Employee's right hand was crushed between the knuckle and wishbone on an A-40 haul truck when the hydraulic jack they were using released during the removal of the bearing and race, causing fractures and laceration to the right hand.

At approximately 3:00am a ground failure occurred at the HB pit on the east side access road. Ground failure was approximately 150 in length and approximately 46 feet in width, blocking access to the pit via the east access road. Extremely wet conditions, coupled with freeze/thaw episodes contributed to the failure. No persons were injured or trapped during this ground failure.

Employee (HEO) was walking backwards and tripped over the implement of EE's blade at ground level while conducting a pre-op on that equipment. Employee injured EE's neck and required possible medical treatment. The diagnosis is still outstanding due to lack of specialist care availability and a request to reclassify this injury may be made in the near future.

a mine mechanic had a seizure while assisting in the maintenance on the Hitachi ex600 excavator. the employee had suffered a head injury in 2013 from a motorcycle accident. this is believed to have been the contributing factor that caused this incident.

HE RECEIVED A STRAIN/SPASM TO HIS RIGHT MID UPPER BACK. TREATING DR. GAVE EE SOME MUSCLE RELAXERS AND PAIN MEDICATION AND REFERRED HIM TO A CHIROPRACTOR THE SAME DAY. THE CHIROPRACTOR MANIPULATED HIS BACK & RELEASED HIM TO FULL DUT

EE STATED THAT HE WAS STEPPING UP IN TO THE MOTOR GRADER (BLADE) WHEN HIS FOOT SLIPPED OFF LOWER STEP CAUSING HIM TO LOSE HIS BALANCE AND FALL TO THE GROUND. EE INJURED LOWER BACK. HE'S HAD A HISTORY OF BACK PROBLEMS, NOT JOB RELATED. HE'S HAD FIVE PRIOR BACK SURGERIES.

HE INJ HIS LOWER BACK & HAD A SHOOTING PAIN IN HIS LEFT LEG. THE HOSP HAS SCHEDULED HIM FOR AN MRI HE WILL NOT RETURN TO WORK UNTIL WE GET RESULTS OF MRI & HAS FULL DRS RELEASE.

ON MON, 3/12/01 AT APPROX 9:30 AM, EE STATED THAT HE WAS CUTTING A BATTERY TERMINAL WIRE ON A SEMI-TRUCK. EE WAS HOLDING ON TO WIRE WHILE CUTTING, THE KNIFE SLIPPED OFF WIRE AND CUT LEFT PALM OF HAND. EE WAS TAKEN TO HOSPITAL WHERE HE RECEIVED TWO STITCHES.

EE WAS GETTING IN TO THE MINE WATER TRUCK WHILE HOLDING A HAMMER IN HIS HAND. HE WAS STEPPING ON TO THE LADDER AND LOST GRIP ON THE HAMMER. HE STARTED TO GRAB FOR THE HAMMER AND LOST HIS BALAN CE AND SLIPPED WITH RIGHT FOOT. HE GRABBED FOR THE HANDLE ON WATER TRUCK AND CAUGHT HIMSELF IN MID AIR. HE SNAPPED HIS NECK AND SHOULDER.

EE REMOVED SAFETY CLASSES AND THEN RUBBED HIS EYE. METAL SHAVING FROM DRILLING WERE ON SLEEVE, GOT INTO EYE EE'S EYE BECAME RED AND SWOLLEN, HAD TO GO TO DR AND HAVE METAL SHAVING REMOVED.
