Saddle Mountain LP · Caught in, under or between a moving and a stationary object

Worker was performing maintenance on their excavator. While doing so, EE was struck/pinned between the grapple attachment and excavator track. The worker sustained severe injury to the left hip, upper left leg and lower left leg.

While attempting to clean up the QP pile an employee backed into the Humble Family truck. Prior to the accident an EE communicated to Humble Family truck to standby. The Humble Family truck disregarded the directions and drove behind the loader which resulted in the accident. The driver of the Humble Family truck complained of back pain and was taken to the hospital.

The employee was taking a nut off the head of the crusher when the nut fell hitting EE in the right forearm below the elbow causing a cut requiring 8 stitches.

Employee was installing a discharge chute to a screen. Bolt holes did not line up. He inserted a pin pry bar to aid in lining up bolt holes. As he began to pry on chute, the pin pry bar slipped striking the employee in the forehead. He received 10 sutures.

Employee was operating an IT-14 front end loader on the quarry upper access road when the loader rolled and ejected him and landed on his legs. It is believed that he was operating the machine with the door open and no seat belt. His legs were crushed and he lost one above the knee and one below.

EE WAS CHANGING THE CUTTING EDGE OF A CAT IT 14 WHEN THE BOLT SPUN AND CUT HIS LEFT MIDDLE FINGER. A VERY SMALL CUT, HE RECEIVED A COUPLE OF STITCHES.
