EE was doing a pre-operational check on the 1994 Fuel Truck, the added oil to the cannister and stepped down, as they did their foot hung up in the hold and they twisted in order not to fall. They reported groin and abdomen strain. It was diagnosed as a hernia and surgery was required.

Electrician was changing out a breaker. Electrician disconnected main Service disconnect and was unaware there was multiple services in the building. An arc flash occurred and their right arm was burned.

An employee preparing a hammer breaker at the jaw crusher when the pry bar EE was holding slid suddenly causing the side of EE's hand to be pinched, resulting in a laceration requiring five sutures.

While striking stud end of jaw wedge bolt with a ¼ swing action, IE struck right index finger against stud of adjacent wedge bolt. No medical treatment did not seem necessary at the time of the incident. Continued discomfort in the employee's finger prompted medical attention on 07/06/2015. Employee had finger surgery on 9/15/2015.

On July 8, EE was dismounting piece of mobile equipment when he felt a pain in lower back. Nagging pain persisted and on August 8, 2010, EE went to doctor and physical therapy was prescribed.

Equipment service technician stepped of the back bumper of his service truck. Right foot caught on the step. Left foot slipped and technician landed on the ground on his left knee. Twisted his knee.

While reaching for the handhold on a fuel truck door, employee jammed right middle finger on the handrail. The physician applied a splint and advised him he could return to regular job duties as long as he wore the splint.

Employee was found unconscious in crusher control house. CPR was administered by staff until EMT service arrived and took over rescue efforts. Employee was transported to a local hospital via ambulance, where he was declared deceased from natural causes. MSHA conducted an investigation that day, and confirmed the cause of death was due to natural causes the following day.

Employee was kneeling on a pivoting motor base removing tensioning nuts. When last nut was removed, base was allowed to pivot, throwing employee to deck below (7 feet 11 inches). Injury resulted in concussion, lacerated tongue, and contusions to right mid-torso.

On 1/26/05, employee complained to plant manager he had pain in his left arm, later diagnosed as tennis elbow. Employee stated he used a 3# mall on the morning of December 8, 2004 to remove a belt splice, and had noticed aching feeling afterwards. Employee did not report incident or pain until 1/26/05.

A LOCKPOST WEDGE IN A 4 1/4 CRUSHER WAS EJECTED FROM THE LOCKPOST DURING AN ADJUSTMENT PROCEDURE, STRIKING THE EE ON THE RIGHT SIDE OF HIS FACE. HIS SAFETY GLASSES ABSORBED THE IMPACT LIMITING THE INJURY TO THE EYE ORBIT AREA. THE RESULT WAS CONTUSION & MINOR LACERATION, BUT NO DAMAGE TO THE EYE. RESTRICTED DUTY.

WHILE PERFORMING MAINTENANCE ON A CRUSHER, EMPLOYEE SUFFERED HEAT EXHAUSTION. HE WAS TREATED AT LOCAL HOSPITAL AND RETURNED TO WORK THE NEXT DAY BUT DID NOT STAY FOR THE WHOLE SHIFT.

EE WAS HOLDING AN ALLEN WRENCH WHILE 2ND EE USING AN AIR WRENCH TO TIGHTEN NUT. THE WRENCH SPUN THE NUT UNTIL IT CAUGHT, SPINNING THE BOLT, WHICH CAUSED THE ALLEN WRENCH TO SPIN. THE ALLEN WRE NCH CAUGHT THE EE'S FINGER BETWEEN THE FRAME AND THE WRENCH. THEY WERE REPAIRING A FEEDER PAN LINER.
