Operator working next to open hole about 4 feet deep building berm. Machine on iced roadway working parallel to hole without berm protection when movement of boom caused machine to slide into hole onto its side. Cab damage and operator had cut hands with no stitching required. Minor concussion. Released from ER within 4 hours

Employee was trying to rotate the cone by pulling the belts by hand. As the belts rotated the employee right pinky finger entered the pinch point between the outer belt and the shiv grooves. The plant was properly locked out at the time of the incident. The employee received 3 stiches to the finger as a result of the incident.
