EE received stitches after laceration to right elbow area. Employee received laceration as a result of horseplay in 4x4 vehicle.

Foreign object in EE eye while air lancing fly ash tank. Debris flushed out, but employee received antibiotic as preventative care.

Treated at hospital due to heat exposure. EE sustained heat exposure after entrapment 503.7J1, process passenger elevator, for 3 hours. Removed by fire and rescue.

EE was struck in chin by rock thrown by a 2nd employee. The struck employee required 3 stitches.

EE experienced a strain to neck and left trap muscle while actuating a butterfly valve. Injury occurred on 6/12; was treated with first aid. Pain continued & EE was seen at clinic on 6/14; was placed on restricted duty.

EE exited elevator on top of truck silo when they stumbled. They did not fall, but stepped awkwardly and stressed their left knee. EE felt pain in left knee. Sprain to left knee, restricted duty until MRI can be performed.

EE was clearing brush with a weed trimmer on 08/08/2022. It was determined that the EE contacted poison ivy at some time during shift. EE reported poison ivy to management 08/15/22. On 08/16/22 employee received steroid injection to alleviate symptoms.

EE struck by falling object. EE was inspecting bucket elevator in plant when light fixture fell striking a glancing blow to EE's hard hat. EE had been treated with first aid until 8/26/22 when physical therapy was prescribed.

On 7/22/2022 the employee was inspecting the gate on Silo #24 looking into the inspection porthole. The employee struck left front tooth on the porthole flange. On 7/28/2022 employee was seen by a dentist and received care for a chipped tooth.

On 04/18/2022 the employee was using a pipe wrench to open vessel doors on the HW Kiln Preheat Tower. The employee felt a pop in right shoulder while trying to open a vessel door. On 4/21/2022 employee was placed on restricted duty.

A repairman was loosening a spectacle valve and lacerated a finger when the nut broke free resulting in them smashing a finger between the transfer pipe and the loosening tool. They received 4 stitches and was diagnosed with a chipped bone.

On 2/22/22 the employee was using an air lance to clean a chute. In the process, EE hit a frozen chunk of material that was hidden behind non-frozen build-up. This caused a strain in EE's right shoulder from the jamming of the lance into the build-up. On 3/2/22 EE was placed on restricted duty for lifting capacity.

The employee was performing inspection functions of the job around the clinker cooler when cooler dust exited from a pinhole in the cooler sidewall. The dust got behind EE's safety glasses and into the left eye. The employee was treated for an abrasion to the left cornea and prescribed preventative antibiotic drops. No vision loss or permanent damage occurred from this incident.

The employee was lifting a vibrator (approx. 20-25 pounds) to place it onto a rail car hopper and felt a strain on left rib cage.

The employee was cleaning down a chute with a pick. The employee hit a chunk of material and felt a pop in the left shoulder.

Employee was scraping down a synthetic gypsum chute of material when employee pushed the scraping tool into a large chunk of material and felt a pain in left shoulder/ left pec.

Employee was disconnecting the supply air hose from the air arc device. A stream of air from the quick connect air chuck shot up & under employee's glasses into left eye. Employee was taken to a doctor the morning of 06.15.21 where employee was evaluated for an air abrasion to the left eye. Employee was prescribed antibiotic eye drops as a preventative measure against infection.

Employee opened a port to try and clear the chunk after closing the manual gate. Once the chunk cleared, built up pressure in the line was released. This resulted in cement dust being blown into EE's face, knocking off safety glasses. The employee utilized an eye wash station to clean out eyes. Within several hours after the incident reported having pain in the right eye.

Employee was cutting the wire tie off a bag of cement with a utility knife. When it cut through the tie, the force continued into employee's thumb on left hand.

Injured employee was welding in the shop and in the field on what we believe is the day of the incident. EE does not recall being flashed or having issues with welding protective eye wear. That night, EE was awakened by a burning sensation in EE's eyes. EE went to the doctor and was diagnosed with ocular welder's flash. Prescription medication provided.

Injured employee (IE) was struck under the chin by a blunt force causing a laceration requiring five sutures; MSHA recordable incident. IE used the portable industrial shotgun, through an eight-inch porthole, to help knock down material in a silo. When material fell it pushed the shotgun barrel down and the stock up. The stock hit the miner in the chin causing a laceration.

Injured employee (IE) slipped and fell walking down a recently shoveled outdoor forklift ramp. It's believed left hand struck an adjacent guardrail or toe board while falling. Overnight it hurt and swelled so they sought medical attention the next morning. Doctor's x-ray did not show broken bones; IE placed on restricted duty and Rx meds prescribed.

Process Utility Repairman strained left shoulder while manually cleaning out a material chute with a handheld scraper. Discomfort continued for a couple of hours afterwards so employee sought medical attention and was placed on restricted duty (keep elbow at side at all times until next visit).

Injured employee strained EE's knee after stepping in a small, approximate one inch deep, cavity in a concrete covered area. Time didn't make it better, so EE's doctor ordered an MRI. The MRI revealed an issue requiring surgery. Injured employee had surgery 02/24/20 making this a reportable event.

A process utility person was stuck in a passenger elevator just above the 3rd floor. Elevator service company was called for assistance, but since it would be over 30 minutes until employee was out MSHA was notified (entrapment). 103 K order issued and released the following morning by on site inspector.

