Employee had an accident yesterday while ee was assembling a electrical cord. EE was using a screwdriver to tighten a screw when the screwdriver slipped and punctured hand. EE immediately cleaned it and applied a dressing, but today hand is quite swollen.

While operating a dozer the track slid on a boulder causing the operator to hit the left side window of the cab.

While loading blast holes employee slipped into wet and muddy hole and scraped his shin. The injury was not bleeding and the employee did not seek first aid treatment. Eight days later the employee went to doctor because of pain and swelling.

Employee got out of truck to unhook trailer from pickup. When employee walked around trailer, he slipped on a sheet of ice under three inches of new snow. The combination of twisting and falling resulted in a fractured left leg.

EE STATED THIS WAS RELATED TO PREVIOUSLY WORK RELATED INJURY IN 1988 AND IS DUE TO LIFTING AND MOVING HEAVY OBJECTS. WE ARE REQUESTING A DETERMINATION IF THIS IS A NEW INJURY OR CONTINUATION OF PREVIOUS INJURY.
