The employee was checking a ditch when ee tripped over vines and rolled left ankle.

Employee was helping to rebuild crusher. In an attempt to line up bolt holes employee used a crusher line up pin as a hammer to knock the bonnet leg into alignment. While swinging the line up pin, employee smashed right pinky finger. 9 stitches were needed.

Employee was trying to pry a rock out of the tail pulley when he felt something in his back "pop". Employee came to the office at 3:18pm to tell the plant manager that his back was hurt. *** Reportable Lost Time because employee did not come to work August 30 & 31.

On his way out of the screen EE lacerated his right arm just above the elbow on a sharp rock.

EE WAS ON OBSERVATION DECK WHEN SHOT IGNITED AND EE WAS STRUCK IN THE FACE WITH A ROCK.