On August 6, 2019 the injured employee tripped and scraped their right shin on railroad track while clearing brush along the track line. First aid care was self-administered. On August 11, 2019 the leg was hurting so they went to the doctor and was diagnosed with an infection and prescription medication was administered.

Injured employee smashed right hand pinky finger between a shaft cover/guard and the top rail of a guardrail system while manually handling an approximate five-pound shaft cover resulting in a doctor's visit, five sutures, and prescription medication.

After 4:00 break walking back to mill collapsed from Heart Attack. Two G&R employees applied CPR. Later died at hospital.

Injured Employee (IE) was working on an air slide in the mill building. IE was breaking for lunch and walked away from the worksite, but returned to drop off knee pads. After dropping them off, IE turned, took a step, tripped on air slide screen, and fell onto left shoulder. IE sought medical treatment, was diagnosed with a shoulder sprain, and placed on restricted duty.

One inch cut on left hand while removing keeper for chain connecting link with pliers. When they pulled back, the link struck the left palm causing a one inch laceration. Due to the location of the cut, the doctor thought it best to place four sutures to keep it closed/help it heal.

Employee was traveling down in a passenger elevator and it stopped between the third and second floor. Elevator service company called/responded. MSHA also called because it would take longer than 30 minutes to get the employee out. Employee out in approximatively two hours. 19 Communication maintained. No injuries.

Injured employee was clearing chunks in an air slide. During the process, dust got past the safety glasses and irritated eyes. Employee rinsed eyes, but the left eye still felt a little irritated. The next day employee decided to go to the doctor and the doctor issued prescription eyedrops making this a MSHA recordable event.

Injured employee got dust in the left eye while cleaning under the clinker cooler drag. Irrigated the eye at work and thought a shower later that night would take care of any remaining irritation. The next day the eye was still bothering them, so they decided to go to an eye doctor. The eye doctor prescribed ointment and it felt better later that day.

On 2/22/18, Injured Employee (IE) tweaked back moving a wheelbarrow full of material during a housekeeping task. On 3/20/18, IE went to doctor because ee was not getting better. IE's doctor stated back is sore, ee needed time to heal, and placed ee on two months of restricted duty.

Injured employee (IE) was backing a forklift down a concrete forklift ramp. When the forklift got near the bottom a piece of the concrete broke loose and caused a hole approximate 12 inches in diameter. The forklift tire went into the hole and the front end of the forklift quickly dropped approximately four inches. The sudden drop and stop jarred the IE and injured IE's back.

On 18 Jan 18, injured employee (IE) strained left knee while moving 40' of vac truck hose. IE felt slight pain, reported the incident to supervisor, and started first-aid/self-care treatment. On 29 Jan 18, IE wanted to go to the doctor because IE felt the knee was unstable. The doctor prescribed six physical therapy treatments and the IE is feeling better.

Injured Employee (IE) was torch cutting an approximate 3-inch hole through 3/8-inch steel with refractory behind the steel. Unknown to IE, a piece of molten metal fell, hit IE's blue jeans and caught left lower pant leg on fire. IE's coworker noticed and smothered the fire. The result was an approximate 3-inch diameter second degree burn.

While backing a 775 CAT haul truck to dump material the rear wheels slide sideways into a slight depression and slowly tipped onto its right side. The driver was wearing seatbelt, was assisted out of the cab by a coworker, but bruised inner left thigh during the incident and was place on light duty.

On 15 Oct 17, employee was tasked to replace a light bulb. While doing so, the heavy light cover came off and either struck or strained a finger during the attempt to stop the cover from falling. Initially they thought the finger was sprained, but a follow up doctor's visit on 17 Oct 17 revealed a broken ring finger.

Injured employee was tasked to repair/replace a transfer pump seal. While removing the pump impeller with gloved hands, the unexpected sharpness of the blades cut through glove and into left middle finger. The cut required three sutures.

The injured employee was operating a forklift on a gravel drive / parking area to empty a material tube. When returning to EE's area the forklift was struck by a semi-truck traveling at approximately 5 mph. The impact jostled the lift and operator resulting in a cervical and lumbar strain and restricted duty.

Injured employee was attempting to seal a leaking port hole on a cement silo. Cement dust got under or around EE's safety glasses and into EE's eyes. EE rinsed EE's eyes thoroughly, but an hour later felt there was still dust or a scratch in EE's left eye so EE was taken to the doctor for additional care. EE received prescription eye drops for EE's left eye.

Employee was using a kiln ring blaster eight-gauge shotgun to remove a ring in the back of the kiln. After approximately thirty shots, three small pieces of material or shotgun slug projectile ricocheted back and struck face resulting in two sutures on the bridge of nose.

Employee was drilling a 2.5' hole for an additional air sweep in a bag house. It was outdoor work in windy conditions. EE didn't note debris getting in eye during the task; however, the next day EE's eyes were irritated so EE sought treatment. Doctor found a dust size particle imbedded in right eye. Doctor removed particle & EE received Rx meds. Safety glasses worn.

On 3/15/17, employee tripped, fell on right knee, and experienced minor knee swelling. EE was recovering as expected until EE started performing a lot of maintenance work on EE's knees. On 3/31/17 EE's conditioned regressed and EE sought treatment from EE's PCP. X-rays revealed no broken bones; diagnosed with deep tissue bruise on knee cap and placed on restricted duty.

