Miner was working on removing stones from a chute when EE started feeling discomfort in back, backend, arms and neck area. Next day miner was in pain.

Employee stepped off the excavator step (maintaining 3 points of contact) onto uneven ground and rolled their ankle.

Driver was dumping material when the cab rocked back and forth causing Miner to hurt EE's back.

While stepping down from a platform, the employee rolled their ankle and injured left ankle/leg. Employee was unable to walk so 911 was called and the employee was taken to the hospital for evaluation.

Employee was closing the hood of the service truck. When they closed the hood of the truck, they noticed a pain in their right upper pectoral area.

Miner was tested for annual hearing loss. Retesting has stated that the miner is above the threshold of 25% deeming a hearing loss on miners part 62.

A few hours after the employee's shift ended EE called saying EE felt a tweak in left shoulder at work while using the high torque wrench to tighten the primary crusher bolts. When EE got home EE noticed some swelling and the pain got worse so EE went to the ER to get shoulder checked. EE was given prescription pain medication.

Employee was cutting rotting pieces of tower floor with a torch. While removing a piece with a pry bar the slag from the previously cut welds hit employee below the safety glasses. Employee reported a burn and swelling under EE's eye.

Employee felt discomfort in EE's right eye while grinding. EE thought EE flushed everything out but woke up the next day with continued irritation so EE went to the doctor. Doctor confirmed there was no foreign bodies remaining in eye but prescribed eye drops out of precaution. **Employee wears prescription safety glasses

While attempting to remove a wheel chock that was stuck underneath the operator's haul truck EE kicked the chock and fractured EE's right big toe.

Reportable hearing threshold shift during annual hearing test. Retested.

Employee was moving a boulder in the pit when they experienced back discomfort. Was not reported until 3 days later.

Head injury due to sudden release of air pressure in beetle drill.

Customer driver exited the truck, stood on the back bumper to clean back of truck, when frozen material came down pinning customers leg between body of truck and frozen material. Customer was able to release their leg and remained on the ground for EMS arrival. Customer was taken to hospital. Compound fractures.

Employee had out patient surgery on right knee to remove a small fragment of chipped bone. Employee on 1/16/19 reported knee discomfort when employee went to start up a power washer that had a hand pull power cord. On 3rd pull, felt knee discomfort.

While lancing lower hoist pin with lancing equipment, slag dripped onto steel toe shoe landing on cloth tongue burning through and top of foot. 1/25 original incident date-no recordable medical attention, 2/6 follow up request - prescribed ointment provided for 7-10 days. (Recordable prescribed ointment)

Hearing testing showed threshold shift in left ear. employee refused to complete follow up test to confirm shift, employee retired October 2017.

While removing the counterweight from a Vibrating Screen Shaft, the Counterweight shifted/motioned to the right due to the location of the chain used to hold it. Employee's right hand remained on counterweight with motion and became pinched with stationary Shaft next to it.

Employee was pulling hose from maintenance truck and hose kinked. EE increased their force while standing sideways and felt right lower leg give out.

Employee operating walk behind skid steer to clean stone spillage bottom level of screen tower and while backing, left elbow struck cross member causing a jam of wrist at controls resulting in a left wrist fracture.

Employee walking on clean conveyor walkway and heard pop in knee. No days away from work and working full duty and Meniscus tear not discovered until 6/3/13 after MRI.

Employee used utility knife to cut a hole in old belt and cut his thumb and received 2 stitches

Employee attempted to remove some buildup on conveyor frame with a 3/4" bar and bar struck moving pulley causing bar to pull free causing laceration to underside of finger

Using jackhammer to remove frozen fine material behind screen deck and when it broke through the edge of material, the jackhammer's downward motion caused left hand finger to become pinched between the handle and the top of a handrail adjacent to work area.

Employee tripped walking behind trailer from personal car and received laceration on ring finger and bruise on knee

Employee was trying to repair a hydraulic hose leak on crusher and hose blew off at connection and hose end struck left shin, resulting in 4 stitches.

Employee was installing a cover plate on loader, when the plate slipped and struck his finger.

Climbing ladder to enter cab of haul truck, employee felt a pop in his knee. He was originally diagnosed with a knee sprain. Further treatment and MRI revealed a torn meniscus. Surgery was performed on 8/28/2007

Employee suffering with chest pains and numbness in arm. Emergency personnel was contacted and employee transported to local hospital where he was diagnosed with a heart attack.

Employee arrived at work complaining of chest pain and arm pain. Pain increased during evening and 911 was contacted.

Emp was struck in the face while hooking up a hose from water truck to crusher.

employee slipped on gravel while walking down the road towards c-1 tailpulley injured his Right side shoulder, elbow, knee and ankle. Injury date ws 10/30/2004 employee did not have surgery until 3/9/2005. Became recordable on that date.

EE was working with another employee to determine a problem with 375 cat. While checking out the equipment a piece of metal shot off striking ee in thigh.

Emp. slipped while walking around back of truck twisting his ankle. Employee became a lost time as of 8/17/04.

Ee was using a pry bar to push fitting on water truck when bar slipped and ee cut pinky finger.

Employee was hit in the foot above his steel toe by a steel impact plate.

EE WAS INSTALLING AN ACCUMULATOR ONTO A CRUSHER BEING REPAIRED ON THE GROUND BELOW THE CRUSHER PLATFORM, USING THE ASSISTANCE OF A BACKHOE, THE EE WAVED THE BACKHOE THAT HE WAS DONE WITH HIM A S THE BACKHOE OPERATOR LIFTED ONE OF HIS STABILIZERS, THE BUCKET DROPPED.

EMPLOYEE CAUGHT GLOVED HAND IN CONVEYOR HEAD PULLEY WHILE APPLYING BELT DRESSING.

WHILE TRYING TO REMOVE A WELDING ROD, JAMMED IN DOOR, EE'S FINGER BECAME CAUGHT IN DOOR.

EE WAS REMOVING IDLERS FROM A 988 LOADER TO REPAIR A CONVEYOR.UPON REMOVING TROFFLING IDLERS FROM LOADER, LOADER MOVED AND CAUGHT 2 FINGERS ON EE'S LT HAND.

EMPLOYEE WAS RUNNING CONTROL TOWER WHEN BF-1 BECAME CLOGGED WITH ICE. EE TURNED OFF BF-1 THEN C8 LOCKED AND TAGGED BOTH. GOT A SLEDGE HAMMER STEPPED UP ON THE LOWER BEAM, REACHED UP AND LOST F OOTING. HIT SHINS ON BEAM ON THE WAY DOWN. EE LANDED ON HANDS AND FACE ON STOCK PILE BELOW.

EE WAS CUTTING ROPE WITH UTILITY KNIFE. KNIFE SLIPPED, CUTTING LEFT MIDDLE FINGER.

EE WAS ASSISTING ANOTHER EE IN PLACING SCHREEN PANELS. THEY WERE INSTALLING ON ROW WHEN THE SCREEN TUBE. ONE EE WAS HAMMERING THE PANEL IN PLACE WHILE STUBLILIZED THE PANEL. THIS EE 'S HAND SL IPPED AND WAS STRUCK BY THE HAMMER BREAKING HIS LEFT LITTLE FINGER.
