Martin Marietta Materials Inc · Struck by flying object

Night shift EE decided to retrieve EE's RC car from the employee parking area. The EE retrieved the RC car, placed it in the company truck, and stopped by the granite sand pile. EE began operating the RC car and during this process, EE lost control of the RC car and it jumped off the ramp at approximately 20 mph and struck the EE in the face, causing a laceration on EE's chin.

A Volvo A40G off road 6x6 truck made contact with a Komatsu PC360 excavator while approaching from the blindside of the excavator. This incident was investigated by Mine Safety and Health Inspector on 09/12/2016. The complaint ID is 966940. We were notified on 10/03/2016 that the operator of the Volvo 6x6 truck had filed a claim of injury.

Employee was moving some chains and was apparently bitten by an insect. Area became inflamed and he was seen by a physician on 6/12.

Employee pulled some electrical cable by hand to move it from one area to another and felt numbness and tingling in his left elbow and fingers.

Employee was pulling a piece of wire from in front of the haul truck he was preparing to operate. He felt a pain in his lower back and went to the clinic where he was diagnosed with a mild back sprain. He returned to his regular job as truck driver. His condition improved until 06/06/2011 when it unexpectedly got worse and the doctor put him out of work.

Employee was cutting a piece of conveyor belt to make a patch. The utility knife slipped while he was pulling towards himself and struck the right leg just above the knee, inflicting a gash about one and one half inches. Required several stitches to close.

A brush and grass fire was observed on mine property and was called in to the emergency dispatcher. The Lemon Springs Fire Department was dispatched and was on scene in 17 minutes. The fire was controlled and the responding units left in one hour and 46 minutes. The fire was immediatly reported to MSHA. The cause of the fire was not determined.

EE had his finger mashed in the door of the haul truck cab. He was pulling the CB cable that was dragging inside the cab. Another EE was training on the truck. He got out of the cab & pulled slack out of the CB cable line. Injured was trying to hold the cable out of the way while the door was shut. Injured thought he was OK & told other EE to shut the door. Finger was fractured.

Ee was lifting screens to the top of the screen deck and felt an onset of back pain. He thought it was pulled muscle but when it didnt get better he went to the dr. and after it did nfot improve, an mri was done showing a herniated disc. because his contition did not improve any the dr. took him out of work on 7/16/07.

Employee was holding a side panel of polyurethane screen deck on his right knee and trimming it with a utility knife when the knife slipped and cut his leg. The resulting laceration required four stitches to close. Medical only, no lost time.

A screen fire was started during construction of the new secondary plant. Belt 26 was started & it was rubbing on the structure, so EE trimmed a small piece of metal with a torch. Some of the slag fell to the screen deck below & started a fire on the rubber screen deck. The fire was extinguished in about 45 minutes. No injuries.

While changing jaw dies on the jaw crusher, Employee was getting set up to trim an old die to fit the rock box between the feeder & the jaw. While moving this die that weighed approx.1000#, the lifting chain came unhooked alowing the die to fall over the work platform. As it fell it struck EE's rt. leg scraping the skin. It also smashed his steel toe in his shoe.

PERFORMING ROUTINE ELECTRICAL HELPER INSTALLATION TASKS. EE WAS IN THE PROCESS OF CARRYING CONDUIT FROM A STORAGE PILE TO THE WORK SITE. WHILE CARRYING CONDUIT ON SHOULDER, STEPPED INTO A BLIND HOLE APPROX 12" DEEP. EE FELT SORENESS IN HIP AREA. NEXT DAY, EE WAS IN PAIN WHEN REPORTING FOR WORK, SENT TO HOSPITAL. XRAY REVEALED A FRACTURE OF THIRD VERTEBRA.

EE CUT RIGHT HAND BETWEEN THUMB AND WRIST WHILE TRYING TO CHANGE CUTTING EDGE ON 988F WEARPLATE.HE WAS HOLDING THE BOLT UP TO STOP IT FROM TURNING. THE IMPACT WRENCH PUSHED BACK THE BOLT CAUSI NG IT TO SPIN CUTTING THROUGH HIS GLOVE INTO HISHAND.