Three contract miners were reported stuck in the Process passenger elevator between the first and second floor. The on duty supervisor called our contract elevator service company and the miners were out in approximately two hours later. Communication was maintained. No injuries sustained.

An employee entered the elevator at the second floor to go to the seventh floor of the pre heat tower; however, the elevator stopped just before the third floor and wouldn't move up or down. Contract elevator service company called and the employees was out of the elevator after approximately 1.5 hours. The issue was a bad reverse phase relay.

Two employees were going up to the burner floor in the freight elevator. The elevator stopped and they were stuck in the elevator. Our contract elevator service company was called and responded quickly, but employees would be in the elevator longer than 30 minutes; therefore, MSHA was called. Total time in elevator was 45 minutes. Communication was maintained. No injuries.

The mill building elevator stopped between the first and second floor with one employee inside. Employee was going to be stuck for more than thirty minutes; therefore, MSHA was called. The elevator contractor arrived on site, determined a 3 amp control fuse had blown, replaced the fuse, and the employee was released. Total time in elevator was 55 minutes.

Injured employee was outside and walking towards the mill building entrance. As employee approached, slipped on ice, that was on a vehicle pathway, and fell on back. Employee reported a sore lower back and was instructed to implement first aid/RICE. Four days later employee felt they needed to go to the doctor and was placed on a 20 pound lift restriction.

Injured employee (IE) was assisting in the retraction of an inlet probe by means of a hand operated chain lever hoist. During the retraction process a ¼" eye and eye wire rope sling being used failed/broke and the rope snapped back and struck the IE's right wrist and IEE also lost balance and fell onto left buttocks resulting in wrist and coccygeal strain and contusion.

Injured employee was troubleshooting a baghouse tipping valve. After isolation and removing material (lime dust) EE moved in for a closer look through an approx. 1/2"H x 8"W opening in the valve. Unexpectedly, dust came out of the opening and hit the employee in the face resulting in dust getting into the employees left eye, medical treatment, and prescription medication.

The mill building passenger elevator stopped between the first and second floors with two employees inside. Employees were going to be stuck for more than thirty minutes; therefore, MSHA was called. The elevator contractor arrived on site, determined a control fuse had blown, replaced the fuse, and the employees were released. Total time in elevator approximately 1hr 17 min.

Injured employee (IE) and two others were pinning a feeder. During the work the IE states they felt a slight discomfort in the left abdomen. IE continued to work without incident. Three days later went to the emergency room because of pain, was told they had a hernia, and the doctor said stay home for one day. IE is on a 10 pound lifting restriction.

Three contract office cleaning personnel contacted the control room operator and told EE the passenger elevator stopped working. The operator notified the shift supervisor. The shift supervisor told the operator to have the contract elevator company respond. They were in the passenger elevator for more than 30 minutes and MSHA was notified of the event.

Injured employee (IE) was trouble-shooting controls for an overhead hoist from a coal mill walkway. IE left the area and fell/stumbled forward while transitioning from one walkway to another. IE's right ear struck the mid-rail of an adjacent guardrail resulting in a 1cm laceration requiring two sutures. This is the first time the IE worked in the area.

Injured Employee (IE) was moving/lifting a mobile stand so he could remove a screw trough cover to replace a broken dust collector screw. The stand got hung up so he lifted it a little and it shifted slightly down instead of up, as expected, and pinched his right thumb tip between the stand and the screw trough cover. Result was a fractured thumb tip and nail avulsion.

Injured employee was cutting a zip-tie holding four half inch air hoses together. After positioning his utility knife blade between the zip-tie and hoses he pulled the knife upward. Apparently, his gloved left hand was closer to, or was pulled closer during the cutting and pulling action, the blade than expected because his left hand/thumb area was cut and required six sutures.

The injured employee was tasked to drill holes through the ACL Kiln pelletizer pan so new liners could be installed. Apparently, and unknown at the time, a piece of material got in his left eye. After work, he sought medical attention for eye pain and his eye doctor removed a foreign body and eye drops were prescribed. Incident reported/investigation began 04/21/15.

Cutting out angle inside of the kiln and when he cut the piece, there was another piece that was already cut which he didn't see causing the angle to fall. He fell down inside the kiln and landed on his right side shoulder and arm.

Injured employee (IE) and a coworker were removing a pipe (10 round, 2 long, approx. 30 lbs). IE was holding the pipe with gloved hands to control it when it became free. Once free, the pipe slipped from his left hand and the weight moved his right arm downward quickly. His right elbow struck the corner of a ladder resulting in a hyperextension and bicep tendon tear.

The injured employee (IE) was using an approximate 2" round by 6' long pipe to slowly rotate each bypass fan blade for cleaning via air lance (vibration issue). While applying downward pressure on the bar the end contacting the fan blade slipped and the end the IE was holding struck the upper rib resulting in a chest wall contusion/bruise and 10 pound lifting restriction.

While the injured employee (IE) was descending the front fixed ladder on a Terex haul truck a side strap of the bottom flexible step broke causing the IE to slip and his left foot to get tangled in the side and bottom of the strap/step resulting in a sprain left ankle.

While using a 3/8" rod to help clear chunks of material from a 10" porthole cement unexpectedly followed and it is believed that is when cement/cement dust got into his eye. His eye became irritated as the day went on and at 1320hrs he was transported to the eye doctor and diagnosed with a scratch to his left eye and eye drops were prescribed.

