B V Hedrick Gravel & Sand Co · Struck against stationary object

EE was pulling extra hose into a coarse material washer to wash it out for maintenance purposes and ran EE's right ring finger across one of the flights lacerating the fingertip. EE received 3 sutures making this accident reportable to MSHA.

EE was attempting to climb into an excavator, when grabbed the grab rail to pull self up EE's hand slipped off of it causing EE to fall backwards landing on EE's left side. EE was taken to urgent care and diagnosed with a fractured right index finger and some bruising on EE's left side and shoulder. The fracture makes this accident MSHA reportable.

The EE was walking the ABC catwalk. The EE stumbled and hit left elbow on a piece of angle. EE was able to continue performing normal job functions. Had MRI on 11/19/2020, and Dr. gave light duty due to "pain in elbow." No exact diagnosis.

EE was working on a side door on a step chute at the wash plant. The door hung up going down. Tried tapping on it to get it moving. The plate turned loose, slid down and hit the ring finger and index finger of the left hand, requiring stitches.

EE was helping change segment plates on 990 loader. While taking the segment plates off, the EE pushed the plow bolt up into the slot to secure into place. While holding it, they turned the bolt with a wrench. The bolt head had a sharp edge, and when it turned, it cut EE's right index finger, causing stitches.

EE was cutting down a tree. They were using forklift to guide the fall of the tree. When they were getting the forklift in position, the forks hit a dead limb, a 1" x 6" piece of the limb fell and struck the ee on the ground in the left eye. Had to have surgery on eye to repair. EE was wearing safety glasses.

EE was cutting holes in a piece of metal for bolts to go through with an oxy/acet torch. The EE tried to pull the gas hoses closer to him and they got caught on a piece of metal. He tugged at the lines and the torch fell from his hands. When the torch fell, it turned and burned his left forearm and wrist, causing 2nd and 3rd degree burns.

EE was moving a frozen water hose from the shop. While moving the hose, a chunk of ice fell from the hose. The EE stepped on the chunk of ice and twisted his ankle. The doctor immobilized his ankle with a u-splint, but was able to continue his normal job.

EMPLOYEE STEPPED ON A 3 FOOT PORTABLE LADDER, CAUSING THE LADDER TO SWING BACK AND STRIKE HIM ON THE LEFT KNEE. EMPLOYEE HAD SURGERY ON JAN 21, 2005. TO REMOVE A PIECE OF BONE THAT WAS IRRITATING LIGIMENTS.

EE WAS STTEMPTING TO OPEN AN INSPECTION DOOR ON A CRUSHER. HE OPENED THE DOOR WITH TOO MUCH FORCE STRIKING THE CRUSHER WITH HIS LEFT HAND. HE CONTINUED TO WORK THE 12TH AND 13TH. IT STILL WAS SORE ON MONDAY THE 16TH AND HE WENT TO THE CLINIC FOR AN X-RAY. HE HAD A CRACKED BONE IN HIS HAND. NO LOST TIME OR RESTRICTED DUTY.

EE WAS ATTEMPTING TO HOLD A PLOW BOLT FLUSH IN A SLOTTED HOLE. ANOTHER EE LOOSENED THE BOLT FROM THE OUTSIDE OF THE SCREEN DECK WHEN THE BOLT UNEXPECTEDLY TURNED, CUTTING THROUGH A GLOVE AND C UT THE FIRST FINGER OF THE RIGHT HAND. 5 STITCHES.
