Employee was tracking plant then began changing screens. EE felt something in EE's left eye, flushed it out but it continued to bother EE. Supervisor took EE to Urgent Care where they flushed EE's eye and issued a prescription ointment.

Employee was removing the holder for the laptop in loader #23.600668 when they cut their left hand on a sharp piece of material. Employee went to the clinic where they received three stitches.

Exact date of injury is unknown. Employee was using a welder when sparks penetrated through the long sleeve of EE's right arm. Employee cleaned the arm but pain and swelling increased. Employee went to doctor on 06/27/2020 and was diagnosed with cellulitis.

Employee injured leg by blunt force trauma causing 8 stitches. While attempting to climb onto a flat bed trailer to tie down equipment, the employee struck right shin on the corner of the trailer causing the injury. EE was released to return to work with no restrictions after the stitches were administered. Follow up with physician in 7 days for stitches removal.