On 10/31/13 employee reported a sharp pain in his left shoulder, but related the injury to a task performed on 10/29/13 when he states he developed a sore shoulder. Home care/FA instructions provided and normal work duties followed; however, on 11/13/13, employer discovered employee went to PCP on 11/11/13 and was given restrictions and Rx medication for shoulder injury.

Cleaning mixer, glove of right hand got caught on paddle which pulled hand in. RIGHT HAND laceration, required 13 stitches. No broken bones, no severed ligaments or tendons.

Passing brick. A brick fell and landed on two fingers of left hand.

IE helping Co-EE 15' off ground inside chimney a piece of gunite fell from 200' and hit his hand. Headache rack 3/4 plywood over head and complete pik covered and reached out cutting wire mesh with bolt cutters when he was struck by gunite. Rough estimate on years of experience due to the fact get 1 or 2 jobs per year and sometimes none.

The injured employee (IE) was cleaning chunks of frozen material from under a stopped quarry beltline. While the IE was manually handling a 40 pound piece of material his middle finger was inadvertently pinched between the one he was handling and a stationary piece of material resulting in a fractured fingertip, two sutures, and removal of the fingernail.

While injured employee (IE) pushed a handle on a diverter at the head of a quarry conveyor he felt a very sharp pain in his left calf muscle. IE was transported to the occ. health clinic by his safety mgr and was treated for a strain/sprain/fracture. Follow-up visit diagnosed IE with a strain/partial muscle tendon tear. IE then sought treatment by his PCP and is awaiting a MRI.

Employee sought medical treatment for shoulder pain on 10/24/12 and attributes it to minor pain he felt in his shoulder while moving stock onto a storage rack on 09/19/12. Employee diagnosed 1 Nov 12 with right shoulder pain, rotator cuff impingement/AC joint arthritis.

E1 (injured employee) and E2 were removing an inlet probe. E1 was working it out with gloved hands and E2 was occasionally tapping it with a sledge. Their actions were working; however, coordination failed once the probe starting moving out. The tip of E1's left pinky finger got smashed between the sledge and the flange causing a fracture and laceration requiring sutures.

Welder was welding out plates that had been installed on a day tank. The work was about 15 feet up on a scaffold. Employee was descending the ladder when his foot slipped off of the ladder rung. He fell, landed on his feet and fell back. He bruised the heel on his right foot, cut his right elbow and broke his right wrist.

The injured employee was using a tool to help guide a new drag chain into place. A rough terrain fork truck was also slowly pulling the chain to align it with the next segment. The tool slipped from the employee's hands striking the left side shield on his safety glasses causing a laceration above his left eye requiring five sutures.

Employee had his left hand on a cable to guide it and was pulling it to help move the load when the cable snagged his glove and pulled his hand into the pulley. Employee received laceration and bruising on two fingers and was placed on restricted duty. Employee was released to full duty on 01/16/2012.

Employee strained his back while carrying two 5 gallon buckets of material down two flights of stairs to empty them into the dumpster outside of the lab. Employee reported injury on 12/27/2011, was seen by the doctor and returned to work on restricted duty.

While running Gap Vac to clean up material around feed belt, the hose was not sucking, so employee went to clean the hose out, uncoupled hose and got his right arm caught in 6 inch hose and bruised his arm.

Burlington Northern Railroad was doing repair work on their tracks and property when they caught the grass on fire that spread onto mine property. Emergency services were called and the fire was put out but small spot fires continued to pop up over the next hour due to the wind and dry conditions. The fires were far from plant operations.

Employee was blowing off dust from clothing when a foreign object blew into his right eye. The employee was wearing wrap around safety glasses. The employee attempted to wash out his eye at an eye wash station but was unsuccessful. Employee was seen by a doctor who observed an corneal abrasion and gave him a prescription to prevent infection.

Employee was working on replacing the bottom chute coming from the floor drag to the elevator. While pulling on one of the chain falls, he felt a pop and pain in his right shoulder. He reported the injury but did not want to be seen by a doctor. He continued to experience pain in the shoulder and on 5/3/2011 was seen by a doctor and given work restrictions.

Employee was sent to replace the blower motor for the west air slide system. He was tightening down the set screws on the taper-lock when his right hand slipped hitting an angle iron guard bracket. He cut the top side of his right hand. He was seen by a doctor where they put 5 sutures in the cut. He was put on restricted duty until 4/1/2011.

While checking for a possible plug in the posimetric feeder the unit 5 utilityman and I opened up the feeder to get it clear when the temperature spiked on the mill causing an explosion. No one was injured and there was no property damage that occurred from this incident. The explosion was contained within the coal mill.

Laceration to tip of right ring finger. One man was installing plates on upper side of beams while injured worker was guiding bolts on plates into bolt hole on beams. Threads on a bolt caught the finger of the employee and pulled it into the bolt hole causing injury to the finger. Laceration required one stitch. Employee returned to full duty the same day.

Employee was grinding a clinker sample in the impactor. When the employee bent down to retrieve the sample bucket, a piece of material fell in his right eye. The employee was wearing safety glasses. Employee received medical treatment and given prescription eye drops. He returned to work the same shift without restrictions.

Employee was cutting wizzer blades out of the separator using a welding torch when he reached out to remove a piece he passed his left arm through the flame of the torch exposing the skin between the flame retardant jacket and his glove. Injury became infected on 5/29/2010 and he received medical treatment.

