Removing front axle housing to replace ball joints using hammer to drive housing off. Hit ball joint at an angle causing hammer to hit top of right hand, pinching the palm against the housing. First aid administered on site. Injury appeared to be minor in nature. Sought medical care after shift. Released for full duty, no work restrictions. Returned to work on next scheduled shift.

Winter conditions. Contract employee slipped and hit head. EE walked to the supervisor to report the fall. There was no accident witness. Took employee to the hospital. EE suffered a severe concussion, cerebral hemorrhage and a skull fracture. Contractor filed 7000-1. Late submission due to not knowing we had to file a report as well.

Miner was using a scaling bar to knock small rocks through the welded wire mesh. He bent over to pick up the scaling bar and noticed pain in his lower back.
