Operations Metal/Non-Metal

Undetermined. Team member (T/M) felt irritation in left eye Sat. Nov. 19 approx. 4:00pm. Could not attribute to anything but performing grinding on Fri. Nov. 18th. approx. 5pm. Investigation showed t/m was following all safety expectations including elevated PPE requirements. T/M also stated there was no indication of irritation during or after completion of work.

Working from 5 step rolling ladder, Team member (t/m) attempted to close hopper hatch to control release of fine material. T/M indicated hatch may have made contact with face, but cannot remember. We believe t/m may have jumped from ladder, causing foot injury, then while attempting to flee area, made direct contact with a structural crossmember injuring EE's face.

Team member was raking lime coating out from kiln when rake got caught on grate. This sudden stop of motion caused T/M to lose balance and fall onto buttocks. A hot piece of lime pebble flipped up and down backside T/M's undergarment, causing a small burn to buttocks.

Employee was clearing out a plug up from the kiln feed hopper through a side hatch cover. As the employee was performing this task, hot debris came out of the hatch cover dusting the employee resulting in burns to the employee's forearms.

EE was trouble shooting a plugged lime handling elevator when lime from above fell causing lime dust to puff out and get in both eyes. Injury - Chemical Burn / Ophthalmologist treatment /Rx Eye drops.

EE was moving the hood rake to sweep the floor upon moving the rake, loose dust from above fell down and got in between the safety glasses effecting only the right eye. EE was treated for the injury by a Dr. on 2/1/18 and received a Rx for eye drops to treat the eye.

Employee was walking on clean hard surfaces (asphalt & concrete) from one end of the plant to the other and slipped on ice when between the pugmill and concrete silos. Medical Treatment for Ankle Fracture.

The #2 Kiln Cooler Conveyor Belt was discovered to be on fire by the kiln operator. When discovered the belt was completely engulfed in flames.

The kiln operator received an interrupt alarm that #2 Kiln stone feeder weight conveyor belt had stopped. The Utility attempted to reset the belt at the ground level MCC VFD with no success. By the time the Utility made it up to the belt to investigate the issue it was already completely engulfed in flames.

EE was tasked with opening a lime chute slide gate. The gate was stuck and EE had to use a sledgehammer to work the slide back & forth to get it unstuck. While doing this EE's right wrist started to hurt. The pain continued for a few days when EE was treated @ the local Occ. Health for a wrist sprain. Dr. Rx modified duty, splint, and Rx meds.

Cleaning a screen and chute with the employee strained their shoulder. Occ-Health Treatment on 3/6/17. Treatment plan included Rx Meds and Restricted Duty

EE had finished setting a handbrake from the railcar platform. During the transition from the end ladder to the side ladder his left foot slid across the foot rung causing him to lose his balance and grip on the ladder. He fell back 3ft and as he hit the ground he put his right hand out to break his fall. The employee suffered a broken wrist.

Victim was moving skid steer in tight quarters and pinned himself between skid steer and column

Employee was walking from office to the warehouse when she slipped on ice and broke her ankle

EE was moving a vibrator from the pocket of one railcar to the the next for transloading material from railcars to trucks when he strained his back. EE did not seek medical treatment until 6/30/14 where he was treated with Rx meds, time off, and modified work restrictions.

While doing a team lift with 3 others on an awkwardly sized piece of lab equipment the employee felt a pop in his right knee as they lowered the piece of equipment to the ground after lifting it off of the counter. EE received Rx modified duty.

EE got chemical lime in his right eye when cleaning out the cooler bin. EE had on all required PPE on at the time of injury. EE was given Rx eye drops at hospital.

Maintenance was patching holes by spot welding to the bottom of an enclosed belt conveyor. The heat from welding transferred through the 3/16" thick steel substrate to the UHMW lining and to the rubber cleated belt with sidewalls causing it to ignite and catch fire.

EE was cleaning pugmill when he reached in the pugmill with sledge hammer to clean the paddles and swung sledge. He felt sharp spasm of pain on right side of back. First time employee received medical treatment was 6/1/13. He received work restrictions.

