Plant Foreman was contacted after hours and informed that a transformer had caught fire on the backside of mine property. Local Fire Dept. had already been notified. Cause of fire is believed to be transformer damage due to recent storms.

Two employees were positioning a spray bar into a vise on the back of the truck. One employee let his end slip causing the other end to drop, pinching his left ring finger between the truck and the spray bar. Resulting in a cut to the tip his left ring finger. Five stitches.

Two workers were removing the wear plate on a loader. Co-worker was hammering the plate and a sliver of metal came off and hit injured in the chest. *** The resulting laceration required seven stitches. *** Justifying a reportable medical with no restricted duty and no lost days. ****

EMPLOYEES WERE COMPRESSING A FEEDER SPRING, STRAPPED THE SPRING IN THE COMPRESSED MODE. ONE OF THE STRAPS FAILED CAUSING THE SPRING PRESSURE TO RELEASE MASHING HIS SMALL FINGER ON HIS LEFT HAND.

EE & CO-WORKER (CONTRACT WELDER) WERE WORKING ON CONVEYOR #17 TAKE-UP FRAME REPAIR. EE WAS HOLDING A PIECE OF PIPE THAT CO-WORKER WAS WELDING TO THE FRAME. HE HURT HIS BACK AT THAT TIME.

EMPLOYEE WAS INSTALLING A NEW TAIL PULLEY ON CONVEYOR #4. IT SLIPPED AND CAUGHT HIS MIDDLE FINGER BETWEEN THE CONVEYOR FRAME AND THE TAIL PULLEY CAUSING HIM TO RECEIVE 4 STITCHES.

EMPLOYEE WAS USING A LARGE PRIZE BAR TO FORCE A TAIL PULLEY FROM UNDER A FRAME. THE BELT THE TWOEMPLOYEES WERE WORKING ON WAS ALREADY TENSIONED BY TWO CHAIN JACKS. WHEN THE PULLEY DISLODGED FROM THE FRAME THE FORCE PUSHED THE BAR BACK STRIKING THE EMPLOYEE ON THE RIGHT FOREHEAD. HE WAS GIVEN PROFESSIONAL MEDICAL ATTENTION THAT REQUIRED TEN STITCHES.

ON WED. NOV. 7TH, 2001 @ 2PM, EMPLOYEE WAS OPER-ATING THE JAW CRUSHER, BREAKING A JAM WITH A STEEL WEDGE ATTACHED TO A 1/2" STEEL CABLE. THE EYE BROKE OFF OF THE WEDGE ALLOWING THE WEDGE TO DROP INTO THE JAW. THE WEDGE THEN SHOT UP OUT OFTHE JAW STRIKING EMPLOYEE ON THE RIGHT ARM.

REPLACING RETURN ROLLER ON CONVEYOR #8, USING A COME-ALONG TO LIFT THE BELT. THE HOOK ON THE COME-ALONG DISLODGED ALLOWING THE BELT TO DROP. EE WAS LEANING OVER ON THE BELT APPLYING PRESSURE. WHEN THE BELT DROPPED HE FELL FORWARD HITTING HIS HEAD ON THE CONVEYOR FRAME.

WHILE REPLACING SCREEN PANELS ON 3RD TOWER EMPLOYEE HIT HIS OWN FINGER WITH A 3 LB HAMMER. HE CUT HIS FINGER TIP ON HIS RING FINGER OF THE RIGHT HAND. (3RD FINGER) ALSO, BROKE THE BONE IN THE TIP OF THE FINGER.
