EE was attempting to move a rock that weighed approximately 40 lbs. from the catwalk. When they swung the rock, it did not clear the middle handrail and pinched the tip of their left ring finger between the handrail and the rock.

While switching plant over employee needed to use a come along to flip a gate, once the come along was grabbed the chain was knotted up. The employee hung the come along up to straighten the chain and get some slack in it, when EE pulled the slack the hook came free and struck employee in the mouth breaking one front tooth in half.

Employee was lifting engine with shop hover head hoist to install in Euclid R-40 water truck. While using a Caterpillar lifting Bar the adjustment screw failed pinching employee right ring finger between roller resulting in laceration/amputation to the tip of finger.

METAL STRUCK INJURED IN THE BACK OF HIS LEFT LEG AT THE KNEE AREA.

USING A RAM (500 LBS.) THAT WAS SUSPENDED FROM A CRANE TO LOOSEN THE HEAD NUT ON A SHORTHEAD CRUSHER. WHEN THE RAM CONTACTED THE WRENCH THAT WAS WELDED ON TO TO NUT, THEWRENCH CAME UNCOUPLED A LLOWING THE RAM TO FALL TO THE GROUND AND CONTACTING THE SAFETY SHOE OF THE INJURED EE CAUSING PARTIAL AMPUTATION OF THE LEFT BIG TOE.

EE WAS OPERATING CRANE TO REPLACE THE WATER PUMPIN THE PIT. THE GROUND WAS NOT LEVEL ENOUGHT TO SET THE CRANE UP PROPERLY AS HE CARRIED WOODEN BLOCKS TO LEVEL THE RIGHT REAR OUTRIGGER HE STEPPED ON A LOOSE ROCK CAUSING HIS LEFT ANKLE TO TWIST