EE was traveling around the west end of RLO when he decided to walk over a 3' berm prior to crossing the tracks. As he walked over the berm he stepped in a hole on the down side of the berm "rolling" his left ankle falling to the ground. Medical treatment @ ER - Time Off with Restrictions.

EE strained his back while removing debris from the kiln. Pieces of quad foil that had broken off from the main structure were lifted into wheel barrows and transported to the kiln entrance. It is believed the injury occurred tossing pieces of this material over the discharge dam. Medical Treatment Started on 1/11/13 - Rx Med / Restricted Duty

Moving glycol barrels to separate out a single barrel to move it with the fork lift. Trying to spin barrel off skid pinching left index finger between the other barrels. Felt pain on left hand finger, removed his glove found left index finger bleeding. Require stitches at hospital.

EE was raking kiln grate when his left pinky finger was pinched between the door frame and kiln rake handle. EE was wearing heavy leather welding gloves at time of injury. Finger Surgery on 1/31/12

Employee was lowering the Beck unit down from on top of the kiln hood when it was caught on the lip of the hood. Another employee unsnaged the unit, but by doing so it jolted the employee forward and his harness caught him causing whiplash.(RX Medication and Restricted Duty)

EE was finishing unplugging a vac hose. He walked over to disengage the clutch after he did this he backed up to get a better view of the vac hose. As he backed up he tripped over the hitch trailer for the vac unit which was sitting on the ground next to the unit. As he fell he put his hand down to catch himself injuring his wrist.

EE was carrying box of light bulbs from the warehouse to the back of his truck. As he was carrying the box, he tripped over a screw that was in the isleway outside of the warehouse between the electrician shop and the warehouse door. He fell to the ground and said he felt the wind was knocked out of him. Become recordable injury when on 8/25/10 a CT revealed fractured ribs.

EE was moving a piece of plywood, approx 25 pounds, he sat the plywood down and stated that he had received an electrical shock. The area was looked over and it was found that there was no cords in the area, he stated that he wanted it recorded that he thought he had received a shock.

Note:Report is being filed as a precaution. A fire developed in the exhaust stack of the 3744 Locomotive, that lasted 15 mins. and was blown out by throttling engine up. Approximately 10 min. period of time past without a fire. Then a fire started again in the exhaust stack area and was extinguished by local fire dept. using 1 extinguisher. The second fire burned for 15 mins.

At approx 0600 the chute coming off of E231 plugged. At some point it appears that the V-belts on C132 had burnt off & the belt had stopped allowing hot material to sit on the belt & build more heat. The material was able to get to C132 due to a rotted gate in the C231 to C132 discharge chute. The Fire was found and reported @ 0615 by Kiln Operator.

Employee was passing brick out of kiln opening to brick saw operator. He dropped brick and tried to catch it. Pinching finger against steel plate causing him to cut finger. Left hand index finger.

EE was doing railcar moves as the switch man. He was riding on the locomotive.As he dismounted the locomotive he slipped in mud,fell to the ground.EE has a sore shoulder and abrasion on his shoulder,no medical treatment was given. The shoulder continued to be sore for a long period of time.EE finally saw a doctor and now has received surgery on 9/19/08.

Employee suffered a back strain. The employee was lifting a 200 pound air powered diaphragm pump from the cooler pit when he felt a sharp in his lower back. Went to hospital and recieved Rx Drugs and localized shot for pain.

EE suffered lime burn to both eyes.The EE & his co-worker were raking refractory bricks out of the kiln hood.Was wearing the required face shield/safety glasses, he felt a sharp burning pain in both of eyes.He immediately stopped & flushed eyes out.EE then went to ER & was given Rx Eye Drops & saw a specilist that Monday.

Employee was inspecting around the stone screen when he stepped on some large stones. The large stones rolled when he stepped on them causing his knee to twist. Rx Drugs, Knee Brace, & Modified Duty.

