Nevada Gold Mines LLC · Caught in, under or between a moving and a stationary object

Ee helping move brassboard weighing approx 200lbs from lineout room to the rear of flatbed truck. During team lift, in process of lowering brassboard onto flatbed, ee's left index finger was pinched between edge of brassboard and a toolbox on the flatbed. Seen at clinic, referred to ER for ortho consult. Fracture, released to full duty.

Nevada Gold Mines LLC · Struck by rolling or sliding object

EE was using rap wrench to break bonds on a bundle of spiling (pipe). Pushed wrench on last bond, bond broke, causing the pipe to roll down, pinch ee left middle finger. Seen at ER, xray negative for fracture, sutures required to close the wound.

Ee was tying leads on det cord when some material fell, striking the ee on the head, shoulders, and back. Seen at ER, diagnosed with fractures to the sinus. Released to full duty. Follow-up with ENT confirmed no further care required.

Ee was installing a 90 degree elbow on the back of a sprayer. The elbow fell, landing on the employee's left hand. Diagnosed with fractures to the index and middle finger and put off work pending orthopedic surgery.
