While attempting to flip a gate on screen #1, the gate handle struck the employee's chest, causing him to strike his back on the screen.

In the process of greasing the tail pulley of the conveyor, the employees glove came into contact with a speed sensor on the shaft of the pulley. The speed sensor caused his hand to be pulled toward the rotating shaft, resulting in a cut to his hand and arm.

On 7/7/08, an employee found an abandoned bicycle on the property. The police department was called to pick the bike up. After further investigation by the police dept. A search of the property was conducted. On 7/8/08 at approx. 9:00 am a body of an unidentified adult male was found at the base of the quarry. MSHA and the police dept conducted an investigation.

Employee was constructing a conveyor and was climbing onto conveyor steel frame when his foot slipped on a strut causing employee to lose his balance. Employee grabbed end of strut and his wrist hit an edge of strut cutting his right wrist. Employee was wearing gloves at the time.

EE WAS SLIDING A ROCK BREAKER PART ON A PALLET FROM THE BACKHOE BUCKET INTO THE BED OF A PICKUP TRUCK. THE BOARD ON THE PALLET CAME OFF CAUSING THE EE TO LOSE HIS BALANCE AND FALL BACKWARDS. H E ATTEMPTED TO BREAK HIS FALL WHICH RESULTED IN THE MAJORITY OF HIS BODY WEIGHT ON HIS RIGHT ARM AND WRIST.

HE WAS STANDING ON THE CONVEYER BELT UNDER THE JAW CRUSHER INSTALLING A JAW DIE. HE WAS CUTTING THE LIFTING EYE OFF THE JAW DIE WHEN A PIECE OF HOT SLAG FELL INTO HIS WORK BOOT BURNING THE TOP OF HIS FOOT.

EMPLOYEE FINISHED DRILLING A HOLE AND OPENED THE DOOR TO GET OUT OF THE DRILL. WHILE HOLDING THE DOOR JAM, HE LOOKED BACK TO MAKE SURE THE HOLE WAS O.K. THE WIND BLEW THE DOOR SHUT AND CAUG HT HIS RIGHT HAND BETWEEN THE DOOR AND THE DOOR JAM CAUSING THE INJURY TO THE MIDDLE TWO FINGERS.

EE WAS PULLING ON A 1/2" HOOK ROD TRYING TO UNCLOG A CHUTE. WHEN THE CLOG BROKE LOOSE, IT ALLOWED HIS ARM TO JERK BACK CATCHING HIS LITTLE FINGER ON HIS RIGHT HAND BETWEEN THE HOOK ROD AND A A NGLE IRON BRACE INFLICTING THE INJURY.

EMPLOYEE WAS TIGHTENING BOLTS WHEN HE FELL ON THE GROUND, LANDING ON THE GROUND ON HIS BACK HITTING HIS HEAD ON THE STRUCTURE AND HIS BACK ON A ROCK. HE ALSO HIT BOTH ELBOWS DURING THE FALL. THE FALL WAS BETWEEN 7-8 FEET.

EMPLOYEE WAS STANDING ON THE TRACKS OF AN OLD CRANE WE USE FOR PARTS TO SEE IF IT HAD A GUARD THAT HE NEEDED. AS HE STEPPED DOWN HIS FOOT SLIPPED AND HE FELL FACE DOWN ON THE GROUND. HIS ARM L ANDED IN AN AWKWARD POSITION CAUSING A BROKEN WRIST.