EE was opening a 3 ft by 8 ft fiberglass top railcar lid. He raised the lid and it became stuck. The employee went around hitting the door with a hammer to break the ice free. He then went back to the lid & gave a hard jerk; the lid became stuck again. At that time the employee felt a twinge and pain in his lower back. Rx Pain Drugs and Modified Duty.

Employee was shoveling around the kiln hood when he felt pain/cramping in his lower back and legs. Employee stopped shoveling and went to ER where he was given Rx Drugs and was not allowed to come back to work for 2 days.

Employee was moving the railcar unloader and when the employee went to rehang the 60 pound railcar vibrator he heard a pop and had a sharp pain in his mid and lower back. Employee recieved medical treatment at local ER, was given Rx Drugs and work restrictions.

The maintanence crew was replacing an electric motor. The motor was lined up with two of the bolts and the motor was suspended approximately 2 inches from the base. The EE proceeded to reach between the motor and the base to line up the support bolts. As his hand was under the motor, the motor fell crushing three of his fingers. Rx Drugs, Stiches.

EE was opening railcar lids and was using an air wand to clean out a car to load. When the employee went to lift the air wand out of the car he lost his footing and fell across the open access door. He struck his left shin on the catwalk of the railcar. Employee recieved medical treatment at hosp. - 9 stiches and Rx Drugs.

EE went to spray out the top of the chute from 5/7 ft away from the chute door with a water gun. As he was spraying a back pressure blowout occured, shooting hot mud & lime towards the EE and hitting him in the face. The EE was treated for 1st & 2nd degree burns & was given Rx medical ontiment & Work Restrictions.

While the employee was going to work on a belt he was descending some steps. Two steps from bottom the employee heard a "pop" in his knee and went down. Medical Treatment - Strain/Sprain Leg-hip and thigh, Rx Drugs and Lost Time.

EE was carring lances from the #2 cooling tower to the #1 cooling tower. When he bent down and started to pick up the second lance he felt a sharp pain in his lower back area. Medical Treatment - Lumbar Strain, Rx Drugs and Light Duty.

EE was working in a cooler pit and watery slurry lime got on his pants soaking through to the skin of his legs. The employee's rubber boots continued to rub his legs. Medicial Treatment - 2nd Degree Chemical Burns to Legs - Rx Drugs & Light Duty.

Employee sustained an injury to his knee. The employee was climbing into a bobcat to perform cleanup activities around the Truck Load Out building. As he entered the bobcat his right foot slipped out from under him causing the right knee to strike the metal frame of the bobcat. The employee's knee had swelling and a bruise. The doctor gave one day of no work.

Employee was shovling underneath a conveyor belt when he felt a sharp pain in his back. The employee want to hospital where they gave him Rx Drugs and light duty.

Employee was cutting box open with utility knife. As the final cut was made the employee's hand slipped and cut his other hand (left index finger). Employee recieved 3 stitches at the hospital.

While employee was shoveling lime material at the boot of an elevator, the lime material puffed out of the elevator and got into his eyes. Employee recieve Rx eye drops.

While going to check the oil on the locomotive, there was oil spilled previously and followed by dry lime to soak it up. It rained previously a couple of times turning it into a slick mess. When the employee went to check the oil he stepped in it and slipped off the side hitting his knee on the deck as he went off the platform. Employee recieved Rx Drugs and Restrictive Work.

EE had just completed changing an air lock key from an elevated platform. When the EE started to descend the ladder from the platform, the ladder twisted under his feet causing him to fall approximately 7 feet to the concrete floor. The EE sustained a lost time injury to his back, ribs, and knee.

EE turned ankle in yard which caused him to fall. When catching himself with hand he dislocated / fractured two if his fingers. EE was given Rx, Light Duty, and Rehab.

EE was lifting/pulling baghouse bags and cages out of a baghouse cell when he felt a muscle pull in his stomach area. EE was not seen by a doctor until 3/11/05 and was given Rx drugs and no work.

EE WAS USING THE BOBCAT LOADER TO PUSH UP PILES OF LIME DUST, WHEN THE WIND BLEW JUST RIGHT AND PARTICLES OF LIME DUST ENTERED HIS RT. EYE. THE EE RECEIVED MEDICAL TREATMENT, RX EYE DROPS, AND MISSED ONE DAY OF WORK DUE TO HIS INJURY.

