Employee was turning in reverse on electric pallet jack and hit left fifth finger on metal tote behind EE. EE was taken to hospital and laceration was sutured. EE returned to work the following day with modified duties.

Employee was replacing a vibrator on Dryer. After replacing the vibrator employee looked up to see if vibrator was operating, when a small amount of water with very small amount of acid dripped on employee's forehead and running down into the left eye.

While exiting the area the employee stepped onto a false ceiling and fell. The employee was conscious and alert after the fall and the personal protective equipment in place protected him from serious injury. He was taken to the hospital for evaluation. X-rays were taken of various parts of the employees body and an approx. 1/16" fracture was located in his left heel.

The EE picked up the roll of duct tape and stuck his left hand through the center of the roll and proceeded to pull the tape off of the roll with his right hand, however he did not notice on the backside was a razor blade. Whenever he pulled the tape with his right hand the razor blade made a laceration approx. 3/4" in length on his left middle finger just below the second knuckle.

Employee was driving forklift and whenever he went to turn the wheel he heard a pop in his wrist. He went for a follow up with the doctor on 2/23/2011 and received a cortisone shot in his wrist. There was not any damage to the tendons or bones in the wrist, it is just "painful".

Four employees were in the process of installing metal siding on the side of the plant at the second floor level. A section of overhead PVC pipe that had been secured at the top by rope broke loose, and a section measuring 8" X 5'4" struck the victim in the back of the head. He was wearing a hard hat.

Employee was off loading a tanker, the temp. was very low and ice had formed in part of the tank farm. As he valved off one of his pumps he slipped on a spot of ice causing him to pull his lower back.

Employee was working on a spent HF acid line. After washing the line out with water he began to remove part of the line. His feet got wet during the process. Thinking the line had no acid in it he continued to work on it. Later that night his right foot began to burn. The next day at 0700 hrs there was a blister on it. We then sought medical attention.

EE WAS CHECKING BEARINGS ON THE #6 DRYER. HE BRUSHED SOME SAND OFF OF A BEARING WITH HIS RIGHT HAND. WITH THIS MOTION, HE CONTACTED THE DRIVE BELT AND PULLEY. THE MIDDLE FINGER OF HIS RIGHT HAND WAS CAUGHT BY THE PULLEY, BREAKING THE BONE AND CUTTING THE TIP OF THE FINGER.

THE EMPLOYEE LIFTED THE LID OF THE BIN BUT COULD NOT SEE THE MATERIAL, SO HE MOVED CLOSER. THE DUSR COLLECTOR PULSATED ANDBLEW FINE MATERIAL ALL OVER HIS FACE AND COVEREDHIS GLASSES.