After making repairs to the gate on tank #21, co-worker EE went down to the level below to jog the gate while injured EE stayed on the platform to test if the gate was operating correctly.EE stepped over the chute and tripped catching himself with his right hand on the lip of the gate at the same time it was being jogged when the gate closed on his hand.

Employee was riding in a pickup with a supervisor when they hit a bump in the road, jarring the employees lower back. Employee had continued pain in back and was seen by a doctor on 5/5/2010 and placed on work restrictions.

EE was pushing rock screens with a 375 Komatsu dozer when he began to experience problems with the dozer. Mobile mech.came to diagnose the problem. EE raised the blade put the dozer in drive,the dozer moved forward 5' and then jerked hard twice.The EE continued to push screens up the pile when his neck stiffened up. EE was seen by the doctor and given RX meds.and work restrictions.

While returning from south-side loading to inspect and repair the chain for the scope positioner, EE caught his foot on a step and tripped and fell about four steps to the ground straining his left ankle. Employee was taken to the ER where he was treated and released. Employee is on prescription pain killers and crutches.

EE was using the Vac truck to vacuum out the clinker drag pit. His right arm felt itchy. His arm was partially covered in clinker dust. Upon inspection, he noticed that the upper bicep & lower tricep were red & tender. He washed the arm - 1st Aid. DR determined that he had a 2nd degree burn from an interaction of high alkaline dust with his perspiration.

While trying to push a drag pin back into the link, a hoist was wrapped around the drag link and across the head of the pin. EE was operating the hoist applying pressure to the pin. While applying pressure, the pin moved suddenly, causing the handle of the hoist ot rotate, EE's hand impacted the side of drag housing. EE was placed on restricted duty on 5/29/09 for one day.

On 5-15-09 EE was unloading coal, bending over swinging a sledge hammer to open unloading door when he felt a pain in lower back. After continued pain, was seen by our dr 5/21/09, given pain meds & released to full work. 5/22/09 his condition worsened & he began to experience severe pain down rt leg. Seen in ER 5/23 & 5/25/09. 5/26 his work status was listed as totally disabled.

While closing elevator door on 3rd floor of HW preheat tower, EE grabbed handle to close outside elevator door. As the door came together, EE caught finger between the doors. EE went to the doctor where a splint was put on the finger.

Employee was using a sledge hammer to remove a pin from a chain link when a piece of the pin flew off and struck him in the chin. He was taken to dr and the foreign body was removed. Stitches were required. He returned to full duty the same day.

During the annual outage, employee was replacing liners in the raw mill separator. The employee started to experience soreness in his left shoulder. There was no spcific action or event that the employee can point to as the cause of the soreness. Employee was seen by the doctor on 2/5/09 and was given work restrictions.

The EE claims the injury occurred on 12/15/08 but he didn't report it until 12/29/08. The EE was running exhaust hose for the TGA. He stepped up on the first step of a ladder, lost his balance and in doing so he put his hand out to catch himself and struck his hand on a table. EE was seen by doctor and x-rays were taken revealing a bone contusion to his R little finger.

After the driver had closed the hatch on his truck, he pulled the carbis platform up. The platform did not latch and began to go back down. As it did so the driver tried to catch it and reached across and grabbed a stair. As he pulled it up, his palm was pinched between the stair and the handle, causing the driver to need ten stitches.

While removing a pin on a cable choker, the pin flipped up and pinched his finger between the lock tab and the cable. EE went to the doctor and was given sutures.

A truck mechanic was sitting on a roller stool working on a cat truck. As he rolled out from under the truck and stood up, he felt pain in the right side of his lower back. He called Health Bridge and was advised to ice it and take pain relievers. He went to the dr on 9/2/08 and was given prescription medication and work restrictions. Was released to full duty on 9/8/08

JW Electrician working in a deenergized electrical panel - he was laying on his side on the ground working on the bottom part of the panel terminating control wires. He adjusted his position and hit his side (rib) on a 2-3 inch door locking tab causing a contusion to his rib.

EE was leaving training bldg after a training session, stepped outside and slipped on snow. The area had been plowed, he states that he did not fall to the ground, but lost his footing and twisted his right knee. EE was taken the doctor to be evaluated. EE was released with restrictions and given pain meds.

EE was putting mill liners in & was trying to remove spitzers w/a hammer & chisel to make room for the last liner.Chisel kicked back, hit right lens of his approved safety glasses,causing cracks accross the lens depositing particles of safety glass into eye.EE went to ER & was given prescription and RTW with no restrictions.

Employee was picking up junk iron and loading it on forklift. The wind blew FB into right eye. He was wearing safety glasses. After work he continued to have eye discomfort, redness, and at 7:00pm that night he went to the Hospital E/R for medical treatement; had FB (dirt) removed from right eye, given rx eye drops and returned to work the following day with no restrictions.

EE was working on the analyzer chiller tube when he became overwhelmed by sulferic acid vapors & was rendered unconscious for a short period of time. Upon gaining consciousness he reported this & was taken to hospital where he had blood work & an x-ray done. Oxygen therapy was given & he was released at 2:45 am. There were no restrictions given & ee was released to full duty.

The wind blew material into his right eye. Employee attempted to flush material from his eye but was unsuccessful. He was taken to the doctor where he was given prescription eye drops. He did not miss work and was released to full duty on same day.