The employee was on top of a railcar, grabbing a lime sample with a 5 gallon bucket. The employee lost his balance and fell through the access door of the railcar. The employee fell approximately 3 feet onto the top of the lime in the railcar. The employee breathed in the lime dust and received chemical burns in his esophagus.

IT WAS A WINDY DAY & THE EE WAS WALKING OUTSIDE FROM THE WAREHOUSE TO THE MAINTENANCE SHOP WHEN SOME DUST/LIME GOT INTO HER EYE. THE EE WAS WEARING HER SAFETY GLASSES AT THE TIME. THE EE'S EYE WAS FLUSHED AT WORK THEN TREAT/FLUSHED AT A MEDICAL FACILITY. THE MEDICAL FACILITY GAVE THE EE A RX FOR EYE DROPS AND PAIN PILLS.

THE EE WAS WALKING PAST A KILN THAT WAS PUFFING WHEN SOME LIME DUST GOT INTO HIS EYE. THE EE RECEIVED MEDICAL TREATMENT WITH RX EYE DROPS.

EE WAS WALKING DOWN A STAIRCASE WHEN HE TRIPPED OVER A 6 INCH VAC HOSE THAT WAS ACROSS THE TOP OF THE STAIRS. THE EE FELL ALL THE WAY TO THE BOTTOM OF THE STAIRS ONTO THE CONCRETE. THE EE HAS A BRUISED SPINAL CORD WITH LOST WORK DAYS.

EE WAS REMOVING HIS PERSONAL PROTECTIVE EQUIPMENT TO CLEAN HIMSELF UP AFTER WORKING IN LIME DUST AREA. AS THE EE REMOVED HIS HARD HAT THE LIME DUST THAT HAD ACCUMULATED ON IT FELL DOWN INTO HIS EYES.

EE WAS SHOVELING A LIME SAMPLE OUT OF A RAIL CAR WHEN HE FELT A SHARP PAIN IN HIS ELBOW.

PINCHED FINGER BETWEEN SLEDGE HAMMER HANDLE AND THE TRACK OF THE RAIL CAR'S BOTTOM DROP DOOR.

SHOULDER PAIN AS A RESULT OF FALL WHEN HAND HELDPRY BAR SLIPPED. ALTHOUGH THE INJURY OCCURRED ON 5-7-03, I WAS NOT AWARE OF THE INJURY UNTIL 5-21-03, AND HENCE THE REPORTING IS LATE.

EE TWISTED LEFT KNEE WHILE WALKING AND STEPPING ON SECTION OF CATWALK THAT WAS NOT LEVEL WITH THE FLOOR.

RESULTANT LEFT ELBOW STRAIN AS EE PUSHED ON WRENCH USED TO FREE A SHAFT.

EMPLOYEE EXPERIENCED SHARP PAIN IN LOWER BACK AS HE TURNED IN HIS OFFICE CHAIR.

ANKLE SPRIAN AS RESULT OF EMPLOYEE STEPPING ON LUMP OF COAL WHILE DESCENDING STAIRS. LOST TIME.

RESULTANT BACK PAIN SUSTAINED DURING SNOW/ICE RELATED SLIP AND FALL UNTO CATWALK ON TOP OF RAILROAD CAR BEING LOADED WITH LIME PRODUCT.

RESULTANT LEG PAIN WHEN THE INJURED USED HIS LEFT FOOT TO PUSH ON A SHOVEL DURING CLEANUP NEAR A BELT CONVEYER.

CLOSE WORKING CONDITIONS INJURED WAS CLEANING UNDER A BELT AND TWISTED HIS BACK AS HE CLEANED UP MATERIAL

INJURED WAS USING A SLEDGE HAMMER TO KNOCK MATERIAL OFF OF COLLING TOWER. HIS FINGER ON HIS RIGHT HAND WAS CAUGHT BETWEEN THE COOLING TOWER & SLEDGEHAMMER HANDLE.

