A tree fell on mine property due to high wind, causing it to fall on a powerline, causing it to arc. This resulted in two brush fires to start burning on the mine property.

Suffered a hairline fracture of his pelvis & numerous bruises when the loader overturned. He worked his way around the feeder until he was in a position sideways on the ramp. He dumped a bucket of wet material into the feeder and some of it stuck in the bucket, he backed up & exceeded machine's center of gravity &overturned. He unbuckeled his seatbelt and climbed out.

FALL FROM DRAGLINE BOOM RESULTED IN BROKEN RIBS AND DISLOCATED SHOULDER. EE WAS PERFORMING ROUTINE MAINTENANCE GREASING THE SHEAVES ON THE GANTRY. THE BOOM WAS LOWERED TILL THE BOOM POINT WAS AT GROUND LEVEL AND THE GANTRY WAS LOWERED TO THE BOOM. HE WALKED DOWN THE BOOM AND GREASED THE LEFT SIDE, HE SLIPPED AND FELL 9' TO GROUND STRIKING THE BOOM STRUCTURE ON THE WAY DOWN.

EMPLOYEE RECEIVED CONTUSIONS AND LACERATIONS TO THE HEAD WHEN THE GRADER DOOR SLAMMED SHUT ON HIS HEAD. THE GRADER WAS BEING USED TO LEVEL THE GROUND AT THE STOCKPILE AREA. THE GROUND WAS VERY HARD THE BLADE WAS CATCHING AND BINDING. AT ONE TIME IT CAUGHT HARD AND EMPLOYEE STUCK HIS HEAD OUT THE RIGHT SIDE OF THE CAB DOOR TO SEE WHAT IT WAS CATCHING. BLADE BROKE FREE THEN CAUGHT.