On 9/18/07 the employee got a foreign body in the right eye. He was not wearing goggles. He finished his shift & on his way home experienced pain in his right eye. He went to the ER & had particles removed from rt eye & was given Rx eye drops. A follow up appt w/optometrist revealed more particles that were removed. He was taken off work 9/19, returned to full duty 9/20.

Happened while grinding. Wind was blowing and periodic high gusts of wind. The injured party was wearing safety glasses at the time with a face shield, no specific time of injury was known.

Employee was trying to remove a cap to measure the tanks. He tried to lift off and couldn't so he kicked it a few times and tried to lift again. He then got a scrap metal bar and tried to pry it off and strained his back in the process. Employee went to the doctor on June 20th and was taken off work.

EE was walking & kneeling on uneven scaffolding while welding plates in outer cone in the raw mill separator. Claims right knee pain from working. Went to dr. on 6/25/07 & returned to full duty. After an MRI it was determined that surgery was needed. He had knee surgery on 8/10/07 at which it became reportable. RTW date restricted on 8/17/07.

Forklift operator backed into pedestrain pinning him between the forklift and a bench. Forklift operator did not look before backing and backed up at a high rate of speed. No equipment was damaged.

57 yrs old man work on kiln roll bearing heat exchangers for one hour-worker became dizzy disoriented and overheated. He went to storeroom where he became extremely weak and began turning pale. history of high pressure and currently on medication. treated by emt on site and then transported by squad when he began complaining of chest and arm pain. illness. overexposure

Climbing out of a work area - scaffold plank - slipped off jamming left knee. He has a lot of swelling around left knee.

EE was using a sledge hammer to drive pins into the sand feeder and strained his lower back. EE was seen by the company Dr that day and was given work restrictions. EE had a personal back injury within the last month and has missed several days. He was off work for his back on Feb. 13,14,19-took vac from 2/26 - 3/3 and had been seeing a chiropractor from 2/14 to DOI.

Employee was lifting the lid on top of 34-132 bucket elevator to clean the chute when he felt pain in his right shoulder. After the shoulder pain continued for a few days the employee went to the doctor and was given restrictions on 2/28/07.

Employee walked into the repair shack and slipped on a slick floor. His right foot went forward and he fell back on his left knee. No improvement was seen from home care. He went to the doctor on 1/25/07; an MRI was ordered and followed up on 2/1. He then saw a specialist on 2/14 and was given restrictions and scheduled for a left knee scope at which time it became reportable.

Employee was walking up road leading up to tower 4 when he slipped in the mud, locking up his leg and causing pain in his right calf muscle. EE went to the dr and given work restrictions.

Employee reported pain in his right shoulder from pulling up on blade lever on the TD40 dozer. dup of 220071430052-release for full duty on 1/30/07 until surgery could be schuduled. He had surgery on 3/23/07 and returned to work on restricted duty on 4/23/07. He was released to full duty on 5/18/07.

Employee was taking a section of drag chain & lifting it from the ground onto the cart. When they lowered the chain down into the care, EE's finger was pinched between the chain & a section of another, resulting in a smashed right upper section of his little finger. X-ray on 11/02/06 revealed a broken distal phalnx.

Employee strained/sprained his right shoulder removing air chambers in bag house. There are 50 blasters on this header and all were being removed, which required a lot of repetitive motion. He reported injury but did not request to go to the dr until recently. He was placed on restricted duty on 5/11/07.

Employee was installing damper liners in the #2 finish mill seperator while lying on his side getting a 50 lb. liner into position with another co-worker. EE strained his left shoulder. Injury occurred on 9/12/06 & was reported on 9/21/06. The EE chose to do home care at the time & then requested to go to the Dr. after the pain persisted. EE was given restrictions.

Employee was walking down the stairs from control room at the end of his shift. When he reached the bottom he realized he had forgotten his crusher ticket. He turned to go back up the stairs and stepped off the concrete twisting his knee. Injury was reported on 8/16/06 and employee went to the doctor on 8/21/06. An MRI was done and revealed a miniscus tear on 8/25/06.

EMPLOYEE WAS CLEANING & INSTALLING A/C UNIT FOR THE CONTROL ROOM. THE UNIT HAD TO BE LIFTED APPROX. 8'. HE STRAINED HIS LEFT ARM IN THE PROCESS & THE NEXT DAY A BURSITIS POCKET DEVELOPED ON HIS LEFT ELBOW. HE WENT TO THE DR. ON 4/24/06, AT WHICH TIME THEY DRAINED THE POCKET OF FLUID. MEDICAL ONLY.

Empoyee came into the lunchroom to clock out for the day when he complained of shortness of breath. Plant EMT's were notified and an ambulance was called.

Employee was taking the end off of the feeder when he got dust in his left eye.

EE was using a 20# sledge hammer to beat out liner bolts, when he felt a pull in his back which radiated aroung the left rib cage to the front.

Employee was lying on his right side installing cone castings in the seperator. While trying to drag one into place, he pulled or strained a muscle in his right shoulder.

Employee was going to clean dump truck windows. He stepped from concrete landing to truck wheel well. His foot slipped and he fell down side of truck. Reason for reporting: restricted duty.

