Employee was walking into the "Control Room" when employee tripped and fell on left knee.

Employee was helping to unload parts from the Low Boy trailer EE had driven when EE stepped off the back of the trailer and fell to the ground. When EE fell, EE hit head on the pavement.

Not reported until 5/7/10-Miner was cleaning under crusher platform when he stepped on a 4" rock and twisted his right knee while pulling water hose. Miner's first day of lost work was 6/23/10 for surgery.
