Employee was shoveling off a conveyor belt. Employee felt a pop in their back, and felt immediate pain.
Employee was removing access cover bolts, adjustable wrench slipped off bolt and smashed employee's right middle finger against crusher.
Operating articulating truck, was being loaded with material. In the course of this operation, a large boulder was loaded onto the bed of the truck rocking operator against the armrest.
Employee was using a cutting bolt holes on a conveyer with a torch and had a piece of slag fall into his left ear.