EMPLOYEE WAS PULLING AN IRRIGATION DISCHARGE HOSE AND STEPPED BACK INTO A TIRE RUT AND TWISTED HIS RIGHT KNEE. EMPLOYEE DID NOT GO TO THE DOCTOR AND INJURY DID NOT BECOME REPORTABLE UNTIL 10/27/05. REASON FOR REPORTING: RESTRICTED DUTY.

Employee was holding onto an open electrical box. He moved his hand and cut his thumb, possibly on a sharp corner.

Employee was removing a car shaker from a rail car using both hands. The shaker was stuck when it came loose he lost his grip with his right hand causing the shaker to drop while still holding it with his left hand, which pulled his left shoulder.

EMPLOYEE WAS PULLING LANCE OUT OF HOT KILN AND TOOK OFF GLOVE TO ROLL UP HOSE AND LAID HAND ON HOT LANCE CAUSING BURNS TO THUMB AND FINGERS ON LEFT HAND

EMPLOYEE WAS REPAIRING HOLE IN CHUTE FROM ELEVATORS TO AIRSLIDE AND DUCT WENT INTO HIS EYES

Employee was cutting metal using an iron worker and got his finger in the machine.

EE WAS CUTTING IN THE HW COOLER, USING A CUTTING ROD, AND DIRT WENT INTO HIS EYES. REASON FOR REPORTING: PRESCRIPTION FOR EYE CARE MEDICATION.

Employee says he has a sore hand but he doesn't know how, where or when something happened. Reason for reporting: restricted duty.

COLUMN SLIDE ON EE SHOULDER, WAS PLACED ON RESTRICTED DUTY FOR 3 DAYS.

Stepped in a hole on the cooler platform. Employee received 13 stitches.

EMPLOYEE WAS UNBANDING LONG LENGTHS OF STEEL THAT WERE DRAPED OVER THE FORKS OF A FORKLIFT IN PREPARATION TO PUTTING THEM AWAY ON THE STEEL RACK. WHEN HE CUT A BAND A BAR OF STEEL SPRUNG UP STRIKING HIM IN THE NOSE.

Employee was operating a Bobcat Loader, drove over bump in road, ribs came in contact with safety bar causing bruising to ribs. Reason for reporting: Restricted duty. Employee reported the injury on 1/3/2005.

BENT DOWN TO LOOK AT TIPPING VALVE, WHEN STOOD BACK UP, KNEES GAVE OUT. BECAME MSHA REPORTABLE AFTER MRI RESULTS AND SURGERY WAS SCHEDULED 2/1/05

EMPLOYEE WAS CUTTING WIRE WITH AN UTILITY KNIFE AND AS HE CUT THE WIRE, THE KNIFE CONTACTED HIS LEFT RING AND PINKY FINGERS CAUSING LACERATIONS. REASON FOR REPORTING: STITCHES.

WHILE CHECKING THE OIL ON THE STREET SWEEPER, A WASP STUNG HIS RIGHT WRIST. REASON FOR REPORTING: MEDICAL TREATMENT. RECEIVED PAPERWORK FROM DOCTOR ON 10/7/04.

CLIMBING LADDER TO GET IN DOZER, SLIPPED ON LADDER & FELT PULL IN LEFT ARM WHEN HE HELD ON LADDER. REPORTED INJURY ON 7/22/04 FIRST SOUGHT MEDICAL TREATMENT ON 1/12/05. REASON FOR REPORTING: MEDICAL TREATMENT.

Was walking in a walkway & slipped on some mud & felt a pull in right hamstring. Ee was treated on a 1st aid basis after the report of injury. Ee continued to have pain & was eventually sent to a spine specialist on 8/30/04 where he was diagnosed with herniated disc that will need pain treatment & rehab with a chiropractor. Reason for reporting: Medical treatment starting 8/30/04.

EMPLOYEE KNELT DOWN ON SOME METAL GRADING TO ASSEMBLE A LIGHT FIXTURE. WHEN HIS LEFT KNEE CONTACTED THE GRADING, A FOREIGN OBJECT POKED HIS KNEE. REASON FOR REPORTING: RESTRICTED DUTY.

EMPLOYEE WAS STEPPING DOWN OFF A LADDER ON A RAIL CAR, WHEN HE STEPPED TO THE GROUND HE FELT A POP IN HIS LEFT FOOT. REPORTED INJURY ON 6/2/04 BUT HE DID NOT GO TO THE DOCTOR UNTIL 6/9/04. REASON FOR REPORTING: RESTRICTED DUTY.

EE WAS CLEANING UP WORK BENCH AT THE END OF THE DAY WHEN HE SWUNG HIS HAND PAST A VICE THAT CONTAINED CUT CONDUIT, CUTTING HAND ON CONDUIT. REASON FOR REPORTING: STITCHES.

Employee was lifting a bucket of antifreeze to fill a tank and he felt a sharp pain in his back. Reason for reporting: Medical treatment/restricted duty.

Working on knees cleaning under a conveyer belt, left knee is sore. Reason for reporting: physical therapy for knee, first went for treatment for knee on 5-10-04.

EE WAS CLIMBING DOWN OUT OF THE FUEL TRUCK AND WHEN HE STEPPED DOWN TO THE GROUND, HE STEPPED ON A FROZEN DIRT CLOD AND TWISTED HIS KNEE. REASON FOR REPORTING: BECAME REPORTABLE WHEN EE CONTINUED HAVING PAIN IN KNEE AND STARTED PHYSICAL THERAPY 2/24/04.

