EE was sprayed by another EE with water from a water cannon. Seen at clinic, diagnosed with a contusion and on follow-up referred to PT and MRI. Released to modified duty.

Nevada Gold Mines LLC · Struck by falling object

Approximately 30 kg rock released from the face near the springline in active underground workings. Rock fell 5-8 feet and impacted the miner's arm against the workbasket handrail. Rock was partially restrained behind overlapping face mesh. Miner sustained arm injury.

Bolter operator and mechanic were aligning carousel on bolter. Ee using a short scaling bar to rotate carousel for alignment. While pulling back, the bar slipped off the carousel collar, causing ee to stumble backwards and fall, hitting EE's head on bolt rack on front of bolter. Seen at ER, lac closed with staples, released with restrictions.

EE was working on a pressure washer. They plugged it in and was checking belts. They leaned over onto the power button. The washer turned on, and the pulley belt pulled their glove in, amputating the tip of their left middle finger. Seen at ER, diagnosed with traumatic partial amputation, released to modified duty.

Ee was drilling and stopped to install collar pipe in face and rock fell, hitting ee on head. Seen at NNRH, diagnosed with lacerations on scalp and face, treated with staples and sutures. Released to full duty 10/22/25.

Ee was showering after work using Dawn dish soap. Soap fell off shelf, ee tried to catch it, squeezed soap in eye. Seen at clinic, diagnosed with irritation, put off work. Seen at Total Eyecare the following day, diagnosed with abrasion to right eye, put off work until full duty release on 9/30.

A heavy toolbox being moved on fork tines fell to the ground. Two employees attempted to manually stand it upright. During the lift, one employee lost control, causing the box to shift and trap the other employee's finger between the box, the ground, and EE's boot, resulting in a laceration. (requiring several sutures)

Ee was moving auger and pinched right ring finger between auger and fitting on the pug mill belt. Was seen at ER, diagnosed with open fracture, referred to orthopedic hand specialist, released with restrictions.

Issue not immediately reportable, however NGM did report as ee was found unconscious in EE's truck at end of shift. Ee transported to ER, diagnosed with a non-occupational seizure, and released to full duty effective 04/12/25.

Ee lifted a pump weighing approximately 30lbs and carried it. Ee reports feeling a "thud" in ee's back and having back "lock up". Ee reported this to workers' comp on 3/10 and was seen at the clinic on 3/11. Diagnosed with sciatica and a strain, aggravating a pre-existing work related injury. Released with restrictions.

IP was removing drill pipe out of presenter on a DU431. IP noticed pipe was hotter than usual and went to place pipe on rack and caught finger between pipe racks ear and pipe. Seen at U of U on 12-26-2024 Diagnosed with Longitudinal fracture of the middle phalanx involving the PIP joint on right index finger requiring 11 stitches and 2 Pin. Surgery scheduled 12-27-24

Ee was chipping slag off door with a chipping hammer, and a piece of slag flew up under EE's safety glasses and got in EE's eye. First aid was provided and ee seen at the ER, released to full duty, and referred to an eye doctor. Eye doctor also released to full duty. Diagnosis corneal abrasion, prescription given.

EE was grabbing a 10-foot HQ rod from the rod sloop, to add to the system. EE tripped, catching their right foot on the sloop frame, causing EE to fall with the rod still in hand. As EE landed, their left hand was caught in between the outer rod sloop frame, and the rod, which caused a laceration on their left ring finger.

The employee was greasing the tracks on an Excavator and felt pain in the low back after climbing into the operator's cabin. The employee reported previous back issues, and this was considered an aggravation of a pre-existing back problem. MRI results received on 8/21/2024 and are indeterminant if the results were pre-existing.

Cable to dumpster caught behind no parking sign. EE went to get cable unstuck, the cable came free, lid shot up, catching their left hand between cable and sign. EE put off work pending surgery on 6/4. Released with restrictions effective 6/10.

