The employee was removing metal from a conveyor, and while going to dispose of it, the employee lost control of the metal object and pinched their hand between the metal and the handrail. This resulted in a laceration and a broken finger.

The employee was cutting 1 inch steel plate for a chute repair. Upon cutting the metal that was on a utility tractor fork, some of the plate shifted and fell on EE's left foot catching EE's left pinky toe just beyond the protection of EE's steel toe.

Employee was changing wear plates/rubber screens at Class B chute of primary plant. Employee moved to allow removal of the wear plates from the screen deck and stepped down onto a ledge covered with rock twisting ankle in the process.

EE & co-worker were working on CAT 988 loader bucket. EE was inside & Co-worker outside. They were removing cutting plates so that they could replace the teeth on the bucker. The bolt that ee was working with cut through glove, injuring left hand.

Employee was trimming center hold-down rubber on screen with an auto-retracting utility knife when the knife slipped and he stabbed himself in the left wrist. The resulting puncture wound required three stitches to close.

At approx. 8:30 the mgr.was notified of the arrival of EMS at the plant gate. They said they were responding to a call from a Customer Truck Driver who thought he was having a heart attack. They started treatment and transported him to Wake Med. MSHA was notified at 8:45. We did not get any info on his condition. EMS did not think it was very serious.

EE sustained a hernia while stretching over a cross-beam and pulling on a wrench to loosen a hydraulic hose. He felt a pain in the abdomen and it was still hurting the next day, so he was sent to the Dr. The condition did not improve any and surgery was required to correct it. First day of lost time was 12/27/06.

EE cut right hand open when he slipped and fell walking to begin shift. the ee was crossing a wooden bridge from ee building to the stockyard and slipped on the wet/ icy bridge. When he braced against the fall, his hand struck the planks of the bridge and cut open. The ee was transported to MedFirst at 8 AM for treatment. required stitches.

HEART ATTACK. EE WAS OPERATING A HAUL TRUCK IN THE PIT WHEN HE DIED OF NATURAL CAUSES. HE PARKED THE TRUCK AND SET THE BRAKES AT THE ONSET OF THE HEART ATTACK SO THERE WAS NO PROPERTY DAMAGE. OTHER EMPLOYEES ATTEMPTED CPR & CALLED THE GARNER EMS. TRANSPORTED TO HOSPITAL.

EMPLOYEE WAS ADJUSTING A RATCHET TYPE DIRT HANG BELT SCRAPER WHILE THE BELT WAS IN MOTION WHEN THE SCRAPER CAUGHT THE BELT CAUSING THE RATCHET HANDLE TO STRIKE THE EMPLOYEE'S SAFETY GLASSES. A SHARP EDGE ON THE UVEX ASTROSPEC GLASSES CUT HIS LEFT EYE LID REQUIRING 5 SUTURES.
