While performing work on the jaw crusher, a jack accidentally struck the employee in the back of the head, when a weld broke on a plate that was used to secure the jack. This caused a laceration, requiring stitches and spinous process fracture.

EE WAS USING A PIPE WRENCH TO TURN A JACKSHAFT AT THE JAW TO PUT A BELT BACK ON. THE PIPE WRENCHSLIPPED OFF OF THE SHAFT AND THE EE WAS ATTEMPTING TO PUT THE WRENCH BACK ON WHEN THE COUNTERWEI GHT ROLLED SLIGHTLY CATCHING THE EE'S FINGER BETWEEN WRENCH AND A GUARD CAUSING A LACERATION.

A ROCK ABOUT THE SIZE OF AN ORANG E ROLLED DOWN SLOPE STRIKING SHOE THEN BOUNCED ONTO HIS SHIN CAUSING A SMALL CUT. INJURED HAD BEEN HAZARD TRAINED PRIOR TO ACCIDENT.

EMPLOYEE WAS LOWERING A SLEDGE HAMMER TO THE FLOOR. HE STRUCK THE INSIDE OF HIS LOWER RIGHT LEG,CAUSING CONTUSION OR HEMATOMA. SENT HIM TO DOCTOR TO BE CHECKED 12-27-00. DOCTOR SAID HE WAS O.K . BUT TO RETURN FOR FOLLOW UP 12-30-00. AT THAT TIME LEG HAD BECOME SWELLED. DOCTOR MADE INCISION TO DRAIN FLUIDS ON LEG. NO LOSS AT WORK OR RESTRICTIONS.