UG supervisor was performing a routine inspection and discovered a ground fall on the 6190 North-150. It was the 2nd round in, and it was unbolted, it was working and broke back into the last wrap of supported ground. It was starting to work past the ground support and it was decided it needed reported at 13:30. MSHA was called at 13:39, and had the area barricaded off.

Employee was loading trucks with an LHD and started smelling smoke. EE noticed smoke in the air, and hit estop on LHD. When EE got out, EE believed EE saw flame under loader and deployed fire suppression. EE called a Mayday, and mine was evacuated.

Ee reports stepping off lube truck and EE's ankle twisted causing a sprain. Seen at ER then at Golden Health. X-rays negative for fracture. Ee diagnosed with ankle sprain released to full duty for next scheduled shift.

EE was walking through an air door to get to EE's haul truck when EE was hit by a gust of air and fell over. EE struck their head. EE was seen at the ER, x-rays were negative, and a small puncture wound on EE's head was closed with two staples. Released to full duty.

While walking around truck checking to confirm tire chains were in place driver slipped on ice/snow. While falling driver put right arm down to break the fall and caused injury to right shoulder.

While backfilling a stope from the 6245-118 top cut, a Sandvik LH621i loader traveled beyond the initial location of the berm with a full bucket of cemented rock fill (CRF). The loader slid down the angle of repose of the CRF fill and came to rest part-way down the slope of CRF. The loader operator was able to exit the cab of the loader with no injuries.

Ee working underground when a mud wedge fell 18 feet from the face. Ee struck by rock. Transport to a medical center from site by air med. Ee diagnosed with concussion and contusions to back. Released to work full duty per the C4 effective 10/29/22. Ee scheduled to follow-up with occupational medicine at medical center 11/3 for clarification of RTW status.

EE was bolting underground. As EE was backing away from the bolter, EE slipped and fell to the ground causing an abrasion to left hand and "jolting" EE's left shoulder. EE reported pain in the left shoulder. EE was seen at the ER and Golden Health. X-rays negative for fracture, EE referred out for an ortho consult 3/18. Restricted duty.

This was a death by natural causes. Employee collapsed at the mine site at the employee's truck, but passed away at the hospital in Elko, NV. There was no actual accident.

DURING CLEANUP FOR THE DAY WHILE CUTTING RUBBER ROD FOR FLOORING, CONTRACTOR CUT SELF WITH UTILITY KNIFE ON THE WRIST. A BANDAGE WAS APPLIED BY THE EMPLOYEE AND EE WAS NOT CONCERNED, FINISHED CLEANING FOR THE DAY AND LEFT SITE. ON THE WAY HOME DECIDED TO HAVE IT CHECKED OUT BY URGENT CARE, RECEIVED 8 STICHES.

The operator of a mucker was loading a haul truck out of a muck bay at the 6245 level. While backing out of the muck bay the operator had the bucket in the air while fully articulated and on a 2-3% grade. The mucker became unstable and slowly tipped against the rib. The operator was not injured but needed a man basket to exit the mucker.

Operator was moving empty emulsion totes from the UG powder mag to the surface, operator stated they smelled a burning odor from the loader. Operator asked two other employees if they smelled it. They stated yes and inspected under the hood compartment, did not observe any visible signs of a fire. When the two employees went to leave they saw sparks/flames under the loader.

EE was placing a cab hook, cable jerked the cab hook and stuck EE's right hand ripping EE's glove and stabbed EE in the webbing of EE's hand between EE's thumb and index finger.

While working on a cooling pump of a 4 yard loader, the access door came unlatched hitting the employee on the back of EE's hard hat. When hit on the hard hat the force thrust EE forward causing EE to hit EE's nose on the loader frame.

Empl. was being loaded when a rock dropped shaking the truck. Next day at 9:00 am empl. complained of neck pain. Empl. was evaluated by Physician & released to work with cautions to avoid "jarring and neck motion". Empl. chose not to return to work at this mine and has been permanently transferred to another non-mining job.

2 Labors and skid steer operator were installing French drain around portal sets. Labor was holding fabric told the operator to back up while that was happening labor started to stand up while operator was tilting bucket back and pick hand between bucket and wall.
