Employee was helping to rebuild crusher. In an attempt to line up bolt holes employee used a crusher line up pin as a hammer to knock the bonnet leg into alignment. While swinging the line up pin, employee smashed right pinky finger. 9 stitches were needed.

While installing rubber in the second deck of #1 screen, we lifted it up onto the catwalk. It was during the process that I felt my back get hot. * Employee has a strained back. This became a reportable on May 23 when it was determined through doctor's reports that his injury was work related.

EE was exiting screen and fell approx 36 inches and struck his arm on a small stone.320042820011-update returned to work full duty with no restrictions.(EE was inside the screen doing maintenance whent he job was finished other instructed EE to get out of the screen. On his way out of the screen EE lacerated his right arm just above the elbow on a sharp rock.
