The employee was assisting in loading an X-ray machine into the back seat of a pickup truck. The weight of the machine shifted in EE's hands unexpectedly, resulting in EE's hand being pinched between the corner of the machine and the center B pillar/door jam of the vehicle, resulting in a laceration to the right ring finger.

Maintenance Supervisor twisted leg and fell while removing snow from sidewalks around office, worked the rest of the day, then taken to hospital due to not being able to put all EE's weight on it the next day, told it was a sprain and put on restricted duties.

Maintenance employee was fabricating mounting bracket for oxy/acy. cylinders to be mounted upright in new Ford shop truck. A pipe hanger was being modified into a mount when the S.S. Disk on grinder became entangled with uniform shirt pulling itself into their lower right abdomen, resulting in a laceration needing 8 sutures at ER.

Dredge Operator went downstairs to visibly inspect a cable on the Dredge. The employee noticed the employee forgot camera in the control room. As the employee was climbing up the stairs to the control room, employee slipped, fell on right hand, and fractured 5th metacarpal. It was raining at the time of the incident.

The operator was cleaning branches off the Dozer and slipped off the dozer onto the ground. There did not appear to be any weather conditions that affected the accident as it was a partial cloudy day with no rain. When the operator fell, the hit injured right arm.

Employee was attempting to remove a loose pipe from sump 16. As he was pulling on the pipe, he did not realize that the pipe was still attached to the sump. He injured and pulled the muscles in his left arm. He was taken to the company doctor for evaluation and placed on modified duty with a 10 pound lifting restriction.

Employee was closing lids on top of a railcar. Primarily due to unsafe footing of the employee's right foot, his foot twisted and his knee went between the grating and the lid of the railcar. Employee was taken to Inspira Occupational Health (Company doctor) for evaluation. The doctor placed the employee on modified duty due to contusion and sprain of his right knee.

Loadout operator was carrying a sample cup down the steps at rail loadout. When he approached the 3rd step from the bottom, he tripped/fell and reached with his hands out and landed with his full weight on both wrists. Loadout operator was in a hurry during the incident.

Employee was at rail loadout picking up railcar shipment samples. She was talking to a co-worker. As she turned around, she tripped over the "pry" bar that was leaning against the handrail. She fell on her left side and sprained/strained her left forearm area.

While spotting railcars ee stepped out of the walk way and into the area of the car puller. His feet got tangled into the two cables and he fell forward. He called his supv. and reported the incident. He felt pain in his back and neck. He was taken to the company doctor for evaluation. The Dr. restricted the ee work duties. Unimin placed ee on modified duties.

Employee was rushing down stairs at rail loadout. At about third step from the bottom he twisted his ankle and fell the rest of the way down. Employee's safety shoes were not properly laced up which would have provided support to his ankle. Further, he stated that he was not using hand rails to support himself as he came down the stairs. Employee twisted/sprained his right ankle.

EE WAS DRILLING A HOLE THROUGH THE BULKHEAD OF A WORKBOAT. AS THE DRILL BEGAN TO BREAK THROUGH THE OTHER SIDE, THE BIT CAUGHT AND THE DRILL HIT HIS HAND. HE HAD HIS THUMB AROUND THE TRIGGER AND WAS HOLDING THE DRILL IN A WAY IN WHICH IF THE DRILL BIT CAUGHT ANYTHING IT WAS POSSIBLE TO CAUSE AN INJURY.

THE HEAD DRIVE OF THE BE-03 ELEVATOR WAS BEING REPLACED BY EE AND 3 COWORKERS. COWRKER WENT TO GROUND LEVEL TO HOIST UP ANGLE IRON. THAT DONE, HE FAILED TO CLOSE HOIST AREA. LATER THAT AFTERNO ON, EE FELL INTO THE OPENING BREAKING HIS HIP JOINT IN TWO PLACES. HE WAS RESCUED FROM THE TOP OF THE 100' ELEVATOR AND TAKEN TO THE HOSPITAL.
