EE was pouring hyd oil into a crusher console when they tried puncturing a hole in the lid to increase flow, the knife folded cutting their right palm
While changing forks out for the bucket on a CAT skid steer, the injured employee went to knock the pin up so the forks would release. When EE did, the forks came off the frame and landed on EE's left foot, breaking the big toe and causing a laceration
Employee claims that shoulder began to hurt after EE returned home from work. EE alleges that EE may have strained shoulder while using a sledge hammer to drive pins. EE has a slide sledge on truck and has been instructed not to use the sledge hammer. Employee did not report the injury until 1/16/2017.
Injured employee was climbing into a chute directly below the wash screen to apply a patch. The employee slipped while entering and struck their back against a protruding bolt or pad eye on the side wall. The employee was wearing fall protection that caused them to "swing" into the side wall. Employee continued to work until breathing became painful
Employee was changing gauges on anoxygen bottle. The wrench he was using slipped from his hand and struck his upper lip.
At approximately 2:00 pm a group of teenagers trespassing on the property discovered and reported a deceased person on the mine property. This was called in to MSHA under the guidelines of "Immediately Reportable" incident. Investigations by local authorities identified the victim's residence and notified family. Medical examiners report identified cause of death as a stroke.
EE was putting screen cloth in place and felt a slight pain in his shoulder. EE reported the pain on 09/16 and was taken for medical evaluation. EE returned to duty same day with no restrictions. On 09/21, EE complained of pain again and was again taken for medical evaluation, this time, EE was placed on minor restrictions. EE was released to full duty-no restrictions on 09/28
The EE was descending her vehicle via the access ladder and she lost her grip and fell.
While operating a Cat 773D haul truck, employee bumped his head against the window frame, causing a contusion and small cut to the left side of his head. He suffered from headaches and was placed on restricted duty.
Cutting bands off of a stack of steel, one band that had been cut off already swung up and hit his left forearm cutting it (4 stitches).
Carrying a screen up to rollaway box,while putting the screen into position, EE turned wrong & twisted his back.
While unhooking a trailer from a pickup truck, the employee pinched his his right thumb between the trailer hitch and the pickup.
Hanson was notified that employee was diagnosed with Carpal Tunnel syndrome, which the physician determined it was probably work related.
THE CONTRACT MECHANIC WAS RIGGING A HYDRAULIC HAMMER TO BE LIFTED. WHEN HE TOSSED THE CHAIN OVER THE HAMMER IT SWUNG BACK & CONTACTED HIS FRONT TOOTH W/THE LIFTING LUG. THIS CONTACT RESULTED IN A SERIOUS CRACK TO THE MECHANICS FRONT TOOTH.
INSTALLING MOTOR ON JAW FEEDER, EE REPORTED LOWER BACK PAIN. PHYSICIAN PLACED HIM ON RESTRICTED DUTY THRU MARCH FIRST.
EE STRIKING IDLER WITH 2 LB. HAMMER WHEN HAMMER RECOILED, SPRAINING THUMB.
ON 5/24/01 EE STRUCK A LOADER BUCKET TOOTH WITH A HAMMER & METAL SPLINTER LODGED IN RIGHT THIGH.AFTER XRAY DR DECIDED NOT TO REMOVE IT, PUT BANDAID ON IT & EE RET'D TO FULL DUTY. ON 10/02, EE STARTED HAVING LEG PAIN, NO ACTION TAKEN OTHER THAN RX'S BUT PAIN CON'T. 6/10/03 NEUROLOGIST PUT EE ON RESTRICTED DUTY & SCHEDULED NERVE SURGERY 7/3/03 EXPECTED TO BE OUT OF WORK 3 WKS & ON
EE SLIPPED WHILE CLIMBING DOWN LOADER JERKING HIS LEFT ARM. THIS CAUSED A JERKING MOTION THAT POSSIBLY CAUSED TEAR IN HIS ROTATAR CUFF. EE RETURNED TO FULL DUTY ON 10/15/02. LATER HAD MORE PAIN IN SHOULDER, & CURRENTLY ATTENDING PT BUT STILL HAS LIMITATION ON ARM MOVEMENT. EXPECTED TO REACH END OF TREATMENT IN 2 MONTHS.
EE WORKING ON SCALES (WELDING) MADE STEP WHILE TURNING FELT PAIN IN BACK LOWER BACK STRAIN & INFLAMED NERVES AROUND DISC. 1ST REPORTED ON 6/24/01-(NON REPORTABLE) CONDITION WORSENED ON 7/9/01
