Employee was replacing roll liners in a 992 G bucket at Clear Creek Wed morning. While grinding the bucket to prep for new liners to be installed metal shaving from the grinding had gathered on top of EE's grinding shield. Once EE stood up and raised EE's shield the material fell down EE's forehead and behind EE's safety glasses.

EE was in the process of removing the accumulator from the old cylinder when the employee attempted to roll the cylinder over by the manifold. While rolling the cylinder over, EE's finger got smashed between the 2 cylinders resulting in a laceration.

Two employees were conducting maintenance in a man lift. An inadvertent movement of the bucket momentarily caught the employee between the man lift basket and the speed reducer causing the employee to momentarily lose consciousness. After correcting the position of the man lift bucket the employee immediately regained consciousness and walked out of the bucket moments later.

While building a new chute, using flat washers as a spacer, a pry bar was used to remove the washers. The chute slipped from the pry bar smashing the tip of the left index finger.

The employee was changing oil in the Komatsu loader using a retractable hose reel to put oil in a reservoir. The employee was checking the dipstick when the hose reel unexpectedly retracted causing the nozzle to strike his left cheek. This caused a laceration that needed 6 stitches.

EE WAS REPLACING A DUST SUPPRESSION SPRAY BAR UNDERNEATH THE HP500 CRUSHER. THE PIPE WRENCH HE WAS USING SLIPPED AND THE BUTT END OF THE HANDLE STRUCK HIM UNDERNEATH HIS SAFETY GLASSES CAUSING A RUPTURED BLOOD VESSEL IN THE RIGHT EYE.

EMPLOYEE WAS CHANGING A 1 1/2" SCREEN CLOTH ON THE TOP DECK OF SIZING SCREEN. HE PLACED THE SCREEN ON TOP OF MOTOR HANGING IT ON A SIDE RAIL SCREEN BOLT. HE THEN BEGAN TO CLIMB UP ON TOP OF MOTOR TO ACCESS TOP DECK. THE SCREEN CLOTH CAME OFF BOLT LANDING ON HIS RIGHT FOOT, JUST BEHIND THE STEEL TOE. IT BRUISED & LACERATED RIGHT FOOT. HE RECEIVED 2 STITCHES.