EE RECEIVED FLASH BURNS TO EYES WHILE ARC GOUGING WELDS. EE WAS WEARING PROPER PPE ON EYES. IT IS SUSPECTED THAT REFLECTIONS OF THE ARC CAUSED THE CONDITION. EE WAS SENT TO THE DR. AND DIAGNO SED W/ULTRAVIOLET KERATITIS & RECEIVED INJECTIONAND PRESCRIPTION OF TETRACAINE.

EMPLOYEE WAS GOING DOWN THE STEPS TO THE COKE BASEMENT AND TRIPPED ON A DRAIN HOSE.

EE REAGGRAVATED PAST INJURY WHILE PERFORMING NORMAL JOB FUNCTION. EE WAS CLOSING A RAIL CAR & WAS BENT OVER WHEN HE BEGAN COUGHING WHEN HE FELT PAIN EE WAS TAKEN TO HOSPITAL .PAST INCIDEN T WAS NOT MSHA REPORTABLE. COUGHTIN REFLEX & EE POSTION CONTRIBUTED TO ACCIDENT

EE WAS USING A UTILITY KNIFE TO OPEN A BUCKET. THE KNIFE SLIPPED AND SHE CUT HER ARM. EE RCVD. 3 STITCHES AND RETURNED TO WORK. INJURY NOT REPORTED TO SAFETY DEPT. UNTIL 2-28-02..

LIGHTING ROTARY KILN-BACKFLASH CAUSED 2ND DEGREE BURNS & BLISTERS TO RIGHT FOREARM AND SHOULDER (BURNED OUT SHIRT OFF ARM).

EE WAS SHOVELING SPILLED MATERIAL OVER THE COURSE OF THREE DAYS. EE COMPLAINED OF SORENESS AND CONDITION WORSENED IN HIS LEFT SHOULDER. EE RETURNED TO WORK. SORENESS PERSISTED AND EE BEGAN LEA VE ON MARCH 5, 2002.

CUTTING DRYWALL TO SIZE W/ UTILITY KNIFE, WHEN THE KNIFE STUCK A LITTLE, APPLIED MORE PRESSURE. THE KNIFE JUMPED FREE AND SLICED LEFT FRESHY PART OF PALM AT BASE OF THUMB.

AFTER UNLOADING TRUCK WHILE OPENING THE PASSENGE DOOR; I STEPPED BACK INTO A 5' HOLE THAT WAS UNGUARDED (BROKEN GATE).

I WENT TO CLEAN THE CARMEUSE COOLER PIT AND TRUNION PUMP, IT WAS LEAKING THRU A GASKET ON THE SIDE. AS I WENT TO INVESTIGATE, I STEPPED THRU A PILE OF LIME DUST. IT WAS HOT BUT I DIDN'T KNOW T HAT.

REPAIRING LIGHTS IN TRUCK LOAD-OUT AREA. VICTIM WAS FOUND BY FELLOW WORKER ON HIS STOMACH APPARENTLY SHOCKED; HAD FALLEN FROM LADDER-APPROX 4-5'.

CLEANING UP STONE AROUND COOLING TOWER, DOOR FLEW OPEN, WHEN HE REACHED OVER TO CLOSE DOOR, HIT A BUMP CAUSING THE DOOR TO FLY BACK, HITTING THUMB PINCHING IT BETWEEN THE DOOR AND FRAME OF LOADER.

CLOSING DOORS ON BOTTOM OF RAIL CAR, TRIPPED OVER PILE OF ROCK, HITTING RIGHT KNEE.

SMASHED RIGHT INDEX FINGER WITH SLEDGE HAMMER WHILE CLEARING CHUTES ON COKE SYSTEM.

AFTER STOOPING AND SHOVELING, EMPLOYEE STOOD UP AND IMMEDIATELY FELT LOW BACK PAIN RADIATING AROUND TO HIS RIGHT GROIN AREA.

EXPOSURE TO HOT WATER AND LIME ON FRONT THIGHS BOTH LEGS AND WRISTS WHILE CLEANING SPRAY TOWER ON #1 KILN.
