While performing maintenance on stacker, employee struck conveyor belt and made contact with other employee.

We were working on steel line, when employee walked up beside me and pipe wrench slipped off. Employee was hit in the forehead. Took 6 stitches, then employee returned to work after leaving doctor's office.

Employee was repairing tracks and was driving a pin with a hammer when he was struck in the chest by a piece of metal. Reported this to management on 5/29 when he was sore in that area and was sent to medical facility. A small piece of metal was removed from his chest by the physician.

Employee was cutting a section of rubber water hose with a utility knife and cut his arm requiring sutures.

Employee was removing a guard when it bumped his chest causing bruised ribs. Was seen by clinic on 8/2 - light duty until 8/6.

First knowledge of incident at 8:05; reported to MSHA at 8:12. EE suffered a massive heart attack while driving from the pit to the shop. He pulled over to the side of the roadway at the top of the pit and was found by other EE's following him. CPR was started and EMS was called. He was transported to Trauma Center and was pronounced dead @ appox 9:15 AM.

EE was putting adapter on the bucket cutting edge of the Cat 990 loader when he was struck by a metal chip. He was driving on the adapter with a 12 lb. hammer when a chip flew off and struck him in the upper right thigh. EE was taken to Pitt Memorial Hospital ER where the chip was removed and the wound closed with 7 stitches. Medical only. No lost time.

CHRONIC SILICOSIS. OUT OF WORK UNTIL FURTHER NOTICE. EMPLOYEE'S CONDITION WAS DISCOVERED DURING THE COMPANY'S ANNUAL ROUTINE MEDICAL SURVEILANCE. HE WAS REFERRED TO A SPECIALIST FOR FURTHER STUDY AND REMAINS OUT OF WORK PENDING DOCTOR'S RECOMMENDATIONS.

Cleaning gyratory bin, he stumbled, grabbed a chain that was hanging on a hoist and a second chanin that was on hoist struck him in the mouth, chipping front tooth.

EE IS A MECHANIC. HE WAS REPLACING A TRANSMISSION OIL COOLER ON A CAT 990 MACHINE. DURING THE PROCESS HE WAS USING A PRY BAR TO MOVE THE TRANSMISSION FOR POSITIONING AND THE BAR SLIPPED AND SMASHED HIS FINGER AGAINST THE TIRE.

THE 2 WITNESSES WERE REPLACING TRACKS ON EXCAVATOR. THE INJURED EE WAS STANDING 10-15'AWAY & RECEIVED A SMALL PUNCTURE WOUND WITH EMBEDDED METAL TO HIS RT. LEFT SHOULDER WHEN THE DRIVE SPROCKE T SLIPPED AGAINST THE TRACK BAR CAUSING METAL FRAGMENT TO FLY OFF AND STRIKE HIM.

MASHED FINGER IN FLOOR JACK WHILE CHANGING OIL IN PICK-UP TRUCK. THE FLOOR JACK HAS TWO HANDLES, ONE APPROC 4'LONG (FOR HAND) THE OTHER SHORT (FOR FOOT). WHEN THE LONG HANDLE COMES DOWN IT COM ES IN CONTACT WITH SHORT HANDLE, CAUSING IT TO COME DOWN ALSO. THIS CAUSES A PINCH POINT BETWEEN BOTTOM OF SHORT HANDLE & LIP ON JACK FRAME. HIS LEFT INDEX FINGER WAS IN THE PINCH POINT WHEN

EMPLOYEE CUT RIGHT LEG JUST ABOVE KNEE WITH A UTILITY KNIFE WHILE CUTTING A PIECE OF CONVEYER BELT, REQUIRED 3 STITCHES, MEDICAL ONLY, NO LOSS TIME.
