Employee walked from driver side of vehicle around the back of the truck and struck his face against a sign post that was sticking out from bed of truck.
Slipped while descending steps of 992 loader and fell to ground. Fractured finger.
While removing a grease fitting on a cylinder, the grease ejected into the employee's thumb.
A loader and haul truck struck each other, causing the employee to hit head against the radio system attached to the roof of the cab.
While exiting a dozer, during rainstorm, employee slipped (did not fall), striking leg against dozer.
EMPLOYEE WAS CUTTING A SCRAPER WITH A UTILITY KNIFE FOR CONVEYOR #14. UTILITY KNIFE SLIPPED CUTTING EMPLOYEE'S RIGHT THIGH. LACERATION REQUIRED FIVE (5) STITCHES.
EE WAS WALKING UNDER A CONVEYOR BELT. DUST/DIRT COMING OFF BELT ENTERED HIS RIGHT EYE.
DETERIORATED AND RUSTED FLEXIBLE CONDUIT (ELECTRICAL) WAS BEING REMOVED FOR REPLACEMENT; WHEN EMPLOYEE PUT PRESSURE ON CONDUIT TO REMOVE FROM FITTING, CONDUIT BROKE APART IN HIS HAND, GASHING PALM.
EE WAS ATTACHING A HOSE TO THE WATER TRUCK. HE WAS STRUCK BY HOSE CAUSING HIM TO LOOSE HIS BALANCE AND FALL.
EE WAS GRINDING A 992 LOADER HIGHLIFT BUCKET WITH A 5900 RPM MILWAUKEE GRINDER WHEN THE GRINDING WHEEL CAUGHT ON A CUT IN THE BUCKET AND CAUSED THJE GRINDER TO KICK HITTING THE EE IN THE LEFT LEG JUST ABOVE THE KNEE.
EE WAS INSTALLING A WEAR PLATE ON THE CHUTE THAT FEEDS 3 & 4 CRUSHER. THE PLATE WAS 1" THICK MEASURING 14" X 18". THE PLATE WAS BEING HELD IN PLACE BY ANOTHER EE WHEN HE LOST CONTROL AND THE P LATE SLID DOWN ON INJURED EE'S LEFT INDEX FINGER.
