On 3-7-23 While hammering screens into place of scalping screen, employee missed the screen and contacted EE's finger with the hammer. EE was taken to the clinic and received 4 stitches

Three employees were changing springs on a feeder. A pry bar was employed by two of the employees to expand the bracket, allowing the spring to go into position and insert the bolt. The injured employee was pulling on the bar while on one knee. EE's hands came off the pry bar and EE's body weight came down on EE's left leg and/or twisted EE's left ankle resulting in a fracture.

Attempting to clear a jammed rock from the jaw crusher, the employee donned fall protection, and descended a ladder placed in the opening of the crusher. The ladder slipped and twisted. The employee's left leg got between two of the ladder rungs resulting in the left ankle being fractured.

Employee was helping to change a return roller on a conveyor, holding the roller can in position, ee was losing grip. To get a better grip on the can the employee took glove off and placed it back on to the roller can. When the roller seated in the frame, the tip of left little finger got pinched between the roller can and the bracket resulting in 6 stitches.

Repair of RS Conv 99% complete w/severe storm minutes away. 2 riggers supplied by Edwards, Inc on catwalk loading tools on deck of hyd. manlift supplied by Hanson Agg. 1 rigger thrust a heavy steel pinch bar on deck of manlift & it slid across deck, under entrance w/no kick plate, fell onto conv. carriage, glanced toward EE who threw rt arm up as shield, hit his arm & broke it.

Employee was changing skirt board rubber on conv. 30. Has he was sliding the rubber back in, his hand slipped and hit the knife that he placed on the shelf above where he was working.

EE was dismounting loader and foot slipped on ladder rung. He caught himself by grasping the handrails which caused a twinge in his shoulder. He did not think medical attention was required at the time. On July 6, EE stated that his shoulder still hurt and requested medical attention. He was immediately taken for medical evaluation. Physical therapy prescribed, thus reportable.

While changing bits the starter rod came loose from the rotary head. When it fell the driller's fingers were caught between the hammer wearsleeve and chuck.

While attempting to manually shut the tailgate of his dump truck, He caught finger in tailgate causing amputation of the tip of the index finger of the right hand and slo sraping his middle finger. He was at sight to pick up a load of rock.

While moving a home made steel tool box by hand, employee suffered a strained muscle in his lower back.

Employee was assisting in installing a beam on the wash tower screen. The beam slipped and pinched the employee's finger between the beam and the screen frame.

Employee was driving a loaded Cat 773 haul truck down the pit road. Employee states that the truck bounced, causing him to strike his left knee against the door panel. Employee suffered a contusion to his knee, and was placed on restricted duty.