EE WAS WALKING OUTSIDE ALONGSIDE THE TRAINING BUILDING WHEN HE SLIPPED ON A PATCH OF ICE. EE DID NOT FALL BUT TWISTED HIS BACK. REASON FOR REPORTING: MEDICAL TREATMENT.

EMPLOYEE WAS CARRYING MULTIPLE 55 POUND BOXES, ONE AT A TIME, IN THE QUARRY. FELT THE STRAIN IN HIS LOWER BACK THE NEXT DAY. REASON FOR REPORTING: MEDICAL TREATMENT.

COMING DOWN A FOUR STEP LADDER, HE TWISTED HIS LOWER BACK WHEN HE STEPPED DOWN TO THE FLOOR LEVEL. REASON FOR REPORTING: MEDICAL TREATMENT.

REPETITIVE PUSHING & PULLING, HE FELT MORE PAIN WHILE USING A SLEDGE HAMMER. WAS REPORTED FIRST ON 03/18/03. WENT TO A DOCTOR FOR THE INJURY ON 10/15/03. REASON FOR REPORTING: RESTRICTED DUTY.

EMPLOYEE WAS CLIMBING A LADDER IN THE PACKHOUSE. AS HE WAS CLIMBING, HE FELT A POP ON THE TOP OF HIS LEFT FOOT.

GREASING A MOTOR BEARING, GREASE GN SLIPPED OFF THE GREASE FITTING, EMPLOYEE'S HAND HIT THE COUPLER.

REPAIRMAN WAS WORKING INSIDE THE COAL MILL ON HIS KNEES, HE WAS LIFTING SECTIONS OF THE DIE RING AND HE FELT A MUSCLE PULL IN HIS ABDOMEN. IT WAS LATER DIAGNOSED AS A HERNIA.

IN THE PROCESS OF OPENING THE MILL DOOR, HE WAS PICKING UP A HEAVY BAR/BLOCK AND HE FELT AND HEARD HIS RIGHT HAMSTRING POP.

OPENING THE DOOR ON THE RAW MILL SEPARATOR WAS STILL MOVE, THE SEPARATOR CAUGHT THE DOOR HE WAS HOLDING AND THE DOOR WAS THROWN INTO HIS LEG CAUSING A LACERATION ON HIS LEFT LEG, INNER THIGH.

EE WAS GETTING OUT OF THE BACK SEAT OF CREW CAB PICKUP AND HE GOT HIS FOOT STUCK BETWEEN THE DOOR FRAME AND THE SEAT AND TWISTED HIS KNEE.

EE REPORTED PAIN IN BOTH WRISTS. HE DOES NOT KNOW THE CAUSE.

EE WAS PULLING A CABLE AND STRAINED HIS BACK.

EE WAS ATTEMPTING TO UNPLUG A VALVE & HOT DUST CAME OUT OF ACCESS PANEL & BURNED HIM. REASON FOR REPORTING: LOST TIME

EE WAS CLEANING A DRAIN AND HE TWISTED HIS KNEE WHEN PICKING UP THE DRAIN HOSE.

EMPLOYEE WAS CLIMBING A LADDER WHEN WORKING ON A WEIGHFEEDER AND STRAINED HIS BACK.

EE WAS LOOKING AT SOMETHING IN THE PLANT. HIS FOOT SLIPPED OFF THE EDGE OF A STEP AND WENT DOWN ABOUT 12" TO THE LEVEL BELOW INJURING HIS ANKLE.

EE WAS REPLACING AIRSLIDE AND SMASHED HIS FINGERBETWEEN AIRSLIDE AND HAND RAIL.

EE WAS TAKING DOORS OFF RAW MILL SYMETRO & FINISH MILL WITH HAMMER CHISEL & NOTICED TENDERNESS IN RIGHT ARM. REASON FOR REPORTING: RESTRICTED DUTY. FIRST MEDICAL TREATMENT 4-23-02.

EE WAS WORKING ON DIVISION HEAD OF FINISH MILL. HE STEPPED OFF OF LADDER INTO MILL AND STEPPED ON A GRINDING BALL. HE TRIED TO CATCH HIMSELF FROM FALLING AND PULLED RIGHT SHOULDER.

EE WAS CLIMBING DOWN A SCAFFOLD, HIS FOOT SLIPPED AND HE GRABBED THE SCAFFOLD TO HANG ON. THE SCAFFOLD CAME APART AND SWUNG HIM OFF, HE FELL 6-7'. FIRST MEDICAL TRMT. 3/12/02.

EMPLOYEE STRAINED HIS RIGHT SHOULDER WHILE REACHING ACROSS THE CAB OF A TRUCK TO SHUT THE DOOR. ON 4/28/03 HE WAS SENT BACK TO DOCTOR BECAUSE HE WAS HAVING PAIN IN SAME SHOULDER. THE DOCTOR PU T HIM ON RESTRICTED DUTY BECAUSE OF STRAIN TO RIGHT SHOULDER.

EE WAS WORKING ON A DUST COLLECTOR AND HIS BACK STARTED TO HURT. FIRST MEDICAL TREATMENT 1/11/02& RESTRICTED DUTY.

EE WAS LIFTING LINERS INTO A MILL AND HIS LT. ARM AND ELBOW BECAME SORE. FIRST MEDICAL TREATMENT 12-10-01. REASON FOR REPORTING: RESTRICTED DUTY.
