EE was walking from the jaw truck to conveyor when EE stepped on a rock and rolled EE's right ankle outward. EE continued working for the remainder of the week and did not seek medical treatment until 4/3/2026.
IW was dismounting the adjustment ring with EE's right hand on the top locking post when the feed cone swung and pinched EE's right middle finger between the locking post and feed cone, resulting in a fracture and laceration to EE's right middle finger.
The employee climbed onto assigned loader to initiate the pre-shift inspection. While climbing down they thought they were on the bottom step when they were actually on the second step. When they released to touch the ground they fell further than anticipated causing them to fall to the ground. They suffered a fracture to the left wrist.
The employee pinched the end of their right thumb between a bolt head and the recessed hole it was in. This occurred when the bolt spun when an impact wrench was applied to the nut on the other end unexpectedly.
Employee was attempting to cut a rubber curtain with a razor knife when employee slipped and cut right forearm.
The employee was adjusting a cone crusher using controls contained in a metal control box. The metal lid was dislodged by the wind causing it to fall and strike the employees finger. The contact resulted in a fracture of middle finger.
While attempting to remove a drill bit from the bit basket employee's left index finger was lacerated.
Employee was climbing back onto EE's haul truck when EE slipped from the first step back to the ground. Its now known that this caused a small tear in EE's right achilles tendon. The diagnosis and treatment plan occurred on 1/7/2020.
Employee was welding on a slide wrench. Employee had welding shield down and didn't notice the slide starting to move. Slide wrench fell on employee's left pinkie finger splitting open top of finger requiring 5 stiches. Employee was wearing thick welding gloves and proper PPE at time of incident.
While sliding a screen panel into place the employee strained lower back.
While lifting primary crusher toggle for repair, lifting device broke and employee fell approx. 3-4 feet on top of falling hydraulic toggle.
Employee was refueling his loader and stepped on the edge of the concrete base, approximately 6 inches above ground level, severely twisting his ankle.
Employee had lower back muscle spasms from lifting and repositioning flatbed truck side panel.
No accident, injury, or illness occurred. Employee felt dizzy and needed assistance to walk.
Employee fainted in the employee parking lot next to his car after his shift was completed.
Employee slipped off of a step while cleaning loose material and sprained his right ankle.
Employee received an insect bite in the groin area, on his right leg.
Employee slipped on loose ground while crossing a ditch.
Customer truck driver suffered a heart attack while waiting to be loaded in the yard. Customer was Rea Construction Asphalt Materials.
Employee lifted a mat, covering a blasting hole, and a spider bit him on the arm.
Employee was prying with a crowbar when he slipped and struck his left hand on the conveyor frame.
When the employee removed a hinge pin on a backhoe bucket, the scissor-type hydraulic cylinder fell and crushed the employee's little finger to his right hand.
Chest x-ray abnormality consistent with pneumoconiosis with ILO reading below 1/0. ILO reading of 1/1.
While standing on the ladder and reaching to close the cab door of a loader, the operator lost his balance and fell to the ground.
Employee lost his footing while walking down a shallow slope in the pit. In trying to break his fall, he sprained his left wrist.
Driller was attempting to remove the hammer from the end of the drill section when the outer collar of the drill steel released from the hold position and pinch the driller's left ring finger between the collar and the hammer wrench he was trying to install.
Employee was striking a wedge pin that holds concrete forms in place when the combination of the hammer striking the pin and the pressure release of the pin caused the pin to ricochet off the concrete form frame and strike the employee in the mouth. The injury required 3 stitches and dental work.
EE WAS TRYING TO CLEAR MUD FROM A DRILL BIT ON A DOWN THE HOLE ENCLOSED CAB DRILL. WHEN THE EE STEPPED OUTSIDE OF THE CAB TO PLACE THE PLATE IN PLACE TO DISCONNECT THE DRILL BIT, THE MUD CLOG RELEASED THE BUILT UP PRESSURE. EITHER THE OVER PRESSURE AIR OR THE LOUD NOISE CAUSED DAMAGE TO HIS EARS. INFLAMMATION AND TRAPPED FLUID CAUSED A HERARING LOSS.
EMPLOYEE STEPPED OUT OF A DOORWAY FROM THE SHOP AREA ONTO LOOSE MATERIAL ON THE GROUND. HE TURNED HIS RIGHT ANKLE CAUSING LIGAMENT STRAIN TO THEINTERIOR OF HIS ANKLE.
THE EMPLOYEE WAS LOOSENING A WEDGE BOLT NUT WITH A KNOCKER WRENCH AND SLEDGE HAMMER. AFTER STRIKING THE WRENCH WITH THE HAMMER, THE WRENCH BOUNCED OFF THE NUT AND STRUCK THE EMPLOYEE ON HIS SA FETY GLASSES AND CUT HIS RIGHT CHEEK, REQUIRING STITCHES.
