Employee was a welder from 10/29/04 to 6/8/10. He claims that he developed CTS as a result. Claim's being contested-Med. from emp. Dr. says may be work related 6/30/11.
Rock from working face rolled onto bucket while mucking pile causing loader to jolt. Employee complained of pain in back. Dr put him off work as of 3/11/10.
Emp. states that he was walking from his haul truck to the lunch room. Passed by conveyor & stepped on a rock twisting his rt knee. Orig report as a notice only.
After loading 20 holes in an 80 hole shot back began to get sore and tighten up. Continued & finished loading shot. Did not seek medical attention for 3 weeks after date of injury.
