EE was walking from the jaw truck to conveyor when EE stepped on a rock and rolled EE's right ankle outward. EE continued working for the remainder of the week and did not seek medical treatment until 4/3/2026.

IW was dismounting the adjustment ring with EE's right hand on the top locking post when the feed cone swung and pinched EE's right middle finger between the locking post and feed cone, resulting in a fracture and laceration to EE's right middle finger.

The employee climbed onto assigned loader to initiate the pre-shift inspection. While climbing down they thought they were on the bottom step when they were actually on the second step. When they released to touch the ground they fell further than anticipated causing them to fall to the ground. They suffered a fracture to the left wrist.

The employee pinched the end of their right thumb between a bolt head and the recessed hole it was in. This occurred when the bolt spun when an impact wrench was applied to the nut on the other end unexpectedly.

The employee was adjusting a cone crusher using controls contained in a metal control box. The metal lid was dislodged by the wind causing it to fall and strike the employees finger. The contact resulted in a fracture of middle finger.

Employee was climbing back onto EE's haul truck when EE slipped from the first step back to the ground. Its now known that this caused a small tear in EE's right achilles tendon. The diagnosis and treatment plan occurred on 1/7/2020.

Employee was welding on a slide wrench. Employee had welding shield down and didn't notice the slide starting to move. Slide wrench fell on employee's left pinkie finger splitting open top of finger requiring 5 stiches. Employee was wearing thick welding gloves and proper PPE at time of incident.

Employee was striking a wedge pin that holds concrete forms in place when the combination of the hammer striking the pin and the pressure release of the pin caused the pin to ricochet off the concrete form frame and strike the employee in the mouth. The injury required 3 stitches and dental work.

EE WAS TRYING TO CLEAR MUD FROM A DRILL BIT ON A DOWN THE HOLE ENCLOSED CAB DRILL. WHEN THE EE STEPPED OUTSIDE OF THE CAB TO PLACE THE PLATE IN PLACE TO DISCONNECT THE DRILL BIT, THE MUD CLOG RELEASED THE BUILT UP PRESSURE. EITHER THE OVER PRESSURE AIR OR THE LOUD NOISE CAUSED DAMAGE TO HIS EARS. INFLAMMATION AND TRAPPED FLUID CAUSED A HERARING LOSS.

THE EMPLOYEE WAS LOOSENING A WEDGE BOLT NUT WITH A KNOCKER WRENCH AND SLEDGE HAMMER. AFTER STRIKING THE WRENCH WITH THE HAMMER, THE WRENCH BOUNCED OFF THE NUT AND STRUCK THE EMPLOYEE ON HIS SA FETY GLASSES AND CUT HIS RIGHT CHEEK, REQUIRING STITCHES.
